Impact of Blood Donor Education on Voluntary Blood Donation Practices among University Students in Nigeria
Abstract
Voluntary blood donation is essential for maintaining adequate supplies of safe blood for patients requiring transfusion during obstetric emergencies, traumatic injuries, surgical procedures, severe anaemia, and other life-threatening conditions. University students represent an important potential source of voluntary blood donors because of their relatively large population and potential to become regular donors. However, misconceptions about blood donation, fear of needles, concerns about weakness or health effects, inadequate knowledge of donor eligibility and safety, and limited exposure to organized blood donation opportunities may discourage voluntary donation among students in Nigeria. Blood donor education provides an opportunity to improve students' knowledge, correct misconceptions, address concerns, and encourage informed participation in voluntary blood donation. Against this background, this study investigates the impact of blood donor education on voluntary blood donation practices among university students in Nigeria. The study will be anchored on the Health Belief Model, Theory of Planned Behavior, and Social Cognitive Theory. The Health Belief Model explains how students' perceptions of the need for blood, perceived benefits of donation, perceived barriers, perceived susceptibility to health emergencies, and cues to action may influence their willingness to donate. The Theory of Planned Behavior explains how attitudes toward blood donation, subjective norms, perceived behavioural control, and behavioural intentions may influence voluntary donation practices. Social Cognitive Theory emphasizes the roles of knowledge, observational learning, self-efficacy, reinforcement, and social environment in developing and maintaining blood donation behaviour. Collectively, these theoretical perspectives provide a suitable framework for explaining how blood donor education may influence voluntary blood donation practices among university students in Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise undergraduate students aged 18 years and above enrolled in selected universities across Nigeria. A multistage sampling technique will be used to select states, universities, faculties or departments, academic levels, and eligible students. Blood donor education will be assessed using indicators such as exposure to donor education sessions, frequency and duration of education, information on donor eligibility, blood donation procedures, donor safety, health benefits and misconceptions, importance of voluntary donation, eligibility criteria, possible adverse effects, donation intervals, and information about available blood donation centres or campaigns. Voluntary blood donation practices will be assessed using indicators such as history of voluntary blood donation, frequency of donation, first-time donation, repeat donation, registration as a voluntary donor, donation without direct replacement or personal request, willingness to donate in the future, and participation in organized blood donation campaigns. Data will be collected using structured questionnaires, blood donation records, donor registration registers, university health records, blood donation campaign records, and relevant programme documents. Descriptive statistics will be used to summarize students' characteristics, exposure to donor education, knowledge and attitudes, and blood donation practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of blood donor education on voluntary blood donation practices. Where a quasi-experimental design is adopted, blood donation knowledge, attitudes, intentions, and practices before and after exposure to structured donor education may be compared, while relevant demographic, socioeconomic, academic, and institutional factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that blood donor education has a significant positive impact on voluntary blood donation practices among university students in Nigeria. Students who receive structured and comprehensive blood donor education are expected to demonstrate better knowledge of blood donation, more favourable attitudes toward voluntary donation, greater willingness to donate, and higher participation in voluntary blood donation than students with limited exposure to donor education. Education may correct misconceptions about blood donation, reduce fear and uncertainty, improve understanding of donor eligibility and safety, and strengthen students' sense of social responsibility. However, factors such as fear of needles, concerns about adverse effects, academic schedules, lack of convenient donation sites, previous negative experiences, perceived ineligibility, and limited access to organized blood donation campaigns may continue to affect participation. The study therefore expects accessible, accurate, student-focused, and continuous blood donor education, combined with convenient opportunities for safe blood donation, to contribute significantly to improved voluntary blood donation practices among university students in Nigeria. The study is expected to contribute to the literature on blood donor education, voluntary blood donation, blood donor recruitment, repeat donation, blood safety, university health promotion, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Blood Service Agency, state blood transfusion services, universities, university health centres, student health organizations, blood banks, healthcare professionals, development partners, and policymakers regarding strategies for increasing voluntary blood donation among young adults. The study will also provide evidence-based recommendations for integrating blood donor education into university health programmes, organizing regular campus-based blood donation campaigns, strengthening student blood donor clubs, improving donor counselling, addressing misconceptions and fears, providing convenient donation opportunities, and developing strategies for encouraging first-time and repeat voluntary blood donation among university students across Nigeria.
