Impact of Community-Based Heat-Health Programmes on Heat-Protection Practices among Outdoor Workers in Nigeria

 

Abstract

Heat exposure is an emerging occupational and public health concern in Nigeria, particularly among outdoor workers who may experience prolonged exposure to high temperatures and direct sunlight during their daily activities. Outdoor workers such as farmers, construction workers, street traders, transport workers, security personnel, sanitation workers, and other labourers may be at increased risk of heat-related health problems, including heat exhaustion, heat cramps, dehydration, heat syncope, and heat stroke. Increasing temperatures, high humidity, limited access to shade and drinking water, prolonged working hours, inadequate rest periods, and limited awareness of heat-health risks may increase occupational heat exposure. Community-based heat-health programmes provide an opportunity to protect outdoor workers through heat-health education, early warning messages, hydration promotion, provision of cooling and shade facilities, recommendations for work-rest schedules, heat-risk communication, community outreach, and referral guidance. Against this background, this study investigates the impact of community-based heat-health programmes on heat-protection practices among outdoor workers in Nigeria. The study will be anchored on the Social Ecological Model, Health Belief Model, and Social Cognitive Theory. The Social Ecological Model emphasizes the interaction between individual, interpersonal, workplace, community, environmental, and policy factors in shaping heat-protection practices. The Health Belief Model explains how outdoor workers' perceptions of susceptibility to heat-related illness, perceived severity, perceived benefits of protective measures, perceived barriers, and cues to action may influence their adoption of heat-protection practices. Social Cognitive Theory emphasizes observational learning, self-efficacy, behavioural reinforcement, and environmental influences on the development and maintenance of protective behaviours. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based heat-health programmes may influence heat-protection practices among outdoor workers in Nigeria. The study will adopt a quantitative quasi-experimental or community-based comparative research design. The study population will comprise adult outdoor workers, including farmers, construction workers, street vendors, transport workers, security personnel, sanitation workers, and other occupations involving substantial outdoor exposure in selected Nigerian communities. A multistage sampling technique will be used to select states, local government areas, communities, occupational groups, worksites, and eligible outdoor workers. Community-based heat-health programmes will be assessed using indicators such as heat-health education sessions, frequency of community outreach, distribution of heat-health information, heat-warning messages, hydration education, promotion of work-rest schedules, provision or promotion of shade, cooling arrangements, heat-risk communication, first-aid education, recognition of heat-related illness, referral information, community participation, and availability of trained community health or environmental health personnel. Heat-protection practices will be assessed using indicators such as regular water consumption during work, use of appropriate clothing and protective head coverings, use of shade, scheduled rest periods, adjustment of working hours during extreme heat, use of cooling measures, recognition of heat-related symptoms, seeking assistance when symptoms occur, and avoidance of prolonged exposure during periods of extreme heat. Data will be collected using structured questionnaires, workplace observation checklists, community programme records, heat-health education materials, environmental assessments, and interviews with relevant community or occupational health personnel. Where feasible, environmental measurements such as ambient temperature and humidity may be collected to provide contextual information on heat exposure. Descriptive statistics will be used to summarize workers' demographic and occupational characteristics, exposure to heat-health programmes, environmental conditions, and heat-protection practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of community-based heat-health programmes on heat-protection practices. Where a quasi-experimental design is adopted, heat-protection practices before and after implementation of the programme may be compared, or workers in communities with active heat-health programmes may be compared with workers in similar communities without enhanced programmes. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based heat-health programmes have a significant positive impact on heat-protection practices among outdoor workers in Nigeria. Outdoor workers exposed to regular heat-health education, heat-warning information, hydration promotion, and practical guidance on work-rest schedules are expected to demonstrate greater adoption of recommended protective practices than workers with limited exposure to such programmes. Improved awareness may encourage workers to drink water regularly, seek shade, take appropriate rest periods, modify work schedules during periods of high heat, recognize early symptoms of heat-related illness, and seek timely assistance when necessary. However, socioeconomic constraints, occupational demands, lack of employer support, limited access to drinking water and shade, long working hours, inadequate community resources, and difficulty modifying work schedules may limit the adoption of protective practices. The study therefore expects accessible, culturally appropriate, regularly implemented, and occupationally relevant community-based heat-health programmes to contribute significantly to improved heat-protection practices among outdoor workers in Nigeria. The study is expected to contribute to the literature on heat-health programmes, occupational heat exposure, heat-protection practices, environmental health, climate-related health risks, occupational health, community health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Environment, Federal Ministry of Labour and Employment, Nigeria Centre for Disease Control and Prevention, state ministries of health, local government authorities, occupational health professionals, environmental health officers, employers, labour organizations, community health workers, development partners, and policymakers regarding strategies for reducing heat-related health risks among outdoor workers. The study will also provide evidence-based recommendations for strengthening community heat-health education, developing localized heat-warning systems, improving access to drinking water and shade at worksites, promoting appropriate work-rest schedules, strengthening occupational heat-safety policies, improving early recognition and management of heat-related illness, and integrating heat-health interventions into broader climate-change adaptation and occupational health programmes across Nigeria.
