Effect of Health Education on Safe Use of Herbal Medicines among Adults in Nigeria

Abstract

Herbal medicines are widely used by adults in Nigeria for the prevention and management of various health conditions and may be obtained from traditional medicine practitioners, herbal medicine vendors, pharmacies, markets, and other informal sources. Although some herbal products may have therapeutic value, inappropriate use, including incorrect dosage, prolonged self-medication, use of unidentified products, improper storage, simultaneous use with conventional medicines, and failure to seek professional healthcare when necessary, may result in adverse health outcomes. Limited knowledge of appropriate herbal medicine use and potential safety risks may increase the likelihood of unsafe practices among adults. Health education provides an opportunity to improve awareness of herbal medicine safety, appropriate use, potential adverse effects, drug-herb interactions, product quality, and the importance of seeking qualified healthcare advice. Against this background, this study investigates the effect of health education on safe use of herbal medicines among adults in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Health Literacy Theory. The Health Belief Model explains how adults' perceptions of susceptibility to adverse effects from unsafe herbal medicine use, perceived severity, perceived benefits of safe practices, perceived barriers, and cues to action may influence their use of herbal medicines. Social Cognitive Theory emphasizes observational learning, self-efficacy, behavioural modelling, reinforcement, and social and environmental influences in shaping herbal medicine-use practices. Health Literacy Theory emphasizes the ability of adults to obtain, understand, evaluate, and apply information concerning herbal medicine safety and make appropriate health decisions. Collectively, these theoretical perspectives provide a suitable framework for explaining how health education may influence the safe use of herbal medicines among adults in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adults aged 18 years and above residing in selected urban, semi-urban, and rural communities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, households, and eligible adults. Health education exposure will be assessed using indicators such as participation in herbal medicine safety education sessions, frequency and duration of education, knowledge of appropriate dosage, product identification, storage conditions, recognition of adverse effects, awareness of possible herb-drug interactions, understanding of contraindications, appropriate duration of use, awareness of product quality and regulatory information, and knowledge of when professional medical advice should be sought. Safe herbal medicine use will be assessed using indicators such as obtaining herbal products from reliable sources, appropriate dosage and frequency, correct product identification, proper storage, adherence to recommended duration of use, avoidance of inappropriate combinations with conventional medicines, awareness and monitoring of adverse reactions, avoidance of unidentified or contaminated products, disclosure of herbal medicine use to healthcare professionals, and appropriate healthcare seeking when symptoms persist or adverse effects occur. Data will be collected using structured questionnaires, herbal medicine safety knowledge assessment instruments, safe-use practice tools, and relevant community or healthcare records where available. Descriptive statistics will be used to summarize participants' demographic characteristics, patterns of herbal medicine use, sources of herbal products, exposure to health education, and safe-use practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of health education on safe herbal medicine use. Where a quasi-experimental design is adopted, safe-use practice scores before and after the educational intervention may be compared with those of a comparison group to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that health education has a significant positive effect on the safe use of herbal medicines among adults in Nigeria. Adults exposed to structured and evidence-based herbal medicine safety education are expected to demonstrate safer practices than adults without comparable exposure. Education may improve awareness of appropriate dosage, product identification, storage, possible adverse effects, herb-drug interactions, contraindications, and the importance of seeking professional healthcare advice when necessary. Improved knowledge may also encourage adults to disclose herbal medicine use to healthcare professionals and avoid combining multiple herbal or conventional medicines without appropriate guidance. However, cultural beliefs, traditional practices, easy availability of herbal products, perceived affordability, limited regulation or product information, low health literacy, and reliance on informal sources of health information may reduce the effectiveness of education alone. The study therefore expects accessible, culturally appropriate, practical, and continuous herbal medicine safety education, supported by effective regulation and professional guidance, to contribute significantly to improved safe-use practices among adults in Nigeria. The study is expected to contribute to the literature on herbal medicine safety, traditional medicine, health education, health literacy, self-medication, herb-drug interactions, pharmacovigilance, health promotion, complementary and alternative medicine, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Agency for Food and Drug Administration and Control, traditional medicine regulatory bodies, healthcare professionals, pharmacists, medical practitioners, community health workers, public health practitioners, traditional medicine practitioners, development partners, and policymakers regarding strategies for promoting safer herbal medicine use. The study will also provide evidence-based recommendations for strengthening herbal medicine safety education, improving public awareness of potential adverse effects and herb-drug interactions, promoting appropriate product identification and storage, encouraging communication between patients and healthcare professionals, strengthening pharmacovigilance and regulatory systems, and developing sustainable interventions for safer use of herbal medicines among adults across Nigeria.
