Impact of Menopause Support Services on Menopause Self-Care Practices among Middle-Aged Women in Nigeria
Abstract
Menopause is a natural stage of the female life course that may be accompanied by physical, psychological, and social changes that can affect women's wellbeing and quality of life. Middle-aged women may experience symptoms such as hot flushes, night sweats, sleep disturbances, mood changes, fatigue, vaginal and urinary symptoms, and changes in body composition. Appropriate self-care practices, including healthy nutrition, regular physical activity, adequate sleep, stress management, appropriate personal hygiene, and timely healthcare-seeking, may help women manage menopausal changes and maintain overall health. However, menopause self-care practices among middle-aged women in Nigeria may be influenced by limited knowledge, cultural beliefs, misconceptions, inadequate support, financial barriers, limited access to menopause-related healthcare, and reliance on informal sources of information. Menopause support services provide opportunities for health education, counselling, symptom-management guidance, psychosocial support, lifestyle advice, and referral to appropriate healthcare professionals. Against this background, this study investigates the impact of menopause support services on menopause self-care practices among middle-aged women in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Health Promotion Model. The Health Belief Model explains how women's perceptions of menopause-related health risks, perceived benefits of appropriate self-care, perceived barriers, and cues to action may influence their adoption of healthy self-care practices. Social Cognitive Theory emphasizes self-efficacy, behavioural capability, observational learning, social support, reinforcement, and environmental influences in the development and maintenance of self-care behaviours. The Health Promotion Model emphasizes individual experiences, perceived benefits and barriers, interpersonal influences, and behavioural factors that influence participation in health-promoting activities. Collectively, these theoretical perspectives provide a suitable framework for explaining how menopause support services may influence menopause self-care practices among middle-aged women in Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise middle-aged women, particularly those experiencing or approaching menopause, attending selected hospitals, primary healthcare centres, women's health clinics, community health programmes, workplaces, and other relevant healthcare or community settings across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, organizations, and eligible women. Menopause support services will be assessed using indicators such as availability of menopause counselling, health education, symptom-management guidance, lifestyle counselling, nutrition advice, physical activity guidance, psychosocial support, peer-support opportunities, healthcare-provider consultations, referral arrangements, follow-up services, and accessibility of menopause-related information and care. Menopause self-care practices will be assessed using indicators such as healthy dietary practices, regular physical activity, adequate sleep, stress-management practices, symptom monitoring, appropriate personal and sexual health practices, avoidance of harmful lifestyle behaviours, use of recommended healthcare services, adherence to appropriate medical advice, and timely healthcare-seeking when concerning symptoms occur. Data will be collected using structured questionnaires, self-care assessment tools, clinic records, health education records, and relevant women's health programme documents. Descriptive statistics will be used to summarize participants' characteristics, access to menopause support services, and patterns of self-care practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of menopause support services on menopause self-care practices. Where a quasi-experimental design is adopted, self-care practice scores before and after participation in structured menopause support services may be compared, while relevant age, socioeconomic, educational, health, cultural, and healthcare-access factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that menopause support services have a significant positive impact on menopause self-care practices among middle-aged women in Nigeria. Women who have access to structured and comprehensive menopause support services are expected to demonstrate better self-care practices than women with limited access to menopause-related support. Counselling and health education may improve women's understanding of menopausal changes and encourage healthy dietary practices, physical activity, adequate sleep, stress management, symptom monitoring, and appropriate healthcare-seeking. Psychosocial and peer support may also improve self-efficacy and reduce feelings of isolation, fear, or embarrassment associated with menopausal symptoms. However, financial constraints, cultural beliefs, stigma, limited availability of specialized menopause services, inadequate healthcare personnel, geographical barriers, and reliance on informal or inaccurate information may reduce the effectiveness of support services. The study therefore expects accessible, confidential, culturally appropriate, and comprehensive menopause support services to contribute significantly to improved self-care practices among middle-aged women in Nigeria. The study is expected to contribute to the literature on menopause support services, menopause self-care practices, menopausal health, women's health, healthy ageing, health promotion, lifestyle management, reproductive health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, women's health clinics, healthcare providers, community health workers, menopause programmes, development partners, and policymakers regarding strategies for improving women's health during the menopausal transition. The study will also provide evidence-based recommendations for integrating menopause support services into routine women's healthcare, strengthening healthcare-provider training in menopause counselling, developing culturally appropriate educational and self-care materials, establishing community-based menopause support programmes, improving referral and follow-up systems, and promoting healthy self-care practices among middle-aged women across Nigeria.
