Impact of Asthma First-Aid Training on Emergency Response Skills among Secondary School Teachers in Nigeria

Abstract

Asthma is a common chronic respiratory condition that may cause sudden episodes of breathing difficulty, wheezing, coughing, chest tightness, and other symptoms that can require prompt intervention. Secondary school students living with asthma may experience acute attacks during school hours, placing teachers in a position to provide immediate assistance while awaiting professional medical care. However, many teachers may have limited knowledge and practical skills regarding asthma first aid, recognition of worsening symptoms, appropriate use of prescribed reliever medication or inhaler devices, positioning, reassurance, emergency communication, and timely referral. Inadequate first-aid response may increase the risk of complications and delayed access to appropriate healthcare. Asthma first-aid training provides an opportunity to equip teachers with the knowledge and practical skills required to respond appropriately during asthma emergencies. Against this background, this study investigates the impact of asthma first-aid training on emergency response skills among secondary school teachers in Nigeria. The study will be anchored on Social Cognitive Theory, Health Belief Model, and the Theory of Planned Behavior. Social Cognitive Theory emphasizes observational learning, practical skill development, self-efficacy, reinforcement, and environmental influences in shaping teachers' emergency-response behaviours. The Health Belief Model explains how teachers' perceptions of the severity of asthma attacks, perceived benefits of appropriate first aid, perceived barriers, self-efficacy, and cues to action may influence their emergency responses. The Theory of Planned Behavior emphasizes attitudes toward asthma first aid, subjective norms, perceived behavioural control, and behavioural intentions as determinants of emergency-response practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how asthma first-aid training may influence emergency response skills among secondary school teachers in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise teachers employed in selected public and private secondary schools across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, schools, teaching departments, and eligible teachers. Asthma first-aid training will be assessed using indicators such as exposure to structured training sessions, frequency and duration of training, recognition of asthma symptoms and warning signs, identification of severe attacks, appropriate first-aid procedures, correct use of prescribed reliever inhalers or spacers where applicable, appropriate positioning, reassurance and communication, removal from known triggers where safe, emergency contact procedures, timely referral, and knowledge of school asthma-management protocols. Emergency response skills will be assessed using indicators such as recognition of an asthma emergency, assessment of symptom severity, appropriate immediate response, correct assistance with a student's prescribed reliever medication, appropriate positioning, monitoring of symptoms, activation of emergency services, communication with parents or designated contacts, appropriate referral, and avoidance of potentially harmful responses. Data will be collected using structured questionnaires, standardized asthma first-aid knowledge assessment tools, practical skills checklists, simulated asthma-emergency scenarios, direct observation, and pre-test and post-test assessments where a quasi-experimental intervention is adopted. Descriptive statistics will be used to summarize teachers' demographic and professional characteristics, teaching experience, previous first-aid training, experience with students living with asthma, and baseline emergency-response skills. Inferential statistical techniques, including chi-square tests, paired and independent t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of asthma first-aid training on emergency response skills. Where a quasi-experimental design is adopted, emergency-response skill scores before and after the training intervention may be compared with those of a comparison group to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that asthma first-aid training has a significant positive impact on emergency response skills among secondary school teachers in Nigeria. Teachers exposed to structured, practical, and scenario-based asthma first-aid training are expected to demonstrate better recognition of asthma emergencies and more appropriate immediate responses than teachers without comparable training. Training may improve teachers' ability to identify worsening symptoms, assist students with their prescribed reliever medication appropriately, position and reassure affected students, monitor symptoms, activate emergency assistance, and arrange timely referral. Practical demonstrations and simulated emergencies may strengthen teachers' confidence and reduce hesitation during asthma attacks. Training may also help teachers avoid inappropriate interventions that could delay effective treatment. However, inadequate access to training, limited availability of school asthma-management plans, insufficient first-aid equipment, lack of reliever medication or spacers when prescribed and available, limited emergency referral systems, and inadequate collaboration between schools, parents, and healthcare providers may reduce the effectiveness of training alone. The study therefore expects regular, practical, school-based, and competency-focused asthma first-aid training, supported by appropriate school health policies and emergency-response systems, to contribute significantly to improved emergency response skills among secondary school teachers in Nigeria. The study is expected to contribute to the literature on asthma first aid, emergency response skills, teacher health education, school health, respiratory health, chronic disease management, adolescent health, emergency preparedness, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, state ministries of health and education, secondary schools, school health personnel, healthcare professionals, asthma and respiratory health organizations, parents, development partners, and policymakers regarding strategies for improving asthma emergency preparedness in schools. The study will also provide evidence-based recommendations for integrating asthma first-aid training into teacher training and professional development programmes, developing school asthma-management protocols, strengthening collaboration between schools and healthcare providers, improving access to practical emergency training, and implementing sustainable school-based interventions that enhance teachers' ability to respond effectively to asthma emergencies across Nigeria.
