Impact of HIV/AIDS Education on HIV Testing Uptake among University Students in Nigeria

Abstract

Human immunodeficiency virus (HIV) infection remains an important public health concern in Nigeria, despite advances in prevention, diagnosis, treatment, and care. Early knowledge of HIV status through voluntary HIV testing can facilitate timely linkage to appropriate care and treatment for individuals who test positive and support preventive decision-making among those who test negative. University students represent an important population for HIV prevention because young adulthood may involve increased social and sexual exposure, experimentation, peer influence, and changes in health-seeking behaviour. HIV/AIDS education is an important health promotion strategy for providing students with accurate information about HIV transmission, prevention, testing, treatment, stigma, and available HIV services. In Nigeria, misconceptions about HIV transmission, fear of a positive test result, stigma, concerns about confidentiality, perceived low personal risk, limited knowledge of testing services, and inadequate access to convenient testing facilities may discourage students from utilizing HIV testing services. Effective HIV/AIDS education may improve knowledge, correct misconceptions, reduce stigma, increase perceived benefits of testing, and encourage students to know their HIV status. Against this background, this study investigates the impact of HIV/AIDS education on HIV testing uptake among university students in Nigeria. The study is anchored on the Health Belief Model (HBM), Theory of Planned Behavior (TPB), and Social Cognitive Theory (SCT). The Health Belief Model explains preventive health behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding students' decisions to undertake HIV testing. The Theory of Planned Behavior proposes that attitudes, subjective norms, and perceived behavioural control influence intentions and subsequent health behaviours, including HIV testing. Social Cognitive Theory emphasizes the interaction between knowledge, personal beliefs, self-efficacy, social influences, and environmental factors in shaping health behaviours. Collectively, these theoretical perspectives provide a comprehensive framework for explaining how HIV/AIDS education may influence HIV testing uptake among university students in Nigeria. The study adopts a quantitative quasi-experimental research design. Primary data will be collected from undergraduate students in selected universities in Nigeria using a structured, anonymous, self-administered questionnaire. A multistage sampling technique will be employed to select universities, faculties, departments, classes, and eligible students. Where feasible, selected groups or institutions will be assigned to intervention and comparison groups. Students in the intervention group will receive structured HIV/AIDS education covering HIV transmission, prevention, risk factors, testing procedures, benefits of knowing one's HIV status, treatment and prevention options, stigma reduction, confidentiality, and locations of available HIV testing services. HIV testing uptake will be assessed using indicators such as previous HIV testing, testing within a specified recent period, willingness to undertake testing, intention to test, and actual utilization of available HIV testing services following the intervention. Where testing services are provided as part of the study, participation will remain voluntary and appropriate counselling, confidentiality, informed consent, and referral procedures will be maintained. Descriptive statistics will be used to summarize students' sociodemographic characteristics, HIV/AIDS knowledge, exposure to education, and HIV testing patterns. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple logistic regression analysis, will be employed to determine the impact of HIV/AIDS education on HIV testing uptake. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and the robustness of the findings. The study anticipates that HIV/AIDS education will have a significant positive impact on HIV testing uptake among university students in Nigeria. Students exposed to accurate and comprehensive HIV/AIDS education are expected to demonstrate improved knowledge of HIV transmission and prevention and a greater understanding of the benefits of knowing their HIV status. Education may reduce misconceptions and HIV-related stigma, improve perceived susceptibility and benefits of testing, and encourage students to utilize available HIV testing services. However, education alone may not guarantee testing uptake because students may continue to experience barriers such as fear of a positive result, concerns about confidentiality, stigma, inconvenient service locations, limited testing hours, and perceived low risk of HIV infection. Consequently, HIV/AIDS education is expected to be most effective when combined with confidential, youth-friendly, accessible, and convenient HIV testing services. This study is expected to contribute to the literature on HIV prevention, health education, adolescent and young adult health, HIV testing, and public health in Nigeria by providing empirical evidence on the impact of HIV/AIDS education on HIV testing uptake among university students. The findings will provide useful information for the Federal Ministry of Health and Social Welfare, National Agency for the Control of AIDS (NACA), Nigeria Centre for Disease Control and Prevention (NCDC), university health centres, university administrators, student health services, community health organizations, development partners, and policymakers regarding strategies for increasing HIV testing among young adults. The study will also provide evidence-based recommendations for strengthening university-based HIV/AIDS education, improving access to confidential HIV testing services, integrating HIV testing promotion into student health programmes, reducing HIV-related stigma and misinformation, strengthening peer-led health education, and expanding youth-friendly HIV prevention and testing services across Nigerian universities.
