Effect of Maternal Oral Health Education on Oral Hygiene Practices among Pregnant Women in Nigeria

Abstract

Maternal oral health is an important component of antenatal and overall healthcare because pregnancy is associated with physiological and hormonal changes that may influence oral health. Pregnant women may experience increased susceptibility to gingival inflammation, dental caries, tooth sensitivity, and other oral health problems. Poor oral hygiene during pregnancy may result from inadequate knowledge, misconceptions about dental care, fear of dental treatment, limited access to oral healthcare services, financial constraints, and the belief that dental treatment should be avoided during pregnancy. Maternal oral health education provides an opportunity to improve pregnant women's knowledge of oral hygiene, preventive dental care, appropriate dietary practices, recognition of oral health problems, and the importance of seeking professional dental care when necessary. Against this background, this study investigates the effect of maternal oral health education on oral hygiene practices among pregnant women in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Self-Efficacy Theory. The Health Belief Model explains how pregnant women's perceptions of susceptibility to oral health problems, perceived severity, perceived benefits of good oral hygiene, perceived barriers, and cues to action may influence their oral hygiene practices. Social Cognitive Theory emphasizes observational learning, social support, behavioural reinforcement, environmental influences, and self-regulation in the adoption of healthy oral hygiene behaviours. Self-Efficacy Theory emphasizes women's confidence in their ability to perform appropriate oral hygiene practices consistently and seek professional dental care when required. Collectively, these theoretical perspectives provide a suitable framework for explaining how maternal oral health education may influence oral hygiene practices among pregnant women in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise pregnant women attending selected antenatal clinics, primary healthcare centres, hospitals, maternity clinics, and community maternal health programmes across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, and eligible pregnant women. Maternal oral health education will be assessed using indicators such as exposure to structured oral health education, frequency and duration of education sessions, information on tooth brushing, appropriate toothpaste use, interdental cleaning, dietary practices, oral health during pregnancy, recognition of gingival and dental problems, professional dental visits, safety of appropriate dental care during pregnancy, and referral for dental assessment. Oral hygiene practices will be assessed using indicators such as frequency and technique of tooth brushing, use of fluoride-containing toothpaste where appropriate, interdental cleaning, reduction of frequent sugary food and drink consumption, regular dental attendance, response to oral health symptoms, and appropriate utilization of professional oral healthcare services. Data will be collected using structured questionnaires, oral hygiene assessment instruments, antenatal records, dental referral records, and relevant healthcare documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to maternal oral health education, and patterns of oral hygiene practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of maternal oral health education on oral hygiene practices. Where a quasi-experimental design is adopted, oral hygiene practice scores before and after the educational intervention will be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that maternal oral health education has a significant positive effect on oral hygiene practices among pregnant women in Nigeria. Pregnant women exposed to structured and practical oral health education are expected to demonstrate improved tooth-brushing practices, better use of appropriate oral hygiene products, greater attention to dietary habits, increased awareness of oral health problems, and more appropriate utilization of professional dental services. Education may correct misconceptions about dental care during pregnancy and encourage women to seek timely dental evaluation rather than postponing care because of fear or misinformation. Integrating oral health education into antenatal care may also provide a convenient opportunity for healthcare professionals to promote preventive oral health practices and referrals. However, financial constraints, limited access to dental services, shortage of oral health professionals, fear of dental treatment, cultural beliefs, and inadequate integration of oral healthcare into maternal health services may limit the effectiveness of education. The study therefore expects accessible, practical, culturally appropriate, and continuous maternal oral health education to contribute significantly to improved oral hygiene practices among pregnant women in Nigeria. The study is expected to contribute to the literature on maternal oral health education, oral hygiene practices, pregnancy-related oral health, antenatal care, preventive dental care, maternal health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, dental clinics, dentists, dental therapists, nurses, midwives, community health workers, maternal health organizations, development partners, and policymakers regarding strategies for improving oral health during pregnancy. The study will also provide evidence-based recommendations for integrating oral health education into antenatal care, strengthening referral pathways between maternal and dental services, training maternal healthcare providers in basic oral health promotion, increasing access to appropriate dental care during pregnancy, developing culturally appropriate educational materials, and promoting good oral hygiene practices among pregnant women across Nigeria.
