Impact of Community Trachoma Screening on Early Detection of Trachoma among Children in Rural Nigeria

Abstract

Trachoma remains an important public health concern in areas where environmental conditions, limited access to water and sanitation, and inadequate hygiene practices facilitate transmission. Repeated infection with Chlamydia trachomatis may cause progressive damage to the eyelid and cornea and, when untreated, may eventually result in visual impairment and blindness. Children may play an important role in community transmission because they can experience repeated exposure and may have difficulty recognizing or reporting early eye symptoms. In rural Nigeria, limited access to eye-care services, inadequate awareness, poor sanitation, geographical barriers, and delayed healthcare-seeking may contribute to late identification and management of trachoma. Community trachoma screening provides an opportunity to identify suspected cases within communities and facilitate timely referral, treatment, and follow-up. Against this background, this study investigates the impact of community trachoma screening on early detection of trachoma among children in rural Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Andersen's Behavioral Model of Health Services Use. The Health Belief Model explains how parents', caregivers', and communities' perceptions of susceptibility to trachoma, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in community screening and subsequent healthcare-seeking. The Social Ecological Model emphasizes the interaction between children, families, schools, communities, healthcare systems, and environmental conditions in shaping trachoma prevention and detection. Andersen's Behavioral Model explains healthcare utilization through predisposing characteristics, enabling resources, and health needs, providing a framework for understanding access to screening, referral, and eye-care services. Collectively, these theoretical perspectives provide a suitable framework for explaining how community trachoma screening may influence early detection among children in rural Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise children within the applicable school-age range residing in selected rural communities in Nigeria and their parents or primary caregivers. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, rural communities, schools or households, and eligible children. Community trachoma screening will be assessed using indicators such as screening coverage, frequency of screening activities, availability of trained screening personnel, community mobilization, eye examination procedures, identification of suspected trachoma cases, screening of school-aged children, availability of referral arrangements, follow-up of suspected cases, and linkage with appropriate eye-care services. Early detection of trachoma will be assessed using indicators such as identification of suspected cases during screening, time from symptom recognition to screening, time from screening to confirmatory assessment, newly identified cases, clinical classification where applicable, identification before advanced ocular complications develop, referral completion, and initiation of appropriate management. Data will be collected using structured questionnaires, community screening registers, standardized eye-screening forms, school health records, referral registers, eye-clinic records, and relevant trachoma-control programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, suspected cases, confirmed cases, and patterns of early detection. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of community trachoma screening on early detection. Where a quasi-experimental design is adopted, trachoma detection rates and time-to-detection indicators before and after implementation of community screening may be compared with those of a comparison community or group to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community trachoma screening has a significant positive impact on early detection of trachoma among children in rural Nigeria. Children who have access to community-based screening are expected to have a greater likelihood of suspected trachoma being identified at an earlier stage than children without comparable access. Community screening may reduce delays caused by limited awareness, geographical barriers, lack of routine eye examinations, and dependence on parents or caregivers recognizing early eye problems. Early identification may facilitate timely clinical assessment and appropriate management, thereby reducing the risk of progressive eye damage and visual impairment. Screening activities may also increase parental and community awareness of trachoma symptoms, transmission, hygiene, environmental risk factors, and the importance of timely referral. However, inadequate numbers of trained screeners, limited access to eye-care professionals, poor referral and follow-up systems, transportation difficulties, inadequate water and sanitation infrastructure, and low community awareness may reduce the effectiveness of screening programmes. The study therefore expects accessible, culturally appropriate, well-organized, and integrated community trachoma screening to contribute significantly to improved early detection and management of trachoma among children in rural Nigeria. The study is expected to contribute to the literature on trachoma screening, early detection of eye diseases, neglected tropical diseases, child eye health, community ophthalmology, school health, water sanitation and hygiene, infectious disease prevention, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, local government health authorities, primary healthcare centres, eye-care professionals, schools, community health workers, development partners, and policymakers regarding strategies for strengthening trachoma control. The study will also provide evidence-based recommendations for expanding community-based trachoma screening, strengthening referral and follow-up systems, training community health workers and screeners, improving childhood eye-health education, integrating trachoma screening with school and primary healthcare services, promoting hygiene and environmental measures, and ensuring timely access to appropriate eye-care services across rural Nigeria.
