Impact of Interactive Voice Health Messages on Health Knowledge among Adults with Limited Literacy in Nigeria

Abstract

Access to understandable and reliable health information is essential for disease prevention, informed healthcare decision-making, and appropriate utilization of health services. Adults with limited literacy in Nigeria may experience difficulties accessing written health information, including printed health education materials, text messages, posters, and digital content, which may restrict their ability to obtain important information about disease prevention and available healthcare services. Interactive voice health messages provide an alternative communication approach by delivering health information through recorded or interactive telephone messages that can be listened to without requiring advanced reading skills. Such messages may cover topics including disease prevention, immunization, sanitation, nutrition, chronic disease prevention, maternal health, screening services, and healthcare-seeking practices. Against this background, this study investigates the impact of interactive voice health messages on health knowledge among adults with limited literacy in Nigeria. The study will be anchored on the Health Literacy Framework, Health Belief Model, and Social Cognitive Theory. The Health Literacy Framework emphasizes the ability of individuals to access, understand, evaluate, and apply health information according to their health needs and literacy levels. The Health Belief Model explains how perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action may influence adults' understanding and responses to health information. Social Cognitive Theory emphasizes observational learning, social interaction, self-efficacy, reinforcement, and behavioural modelling in the acquisition and application of health knowledge. Collectively, these theoretical perspectives provide a suitable framework for explaining how interactive voice health messages may influence health knowledge among adults with limited literacy in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adults aged 18 years and above with limited literacy residing in selected communities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, households, and eligible participants. Interactive voice health messages will be assessed using indicators such as frequency of messages, duration, message content, clarity, language accessibility, use of local dialects, voice quality, message repetition, opportunities for interaction, response options, message timing, and accessibility through basic mobile phones. Health knowledge will be assessed using indicators such as knowledge of disease causes and risk factors, common symptoms and warning signs, disease-prevention measures, hygiene and sanitation, healthy lifestyle practices, vaccination, screening services, appropriate healthcare-seeking practices, and awareness of available health services. Data will be collected using interviewer-administered questionnaires, oral health-knowledge assessment tools, interactive voice message records, call participation records where available, and relevant community health programme documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to voice messages, preferred languages, and health-knowledge scores. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of interactive voice health messages on health knowledge. Where a quasi-experimental design is adopted, knowledge scores before and after exposure to the voice-message intervention may be compared with those of a comparison group to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that interactive voice health messages have a significant positive impact on health knowledge among adults with limited literacy in Nigeria. Adults exposed to regular, clear, and locally understandable voice health messages are expected to demonstrate improved knowledge of disease prevention, symptoms, risk factors, available health services, and appropriate healthcare-seeking practices compared with adults without comparable exposure. The audio-based format may reduce the literacy barriers associated with written health information and allow individuals to access health messages using basic mobile phones. Interactive features, such as the ability to select topics, repeat messages, respond to questions, or request additional information, may further improve engagement and understanding. Messages delivered in local languages and adapted to the cultural context of the target population may increase comprehension and acceptance. However, limited mobile network coverage, poor telephone access, language diversity, unfamiliarity with interactive voice systems, hearing difficulties, message fatigue, technical challenges, and limited availability of trained personnel to manage responses may reduce effectiveness. The study therefore expects accessible, culturally appropriate, multilingual, and interactive voice health messaging to contribute significantly to improved health knowledge among adults with limited literacy in Nigeria. The study is expected to contribute to the literature on interactive voice response health interventions, digital health communication, health literacy, health education, mobile health, health knowledge, health promotion, and inclusive public health communication in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, telecommunications stakeholders, healthcare providers, community health workers, health educators, development partners, and policymakers regarding strategies for reaching adults who may have difficulty accessing written health information. The study will also provide evidence-based recommendations for developing multilingual voice health messages, using simple and culturally appropriate language, incorporating interactive features, ensuring compatibility with basic mobile phones, strengthening community awareness of voice-based health services, and integrating interactive voice messaging with broader community health education and primary healthcare programmes across Nigeria.
