Impact of Mental Health Crisis Education on Mental Health Crisis Knowledge among Emergency Healthcare Workers in Nigeria

Abstract

Mental health crises are important public health and emergency healthcare concerns that may require rapid recognition, appropriate assessment, immediate intervention, referral, and coordinated care. Emergency healthcare workers in Nigeria may encounter patients experiencing acute psychological distress, suicidal behaviour, severe agitation, psychosis, substance-related crises, self-harm, or other urgent mental health conditions within emergency departments and other acute-care settings. However, limited mental health crisis knowledge, inadequate specialized training, staff shortages, high workload, limited mental health resources, and weak referral systems may affect healthcare workers' ability to recognize and respond appropriately to mental health emergencies. Mental health crisis education provides an opportunity to equip emergency healthcare workers with knowledge of common mental health crises, warning signs, risk assessment, de-escalation principles, immediate supportive interventions, safety considerations, referral pathways, and appropriate coordination with mental health professionals. Against this background, this study investigates the impact of mental health crisis education on mental health crisis knowledge among emergency healthcare workers in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and the Crisis Intervention Model. The Health Belief Model explains how emergency healthcare workers' perceptions of the seriousness and urgency of mental health crises, perceived benefits of appropriate crisis education, perceived barriers, and cues to action may influence their acquisition and application of crisis-related knowledge. Social Cognitive Theory emphasizes the roles of knowledge, self-efficacy, observational learning, behavioural skills, and workplace environment in developing competencies for responding to mental health crises. The Crisis Intervention Model provides a framework for understanding immediate assessment, stabilization, safety management, supportive intervention, and referral during acute psychological crises. Collectively, these theoretical perspectives provide a suitable framework for explaining how mental health crisis education may influence mental health crisis knowledge among emergency healthcare workers in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise emergency healthcare workers, including doctors, nurses, community health professionals, emergency medical personnel, and other healthcare workers involved in emergency care in selected public and private healthcare facilities across Nigeria. A multistage sampling technique will be used to select states, healthcare institutions, emergency departments, emergency units, and eligible healthcare workers. Mental health crisis education will be measured using indicators such as exposure to structured crisis education sessions, training frequency and duration, recognition of mental health crises, identification of warning signs, suicide and self-harm risk awareness, assessment of acute psychological distress, recognition of severe agitation and psychosis, substance-related crisis recognition, communication during crises, de-escalation principles, immediate supportive care, safety planning, referral procedures, emergency mental health resources, confidentiality, documentation, and coordination with mental health professionals. Mental health crisis knowledge will be assessed using indicators such as the ability to identify common mental health crises, recognize warning signs, understand suicide and self-harm risks, identify acute psychological distress, recognize severe agitation and psychotic symptoms, understand basic crisis assessment, identify appropriate de-escalation approaches, understand immediate safety measures, recognize appropriate referral pathways, and identify situations requiring urgent specialist intervention. Data will be collected using structured questionnaires, case-based scenarios, and appropriately validated mental health crisis knowledge instruments administered before and after the educational intervention where a quasi-experimental design is used. Descriptive statistics will be used to summarize healthcare workers' demographic and professional characteristics, emergency-care experience, exposure to mental health crisis education, and knowledge levels. Inferential statistical techniques, including paired or independent sample tests, chi-square tests, correlation analysis, and multiple regression analysis where appropriate, will be used to determine the impact of mental health crisis education on mental health crisis knowledge. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that mental health crisis education has a significant positive impact on mental health crisis knowledge among emergency healthcare workers in Nigeria. Healthcare workers exposed to structured mental health crisis education are expected to demonstrate improved knowledge of crisis recognition, suicide and self-harm warning signs, acute psychological distress, severe agitation, psychosis, substance-related crises, de-escalation principles, immediate safety measures, referral procedures, and coordination of emergency mental health care compared with their baseline knowledge or healthcare workers without similar educational exposure. Practical case scenarios, simulation exercises, role-playing, crisis assessment activities, and de-escalation demonstrations may strengthen healthcare workers' ability to recognize and understand mental health emergencies in real clinical settings. Improved crisis knowledge may support earlier recognition, safer initial responses, appropriate referral, and better coordination of care. However, high emergency workloads, limited staffing, inadequate mental health resources, insufficient training opportunities, stigma, limited access to mental health specialists, and weak referral networks may restrict the effectiveness and sustainability of mental health crisis education. The study therefore expects accessible, practical, emergency-focused, culturally appropriate, and sustained mental health crisis education to contribute significantly to improved mental health crisis knowledge among emergency healthcare workers in Nigeria. The study is expected to contribute to the literature on mental health crisis education, mental health crisis knowledge, emergency healthcare workers, emergency mental health care, crisis intervention, suicide prevention, occupational health, healthcare education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital administrators, emergency departments, emergency medical services, healthcare professional bodies, mental health institutions, psychiatrists, psychologists, nurses, emergency physicians, public health practitioners, development partners, and policymakers regarding strategies for strengthening emergency mental health preparedness. The study will also provide evidence-based recommendations for integrating mental health crisis education into emergency healthcare worker training and continuing professional development, strengthening crisis-response protocols, improving access to mental health specialists, establishing effective referral pathways, expanding simulation-based crisis training, and developing sustainable emergency mental health education programmes across healthcare facilities in Nigeria.
