Effect of Clinical Handover Protocols on Continuity of Patient Care in Nigerian Hospitals

 

Abstract

Effective clinical handover is essential for maintaining continuity, safety, and quality of patient care during transitions between healthcare professionals, shifts, departments, and levels of care. In Nigerian hospitals, inadequate clinical handover may result from incomplete communication, inconsistent documentation, time constraints, high workload, staff shortages, lack of standardized procedures, and poor communication between healthcare teams. These challenges may lead to loss of important clinical information, duplication of investigations, medication errors, delays in treatment, missed clinical tasks, and interruptions in patient care. Clinical handover protocols provide standardized procedures for transferring essential patient information, treatment plans, outstanding clinical tasks, changes in patient condition, medication information, and follow-up requirements between healthcare professionals. Effective implementation of such protocols may improve communication and support continuity of patient care. Against this background, this study investigates the effect of clinical handover protocols on continuity of patient care in Nigerian hospitals. The study will be anchored on the Donabedian Model of Healthcare Quality, Systems Theory, and the Swiss Cheese Model of Patient Safety. The Donabedian Model explains healthcare quality through structure, process, and outcome, with standardized clinical handover protocols representing an important process component and continuity of patient care representing a measurable outcome. Systems Theory views the hospital as an interconnected system in which healthcare professionals, patients, clinical information, documentation, communication processes, and organizational structures interact to influence care continuity. The Swiss Cheese Model of Patient Safety explains how weaknesses in multiple layers of healthcare processes may align to produce adverse events, while standardized handover protocols can provide additional barriers against communication-related failures. Collectively, these theoretical perspectives provide a suitable framework for explaining how clinical handover protocols may influence continuity of patient care in Nigerian hospitals. The study will adopt a quantitative quasi-experimental or analytical comparative research design. The study population will comprise healthcare professionals involved in direct patient care in selected public and private hospitals in Nigeria, including doctors, nurses, pharmacists, laboratory professionals, and other relevant healthcare workers. A multistage sampling technique will be used to select states, local government areas, hospitals, clinical departments, wards, healthcare professionals, and eligible patient-care records. Clinical handover protocols will be assessed using indicators such as availability of standardized handover guidelines, use of structured handover tools, completeness of patient information transferred, documentation of clinical updates, communication of medication information, reporting of outstanding tasks, identification of changes in patient condition, shift-to-shift communication, interdepartmental handover, multidisciplinary participation, handover timing, staff training, and compliance monitoring. Continuity of patient care will be assessed using indicators such as completion of planned clinical tasks, timely continuation of treatment, medication continuity, availability of essential patient information, avoidance of duplicated investigations, timely follow-up of outstanding clinical issues, continuity across shifts and departments, referral and transfer coordination, missed clinical tasks, communication-related incidents, and patient-care delays. Data will be collected using structured questionnaires, clinical handover observation checklists, patient medical records, nursing documentation, handover sheets, electronic health records where available, incident reports, and relevant hospital documents. Descriptive statistics will be used to summarize healthcare workers' characteristics, handover practices, protocol compliance, and continuity-of-care indicators. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of clinical handover protocols on continuity of patient care. Where a quasi-experimental design is adopted, continuity-of-care indicators before and after implementation of a standardized handover protocol may be compared. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective clinical handover protocols have a significant positive effect on continuity of patient care in Nigerian hospitals. Healthcare teams using standardized handover procedures are expected to demonstrate more complete transfer of clinical information, fewer missed clinical tasks, improved medication continuity, better coordination between departments, and fewer delays associated with incomplete communication. Structured handover tools may help healthcare professionals consistently communicate patients' diagnoses, current clinical conditions, treatment plans, medication information, pending investigations, and required follow-up actions. Regular training and monitoring may further improve adherence to handover protocols and reduce communication-related patient-safety risks. However, high workload, staff shortages, inadequate documentation systems, interruptions during handover, poor communication culture, limited training, and inconsistent institutional enforcement may reduce the effectiveness of standardized handover procedures. The study therefore expects comprehensive, structured, consistently implemented, and adequately monitored clinical handover protocols to contribute significantly to improved continuity of patient care in Nigerian hospitals. The study is expected to contribute to the literature on clinical handover protocols, continuity of patient care, healthcare communication, patient safety, clinical coordination, healthcare quality, nursing practice, hospital management, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, healthcare administrators, physicians, nurses, pharmacists, allied health professionals, professional regulatory bodies, patient-safety committees, and policymakers regarding strategies for strengthening clinical communication. The study will also provide evidence-based recommendations for standardizing clinical handover procedures, implementing structured handover tools, improving healthcare-worker training, strengthening documentation, reducing interruptions during handover, integrating electronic handover systems where appropriate, monitoring protocol compliance, and establishing institutional policies that promote effective communication and continuity of patient care across Nigerian hospitals.
