Effect of Antenatal Oral Health Screening on Early Detection of Dental Problems among Pregnant Women in Nigeria

Abstract

Oral health is an important component of maternal health, and physiological and hormonal changes during pregnancy may increase women's susceptibility to dental and periodontal problems. Pregnant women may experience conditions such as gingivitis, dental caries, periodontal disease, tooth sensitivity, tooth pain, and other oral health problems that may affect their comfort, nutrition, quality of life, and general wellbeing. Despite the importance of oral health during pregnancy, many dental problems may remain undetected because oral health assessment is not routinely integrated into antenatal care in many settings. Limited awareness, financial barriers, fear of dental treatment, misconceptions about dental care during pregnancy, inadequate access to dental professionals, and weak referral systems may further contribute to delayed detection and treatment. Antenatal oral health screening provides an opportunity to identify dental problems during routine maternal healthcare visits and facilitate timely counselling, referral, diagnosis, and treatment. Against this background, this study investigates the effect of antenatal oral health screening on early detection of dental problems among pregnant women in Nigeria. The study will be anchored on the Health Belief Model, Andersen's Behavioral Model of Health Services Use, and the Social Ecological Model. The Health Belief Model explains how pregnant women's perceptions of susceptibility to oral health problems, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in antenatal oral health screening. Andersen's Behavioral Model explains how predisposing characteristics, enabling resources, and perceived healthcare needs influence women's utilization of oral health services. The Social Ecological Model emphasizes the interaction between individual, family, community, healthcare-system, and broader environmental factors in determining access to and utilization of oral healthcare services. Collectively, these theoretical perspectives provide a suitable framework for explaining how antenatal oral health screening may influence early detection of dental problems among pregnant women in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise pregnant women attending selected antenatal clinics, primary healthcare centres, hospitals, maternity clinics, and maternal health programmes across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, and eligible pregnant women. Antenatal oral health screening will be assessed using indicators such as availability of screening within antenatal services, screening coverage, frequency of screening, use of standardized oral health assessment procedures, availability of trained healthcare or oral health personnel, dental examination, assessment of gingival and periodontal health, assessment of dental caries, evaluation of tooth pain or sensitivity, oral health counselling, referral arrangements, and follow-up services. Early detection of dental problems will be assessed using indicators such as newly identified dental caries, gingivitis, periodontal problems, tooth pain, oral infections, tooth sensitivity, other clinically identifiable dental conditions, suspected abnormalities requiring further assessment, referral for professional dental evaluation, and confirmed dental diagnoses where appropriate. Data will be collected using structured questionnaires, standardized oral health screening forms, antenatal records, dental examination records, referral registers, and relevant healthcare documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, oral health findings, and patterns of early dental problem detection. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of antenatal oral health screening on early detection of dental problems. Where a quasi-experimental design is adopted, detection rates before and after implementation of antenatal oral health screening may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that antenatal oral health screening has a significant positive effect on the early detection of dental problems among pregnant women in Nigeria. Pregnant women who receive oral health screening as part of antenatal care are expected to have a higher likelihood of previously undetected dental problems being identified than women without access to routine screening. Integrating oral health assessment into antenatal services may facilitate early identification of gingivitis, dental caries, periodontal problems, tooth pain, and other oral health conditions and create opportunities for timely counselling, professional dental evaluation, appropriate treatment, and follow-up. Antenatal screening may also reduce barriers associated with making separate dental visits and improve coordination between maternal healthcare providers and dental professionals. However, inadequate availability of oral health personnel, limited screening equipment, financial constraints, fear of dental treatment, misconceptions regarding dental care during pregnancy, and weak referral systems may reduce the effectiveness of antenatal oral health screening. The study therefore expects routine, accessible, standardized, and well-coordinated antenatal oral health screening to contribute significantly to improved early detection of dental problems among pregnant women in Nigeria. The study is expected to contribute to the literature on antenatal oral health screening, early detection of dental problems, maternal oral health, pregnancy-related oral health, preventive dentistry, antenatal care, dental healthcare utilization, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, dental clinics, dentists, dental therapists, nurses, midwives, maternal health organizations, development partners, and policymakers regarding strategies for integrating oral healthcare into maternal health services. The study will also provide evidence-based recommendations for incorporating routine oral health screening into antenatal care, training maternal healthcare providers in basic oral health assessment, strengthening referral pathways between antenatal and dental services, improving access to dental professionals and screening resources, increasing pregnant women's awareness of oral health, and promoting timely diagnosis and treatment of dental problems during pregnancy across Nigeria.
