Impact of Infection Prevention Surveillance on Healthcare-Associated Infection Reporting in Nigerian Hospitals

 

Abstract

Healthcare-associated infections remain an important patient-safety and public health concern because infections acquired during healthcare delivery may increase morbidity, mortality, hospital length of stay, treatment costs, and healthcare resource utilization. Effective infection prevention surveillance is essential for identifying healthcare-associated infections, monitoring infection patterns, detecting outbreaks, assessing infection-control practices, and generating reliable information for timely public health and clinical interventions. In Nigerian hospitals, healthcare-associated infection reporting may be affected by inadequate surveillance systems, limited infection prevention personnel, poor documentation, inconsistent case definitions, underreporting, weak laboratory support, inadequate data-management systems, and limited feedback to clinical departments. Infection prevention surveillance provides systematic processes for collecting, analysing, interpreting, and communicating information about healthcare-associated infections and related risk factors. Effective surveillance may improve the identification and reporting of healthcare-associated infections and strengthen hospital infection prevention and control programmes. Against this background, this study investigates the impact of infection prevention surveillance on healthcare-associated infection reporting in Nigerian hospitals. The study will be anchored on the Donabedian Model of Healthcare Quality, Systems Theory, and the Surveillance and Response Framework. The Donabedian Model explains healthcare quality through structure, process, and outcome, with infection prevention surveillance representing an important healthcare process and healthcare-associated infection reporting serving as a measurable outcome. Systems Theory views infection surveillance as an interconnected hospital system involving healthcare workers, infection prevention and control committees, laboratories, clinical departments, medical records, hospital management, patients, and reporting structures. The Surveillance and Response Framework emphasizes systematic case detection, data collection, analysis, interpretation, reporting, feedback, and response as essential components of effective disease surveillance. Collectively, these theoretical perspectives provide a suitable framework for explaining how infection prevention surveillance may influence healthcare-associated infection reporting in Nigerian hospitals. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise selected public and private hospitals in Nigeria, healthcare workers involved in infection prevention and control, infection prevention and control personnel, laboratory professionals, and relevant hospital records. A multistage sampling technique will be used to select states, local government areas, hospitals, clinical departments, wards, healthcare workers, and healthcare-associated infection records. Infection prevention surveillance will be assessed using indicators such as availability of formal surveillance systems, standardized case definitions, routine case detection, active surveillance, laboratory-supported surveillance, infection monitoring registers, electronic surveillance systems where available, frequency of data collection, completeness of surveillance data, data analysis, outbreak detection mechanisms, surveillance training, staffing, feedback procedures, reporting timelines, infection prevention committee involvement, and availability of surveillance guidelines. Healthcare-associated infection reporting will be assessed using indicators such as number of reported infection cases, reporting completeness, reporting timeliness, accuracy of reported cases, documentation of infection types, laboratory confirmation where applicable, reporting of infection rates, outbreak notifications, completeness of infection records, and submission of surveillance reports to relevant hospital authorities. Data will be collected using structured questionnaires, infection prevention surveillance registers, laboratory records, medical records, infection-control reports, hospital surveillance databases, incident reports, and relevant administrative documents. Descriptive statistics will be used to summarize hospital characteristics, surveillance activities, healthcare-associated infection patterns, and reporting practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of infection prevention surveillance on healthcare-associated infection reporting. Where a quasi-experimental design is adopted, healthcare-associated infection reporting indicators before and after implementation of an enhanced surveillance system may be compared. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective infection prevention surveillance has a significant positive impact on healthcare-associated infection reporting in Nigerian hospitals. Hospitals with comprehensive surveillance systems are expected to demonstrate higher levels of infection detection, reporting completeness, reporting timeliness, and documentation accuracy than hospitals with weak or informal surveillance arrangements. Routine monitoring may facilitate earlier identification of healthcare-associated infections and infection clusters, allowing infection prevention teams and hospital management to respond more quickly to potential outbreaks. Standardized case definitions, laboratory support, trained surveillance personnel, and regular feedback to clinical departments are also expected to improve the quality and consistency of infection reporting. However, inadequate staffing, limited laboratory capacity, poor documentation, inconsistent application of case definitions, insufficient funding, weak information systems, underreporting, and limited institutional support may reduce surveillance effectiveness. The study therefore expects systematic, adequately resourced, and well-integrated infection prevention surveillance systems to contribute significantly to improved healthcare-associated infection reporting in Nigerian hospitals. The study is expected to contribute to the literature on infection prevention surveillance, healthcare-associated infection reporting, infection prevention and control, hospital epidemiology, patient safety, disease surveillance, healthcare quality, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Nigeria Centre for Disease Control and Prevention, state ministries of health, hospital management boards, infection prevention and control committees, healthcare administrators, epidemiologists, laboratory professionals, healthcare workers, professional bodies, development partners, and policymakers regarding strategies for strengthening hospital infection surveillance. The study will also provide evidence-based recommendations for standardizing healthcare-associated infection surveillance, improving infection case definitions and documentation, strengthening laboratory support, training infection prevention personnel, improving electronic surveillance and reporting systems, establishing regular feedback mechanisms, monitoring reporting completeness and timeliness, and integrating infection surveillance data into hospital quality-improvement and patient-safety programmes across Nigeria.
