Impact of Antenatal Anaemia Management on Maternal Anaemia among Pregnant Women in Nigeria
Abstract
Maternal anaemia remains a major public health and maternal health concern in Nigeria and is associated with increased risks of maternal morbidity, poor pregnancy outcomes, preterm birth, low birth weight, postpartum complications, and maternal mortality. Anaemia during pregnancy may result from iron and folate deficiencies, malaria, parasitic infections, nutritional inadequacies, chronic diseases, and other maternal health conditions. Antenatal anaemia management provides an important opportunity for early identification, prevention, treatment, and monitoring of anaemia throughout pregnancy. Effective management may include routine haemoglobin assessment, iron and folic acid supplementation, appropriate treatment of malaria and other infections, nutritional counselling, follow-up testing, and referral of women with severe or persistent anaemia. However, inadequate antenatal attendance, limited laboratory capacity, poor adherence to supplementation, shortages of essential medicines, delayed diagnosis, and inadequate follow-up may reduce the effectiveness of anaemia management services in Nigeria. Against this background, this study investigates the impact of antenatal anaemia management on maternal anaemia among pregnant women in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Health Systems Framework. The Health Belief Model explains how pregnant women's perceptions of susceptibility to anaemia, perceived severity, perceived benefits of screening and treatment, perceived barriers, and cues to action may influence participation in antenatal anaemia management. The Social Ecological Model emphasizes the influence of individual, household, community, healthcare, nutritional, socioeconomic, and environmental factors on maternal anaemia and healthcare utilization. The Health Systems Framework emphasizes antenatal service delivery, health workforce, diagnostic capacity, availability of medicines and supplements, health information, financing, governance, and accessibility as essential components of effective maternal anaemia management. Collectively, these theoretical perspectives provide a suitable framework for explaining how antenatal anaemia management may influence maternal anaemia among pregnant women in Nigeria. The study will adopt a quantitative analytical cohort, retrospective cohort, or quasi-experimental research design. The study population will comprise pregnant women receiving antenatal care in selected healthcare facilities across Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, antenatal clinics, and eligible pregnant women. Antenatal anaemia management will be assessed using indicators such as routine haemoglobin screening, timing and frequency of anaemia assessment, iron and folic acid supplementation, adherence monitoring, nutritional counselling, malaria prevention and treatment, diagnosis and management of parasitic infections where appropriate, repeat haemoglobin testing, clinical follow-up, referral of severe cases, and documentation of treatment. Maternal anaemia will be assessed using haemoglobin concentration and documented anaemia status at different stages of pregnancy, with severity classified according to applicable clinical guidelines. Data will be collected using structured questionnaires, antenatal care records, maternal medical records, laboratory registers, haemoglobin monitoring charts, prescription and medication records, and referral documents. Descriptive statistics will be used to summarize maternal characteristics, anaemia prevalence, screening coverage, treatment patterns, and changes in haemoglobin levels. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of antenatal anaemia management on maternal anaemia. Where a cohort or quasi-experimental design is adopted, maternal anaemia outcomes among women receiving comprehensive antenatal anaemia management may be compared with those among women receiving inadequate management, while relevant maternal, nutritional, socioeconomic, and pregnancy-related factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective antenatal anaemia management has a significant positive impact on reducing maternal anaemia among pregnant women in Nigeria. Pregnant women who receive timely screening, appropriate iron and folic acid supplementation, nutritional counselling, treatment of underlying infections, and regular follow-up are expected to have improved haemoglobin levels and a lower likelihood of persistent or severe anaemia compared with women receiving inadequate management. Early identification may allow healthcare providers to address anaemia before it progresses and reduce associated maternal and pregnancy-related complications. However, poor adherence to supplementation, inadequate dietary intake, malaria, parasitic infections, limited access to laboratory services, shortages of medicines and supplements, late antenatal care initiation, and weak follow-up systems may reduce the effectiveness of antenatal anaemia management. The study therefore expects comprehensive, timely, accessible, and appropriately monitored antenatal anaemia management to contribute significantly to reducing maternal anaemia in Nigeria. The study is expected to contribute to the literature on antenatal anaemia management, maternal anaemia, iron-deficiency anaemia, maternal nutrition, antenatal care, pregnancy outcomes, maternal health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, healthcare facilities, maternal health programmes, obstetricians, midwives, nurses, nutritionists, community health workers, development partners, and policymakers regarding strategies for strengthening maternal anaemia prevention and treatment. The study will also provide evidence-based recommendations for improving routine anaemia screening, strengthening access to laboratory services, improving availability and adherence to iron and folic acid supplementation, strengthening nutritional counselling, improving malaria and parasitic infection management, enhancing follow-up and referral systems, and integrating comprehensive anaemia management into broader antenatal and maternal health programmes across Nigeria.
