Impact of Hospital Oxygen Systems on Respiratory Emergency Care in Nigerian Hospitals

 

Abstract

Oxygen therapy is an essential component of emergency care for patients experiencing respiratory distress, hypoxaemia, severe infections, trauma, and other life-threatening conditions. Reliable access to medical oxygen is therefore critical for timely and effective management of respiratory emergencies in Nigerian hospitals. However, hospitals may experience challenges including inadequate oxygen supply, limited oxygen-generation capacity, insufficient storage facilities, equipment breakdown, irregular power supply, poor maintenance, shortages of trained personnel, and difficulties distributing oxygen to points of care. These challenges may delay oxygen administration and compromise the quality and timeliness of emergency respiratory care. Hospital oxygen systems, including oxygen concentrators, oxygen cylinders, central oxygen pipelines, oxygen-generation plants, storage systems, and associated monitoring and delivery equipment, provide the infrastructure required for continuous oxygen availability. Against this background, this study investigates the impact of hospital oxygen systems on respiratory emergency care in Nigerian hospitals. The study will be anchored on the Donabedian Model of Healthcare Quality, the Health Systems Framework, and the Systems Theory of Patient Safety. The Donabedian Model explains healthcare quality through structure, process, and outcome, with hospital oxygen systems representing an important structural component and respiratory emergency care representing a measurable care process and outcome. The Health Systems Framework emphasizes infrastructure, medical products and technologies, health workforce, service delivery, financing, and health information as essential components of effective healthcare systems. Systems Theory of Patient Safety emphasizes the interaction among infrastructure, equipment, healthcare workers, organizational processes, and environmental factors in determining the safety and effectiveness of healthcare delivery. Collectively, these theoretical perspectives provide a suitable framework for explaining how hospital oxygen systems may influence respiratory emergency care in Nigerian hospitals. The study will adopt a quantitative analytical cross-sectional or comparative facility-based research design. The study population will comprise selected public and private hospitals providing emergency and respiratory care services in Nigeria, as well as healthcare workers involved in emergency and oxygen therapy. A multistage sampling technique will be used to select states, local government areas, hospitals, emergency departments, intensive care units, respiratory care units, healthcare workers, and relevant patient records. Hospital oxygen systems will be measured using indicators such as availability of functional oxygen sources, oxygen-generation capacity, availability of oxygen cylinders and concentrators, central pipeline systems, oxygen storage capacity, reliability of oxygen supply, oxygen pressure and flow adequacy, availability of backup oxygen systems, equipment functionality, preventive maintenance, power supply, oxygen monitoring systems, availability of delivery devices, and staff training in oxygen therapy. Respiratory emergency care will be assessed using indicators such as time from emergency presentation to oxygen administration, availability of oxygen at the point of care, adherence to oxygen-therapy protocols, monitoring of oxygen saturation, timely clinical assessment, escalation of respiratory support, emergency treatment completion, referral or transfer when required, emergency department waiting time, and selected patient outcomes where appropriate. Data will be collected using structured questionnaires, facility assessment checklists, oxygen equipment records, maintenance records, emergency department registers, patient medical records, oxygen-utilization records, and direct observation. Descriptive statistics will be used to summarize hospital characteristics, oxygen-system capacity, healthcare-worker characteristics, and respiratory emergency-care practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of hospital oxygen systems on respiratory emergency care. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that functional and reliable hospital oxygen systems have a significant positive impact on respiratory emergency care in Nigerian hospitals. Hospitals with adequate and continuously available oxygen systems are expected to demonstrate shorter delays in oxygen administration, better adherence to oxygen-therapy protocols, improved monitoring of patients with respiratory emergencies, and more timely escalation of respiratory support. Reliable oxygen availability may also reduce treatment interruptions and improve healthcare workers' ability to respond effectively to patients with severe respiratory conditions. Conversely, oxygen shortages, equipment failure, inadequate maintenance, insufficient storage, unreliable electricity, poor pipeline infrastructure, and inadequate staff training may contribute to delays in emergency treatment and poorer care processes. The study therefore expects adequately equipped, maintained, monitored, and sustainably supplied hospital oxygen systems to contribute significantly to improved respiratory emergency care in Nigerian hospitals. The study is expected to contribute to the literature on hospital oxygen systems, respiratory emergency care, oxygen therapy, emergency healthcare, hospital infrastructure, critical care, respiratory health, healthcare quality, patient safety, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospital management boards, emergency departments, intensive care units, biomedical engineering departments, respiratory care professionals, healthcare workers, development partners, and policymakers regarding strategies for strengthening oxygen systems in Nigerian hospitals. The study will also provide evidence-based recommendations for expanding oxygen-generation and storage capacity, improving oxygen distribution infrastructure, establishing reliable backup systems, strengthening preventive maintenance, ensuring consistent electricity supply, improving oxygen monitoring, training healthcare workers in oxygen therapy, and developing sustainable hospital oxygen systems to support timely and effective respiratory emergency care across Nigeria.
