Abstract

Down syndrome is a genetic condition associated with an additional copy of chromosome 21 and may influence physical development, intellectual functioning, communication, learning, and health across the lifespan. Expectant mothers in Nigeria may have limited access to accurate information about Down syndrome, prenatal screening, associated health conditions, available support services, and the importance of informed and non-stigmatizing healthcare decisions. Misconceptions, fear, stigma, limited prenatal counselling, and reliance on informal sources of health information may further affect expectant mothers' understanding of the condition. Down syndrome awareness education provides an opportunity to improve maternal knowledge, promote informed engagement with antenatal healthcare services, and reduce misconceptions surrounding Down syndrome. Against this background, this study investigates the effect of Down syndrome awareness education on knowledge of Down syndrome among expectant mothers in Nigeria. The study will be anchored on the Health Belief Model, Health Literacy Theory, and Social Ecological Model. The Health Belief Model explains how expectant mothers' perceptions of susceptibility to pregnancy-related concerns, perceived severity of Down syndrome and associated health conditions, perceived benefits of accurate information and prenatal assessment, perceived barriers, self-efficacy, and cues to action may influence their knowledge and healthcare decisions. Health Literacy Theory emphasizes mothers' ability to obtain, understand, evaluate, and apply reliable information about Down syndrome, prenatal screening, diagnosis, available healthcare services, and support resources. The Social Ecological Model highlights the influence of individual, family, healthcare, cultural, socioeconomic, and community factors on maternal knowledge and attitudes toward Down syndrome. Collectively, these theoretical perspectives provide a suitable framework for explaining how Down syndrome awareness education may influence knowledge of Down syndrome among expectant mothers in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise pregnant women aged 18 years and above attending antenatal clinics in selected public and private healthcare facilities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, healthcare facilities, antenatal clinics, and eligible expectant mothers. Down syndrome awareness education will be assessed using indicators such as exposure to educational sessions, frequency and duration of education, information about the genetic basis of Down syndrome, common physical and developmental characteristics, associated health conditions, intellectual and learning characteristics, prenatal screening, diagnostic testing, available healthcare and support services, myths and misconceptions, stigma, family support, early intervention, and sources of reliable information. Knowledge of Down syndrome will be assessed using indicators such as understanding of the genetic basis of the condition, recognition that Down syndrome is associated with chromosome 21, awareness of common developmental characteristics, knowledge of associated health conditions, understanding of available prenatal screening and diagnostic options, awareness of early intervention and support services, recognition of misconceptions and stigma, and knowledge of appropriate sources of professional information and counselling. Data will be collected using structured questionnaires, standardized Down syndrome knowledge assessment tools, scenario-based questions, antenatal education records where available, and pre-test and post-test assessments where a quasi-experimental intervention is adopted. Descriptive statistics will be used to summarize participants' demographic and obstetric characteristics, antenatal-care experiences, sources of Down syndrome information, and knowledge levels. Inferential statistical techniques, including chi-square tests, paired and independent t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of Down syndrome awareness education on knowledge of Down syndrome. Where a quasi-experimental design is adopted, knowledge scores before and after the educational intervention may be compared with those of a comparison group to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that Down syndrome awareness education has a significant positive effect on knowledge of Down syndrome among expectant mothers in Nigeria. Expectant mothers exposed to structured, evidence-based, culturally sensitive, and non-stigmatizing Down syndrome education are expected to demonstrate greater knowledge of the condition than mothers without comparable exposure. Education may improve understanding of the genetic basis of Down syndrome, common developmental characteristics, associated health conditions, prenatal screening and diagnostic options, and available support and early-intervention services. It may also correct misconceptions, reduce fear and stigma, and encourage expectant mothers to discuss prenatal concerns with qualified healthcare professionals. Improved knowledge may support informed engagement with antenatal counselling and appropriate prenatal healthcare services. However, limited access to prenatal screening, financial constraints, inadequate counselling services, cultural beliefs, misinformation, stigma, and shortages of trained healthcare professionals may reduce the effectiveness of education alone. The study therefore expects accessible, culturally appropriate, evidence-based, and sustained Down syndrome awareness education integrated into antenatal care to contribute significantly to improved knowledge of Down syndrome among expectant mothers in Nigeria. The study is expected to contribute to the literature on Down syndrome, maternal health education, antenatal care, prenatal screening, genetic health literacy, reproductive health, disability awareness, developmental health, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, antenatal clinics, obstetricians, midwives, genetic counsellors, paediatricians, public health practitioners, disability-support organizations, development partners, and policymakers regarding strategies for improving maternal genetic-health knowledge. The study will also provide evidence-based recommendations for integrating Down syndrome education into routine antenatal counselling, strengthening healthcare-provider training in genetic health communication, improving access to accurate prenatal information and screening services, reducing stigma and misconceptions, strengthening referral pathways for genetic counselling, and developing sustainable maternal and child-health programmes that promote informed and respectful care for expectant mothers and families affected by Down syndrome across Nigeria.
