Impact of Blood Donation Education on Knowledge of Voluntary Blood Donation among Secondary School Students in Nigeria

 

Abstract

Voluntary blood donation is an important component of a safe and sustainable blood supply because donated blood and blood components are needed for patients experiencing severe blood loss, complications of childbirth, major surgery, trauma, severe anaemia, and other medical emergencies. Developing accurate knowledge and positive attitudes toward voluntary blood donation among young people may contribute to future participation in safe blood-donation programmes. Secondary school students in Nigeria may have limited knowledge about blood donation eligibility, donor safety, donation procedures, blood-group compatibility, temporary side effects, and the importance of voluntary, non-remunerated donation. Misconceptions, fear of needles, concerns about weakness, cultural beliefs, misinformation, and limited exposure to blood-donation education may further influence students' understanding of donation. Blood donation education provides an opportunity to improve students' knowledge of the purpose, eligibility, safety, procedures, and importance of voluntary blood donation. Against this background, this study investigates the impact of blood donation education on knowledge of voluntary blood donation among secondary school students in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Theory of Planned Behavior. The Health Belief Model explains how students' perceptions of the importance of blood donation, perceived benefits, perceived barriers, perceived susceptibility to situations requiring blood transfusion, self-efficacy, and cues to action may influence their knowledge and attitudes toward voluntary donation. Social Cognitive Theory emphasizes observational learning, role modelling, self-efficacy, reinforcement, peer influence, and social support in shaping students' understanding of blood donation. The Theory of Planned Behavior emphasizes attitudes, subjective norms, perceived behavioural control, and behavioural intentions as factors influencing students' views and future willingness to participate in voluntary blood donation. Collectively, these theoretical perspectives provide a suitable framework for explaining how blood donation education may influence knowledge of voluntary blood donation among secondary school students in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise secondary school students aged approximately 12–19 years enrolled in selected public and private secondary schools across Nigeria, with the specific age range determined according to the approved study protocol and ethical requirements. Because blood donation eligibility is generally restricted to individuals who meet minimum age, weight, health, and other national or programme-specific requirements, the study will focus on knowledge and future intention rather than actual blood donation among students who are not yet eligible to donate. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, schools, classes, and eligible students. Blood donation education will be assessed using indicators such as exposure to educational sessions, frequency and duration of education, information on the importance of blood donation, blood transfusion needs, donor eligibility, age and health requirements, weight requirements, donation procedures, donor safety, temporary side effects, blood-group compatibility, infection screening, voluntary non-remunerated donation, myths and misconceptions, and reliable sources of blood-donation information. Knowledge of voluntary blood donation will be assessed using indicators such as students' understanding of the purpose of blood donation, donor eligibility, basic donation procedures, donor safety, temporary reactions, blood-group concepts, voluntary donation principles, blood screening, common misconceptions, and appropriate sources of professional information. Data will be collected using structured questionnaires, standardized blood-donation knowledge assessment tools, age-appropriate scenario-based questions, school health programme records where available, and pre-test and post-test assessments where a quasi-experimental intervention is adopted. Descriptive statistics will be used to summarize students' demographic and educational characteristics, previous exposure to blood-donation information, sources of information, awareness levels, and knowledge scores. Inferential statistical techniques, including chi-square tests, paired and independent t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of blood donation education on knowledge of voluntary blood donation. Where a quasi-experimental design is adopted, knowledge scores before and after the educational intervention may be compared with those of a comparison group to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that blood donation education has a significant positive impact on knowledge of voluntary blood donation among secondary school students in Nigeria. Students exposed to structured, age-appropriate, evidence-based, and practical blood-donation education are expected to demonstrate greater knowledge of blood donation than students without comparable exposure. Education may improve students' understanding of donor eligibility, donation procedures, donor safety, blood screening, blood-group compatibility, and the importance of voluntary non-remunerated donation. It may also correct common misconceptions, such as beliefs that blood donation causes permanent weakness or significant harm to healthy eligible donors. Interactive activities, educational demonstrations, and age-appropriate discussions may improve students' confidence in discussing blood donation and encourage them to share accurate information with their families and communities. However, cultural beliefs, fear of needles, misinformation, limited exposure to blood-donation programmes, family attitudes, and concerns about donor safety may reduce the effectiveness of education alone. The study therefore expects accessible, age-appropriate, evidence-based, and sustained blood-donation education to contribute significantly to improved knowledge of voluntary blood donation among secondary school students in Nigeria and may help prepare eligible students for informed future participation in voluntary blood-donation programmes. The study is expected to contribute to the literature on voluntary blood donation, blood-donation education, adolescent health education, transfusion medicine, health literacy, school health, public health education, and blood-supply sustainability in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Blood Service Agency, Federal Ministry of Education, state ministries of health and education, secondary schools, healthcare professionals, blood-transfusion services, community health workers, adolescent-health programmes, development partners, and policymakers regarding strategies for strengthening future voluntary blood-donor recruitment. The study will also provide evidence-based recommendations for integrating age-appropriate blood-donation education into school health programmes, improving students' understanding of blood donation and donor safety, addressing myths and fears surrounding donation, strengthening collaboration between schools and blood-transfusion services, promoting voluntary non-remunerated donation, and developing sustainable school-based health education programmes that prepare young people for informed and safe participation in blood-donation programmes when they become eligible in Nigeria.
