Impact of Maternal Iron Supplementation on Neonatal Birth Weight in Nigeria
Abstract
Low birth weight remains an important maternal and newborn health concern in Nigeria and is associated with increased risks of neonatal morbidity, mortality, impaired growth, and adverse developmental outcomes. Maternal iron deficiency and iron-deficiency anaemia during pregnancy may negatively affect maternal health and fetal growth and may contribute to adverse pregnancy outcomes, including low birth weight. Maternal iron supplementation provides an important opportunity to prevent or reduce iron deficiency during pregnancy and support adequate maternal and fetal development. However, inadequate antenatal care attendance, poor adherence to iron supplementation, limited availability of iron supplements, side effects, inadequate nutrition education, and socioeconomic barriers may reduce the effectiveness of maternal iron supplementation programmes in Nigeria. Against this background, this study investigates the impact of maternal iron supplementation on neonatal birth weight in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Health Systems Framework. The Health Belief Model explains how pregnant women's perceptions of susceptibility to anaemia, perceived severity of iron deficiency, perceived benefits of supplementation, perceived barriers, and cues to action may influence adherence to iron supplementation. The Social Ecological Model emphasizes the influence of individual, household, community, healthcare, nutritional, socioeconomic, and policy factors on maternal nutrition and newborn outcomes. The Health Systems Framework emphasizes antenatal service delivery, health workforce, availability of essential medicines and supplements, health information, financing, governance, and accessibility as important components of effective maternal nutrition services. Collectively, these theoretical perspectives provide a suitable framework for explaining how maternal iron supplementation may influence neonatal birth weight in Nigeria. The study will adopt a quantitative analytical cohort or retrospective cohort research design. The study population will comprise pregnant women receiving antenatal care in selected healthcare facilities in Nigeria and their newborns. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, antenatal clinics, pregnant women, and eligible mother-newborn pairs. Maternal iron supplementation will be assessed using indicators such as receipt of iron supplements during pregnancy, timing of supplementation initiation, dosage, duration of supplementation, adherence, number of tablets or doses consumed, availability of supplements, antenatal care attendance, counselling on supplement use, and documentation of supplementation. Neonatal birth weight will be assessed using birth weight measured at delivery, with low birth weight defined as a birth weight below 2,500 grams. Maternal haemoglobin concentration, anaemia status, gestational age at delivery, and other relevant pregnancy outcomes may also be assessed to provide additional context. Data will be collected using structured questionnaires, antenatal care records, maternal health records, supplement distribution or prescription records, laboratory records, delivery registers, and newborn medical records. Descriptive statistics will be used to summarize maternal and newborn characteristics, supplementation patterns, adherence, maternal anaemia status, and birth-weight outcomes. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of maternal iron supplementation on neonatal birth weight. Where a cohort design is adopted, mean birth weight and the prevalence of low birth weight among newborns of mothers with adequate iron supplementation may be compared with those among newborns of mothers with inadequate or no supplementation, while relevant maternal, nutritional, socioeconomic, and pregnancy-related factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that adequate maternal iron supplementation has a significant positive impact on neonatal birth weight in Nigeria. Newborns whose mothers receive and adhere to recommended iron supplementation during pregnancy are expected to have higher mean birth weights and a lower likelihood of low birth weight compared with newborns whose mothers have inadequate or no documented supplementation. Appropriate iron supplementation may reduce maternal iron deficiency and anaemia, improve maternal health, and support adequate oxygen and nutrient delivery to the developing fetus. However, birth weight is influenced by several factors, including gestational age, maternal nutrition, malaria, infections, maternal age, socioeconomic conditions, and pregnancy complications. Poor adherence, gastrointestinal side effects, inadequate supplement availability, late initiation of antenatal care, and limited nutritional counselling may reduce the effectiveness of supplementation programmes. The study therefore expects timely, accessible, and adequately monitored maternal iron supplementation, combined with quality antenatal care and broader maternal nutrition interventions, to contribute significantly to improved neonatal birth weight in Nigeria. The study is expected to contribute to the literature on maternal iron supplementation, maternal anaemia, neonatal birth weight, maternal nutrition, antenatal care, pregnancy outcomes, newborn health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, healthcare facilities, maternal and child health programmes, obstetricians, midwives, nutritionists, community health workers, development partners, and policymakers regarding strategies for improving maternal nutritional health. The study will also provide evidence-based recommendations for improving the availability and accessibility of iron supplements, strengthening antenatal nutritional counselling, improving adherence to supplementation, monitoring maternal haemoglobin and anaemia status, addressing barriers to supplement use, and integrating maternal iron supplementation into comprehensive maternal, newborn, and child health programmes across Nigeria.
