Impact of Physiotherapy Services on Functional Mobility among Older Adults in Nigeria

 

Abstract

Functional mobility is an important component of healthy ageing because it enables older adults to perform daily activities, maintain independence, participate in social activities, and preserve their quality of life. However, ageing is often associated with physical limitations resulting from musculoskeletal conditions, stroke, arthritis, chronic diseases, balance problems, falls, and other age-related impairments that may reduce mobility and increase dependence on caregivers. Access to appropriate physiotherapy services may help older adults improve strength, balance, flexibility, gait, endurance, and physical functioning. Physiotherapy interventions may include therapeutic exercises, gait and balance training, mobility exercises, strengthening programmes, functional rehabilitation, assistive-device training, and individualized physical activity programmes. Despite the potential benefits of physiotherapy, access to rehabilitation services among older adults in Nigeria may be affected by limited availability of physiotherapists, financial constraints, inadequate rehabilitation facilities, geographical barriers, low awareness, transportation difficulties, and limited referral to physiotherapy services. Against this background, this study investigates the impact of physiotherapy services on functional mobility among older adults in Nigeria. The study will be anchored on the International Classification of Functioning, Disability and Health (ICF) Framework, the Biopsychosocial Model, and the Donabedian Model of Healthcare Quality. The ICF Framework explains functioning and disability through the interaction between health conditions, body functions and structures, activities, participation, and environmental factors, providing a suitable framework for understanding changes in functional mobility following physiotherapy. The Biopsychosocial Model recognizes the interaction of physical, psychological, and social factors in determining mobility and functional independence among older adults. The Donabedian Model explains healthcare quality through structure, process, and outcome, with physiotherapy services representing an important care process and functional mobility serving as a measurable outcome. Collectively, these theoretical perspectives provide a suitable framework for explaining how physiotherapy services may influence functional mobility among older adults in Nigeria. The study will adopt a quantitative quasi-experimental or prospective cohort research design. The study population will comprise adults aged 60 years and above receiving care in selected hospitals, rehabilitation centres, physiotherapy clinics, and community-based healthcare facilities in Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, physiotherapy centres, and eligible older adults. Physiotherapy services will be assessed using indicators such as frequency of physiotherapy sessions, duration of treatment, type of intervention, therapeutic exercise, strength training, balance training, gait training, flexibility exercises, functional training, use of assistive devices, home exercise programmes, patient education, individualized treatment plans, adherence to therapy, and follow-up services. Functional mobility will be assessed using appropriate standardized measures such as the Timed Up and Go test, gait speed, walking distance, chair-rise performance, balance assessment, ability to perform activities of daily living, use of mobility aids, and self-reported mobility limitations. Data will be collected using structured questionnaires, physiotherapy treatment records, clinical assessment tools, patient medical records, functional mobility assessments, rehabilitation registers, and follow-up records. Descriptive statistics will be used to summarize participants' demographic and clinical characteristics, patterns of physiotherapy utilization, and functional mobility status. Inferential statistical techniques, including paired-sample tests, independent-sample tests, correlation analysis, and multiple regression analysis where appropriate, will be used to determine the impact of physiotherapy services on functional mobility. Where a quasi-experimental design is adopted, functional mobility before and after physiotherapy intervention may be compared, or outcomes among older adults receiving structured physiotherapy may be compared with those receiving usual care. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that physiotherapy services have a significant positive impact on functional mobility among older adults in Nigeria. Older adults who receive structured and appropriately prescribed physiotherapy interventions are expected to demonstrate improved walking ability, balance, muscle strength, functional performance, and independence in mobility compared with their baseline status or older adults receiving usual care. Regular therapeutic exercise and gait and balance training may reduce physical limitations and improve confidence in movement, while individualized rehabilitation programmes may help older adults perform daily activities more independently. Physiotherapy may also contribute to fall-risk reduction and improved participation in household and community activities. However, limited access to qualified physiotherapists, inadequate rehabilitation facilities, treatment costs, transportation difficulties, poor referral systems, low awareness among older adults and caregivers, and poor adherence to prescribed exercises may reduce the effectiveness of physiotherapy services. The study therefore expects accessible, affordable, individualized, and adequately supervised physiotherapy services to contribute significantly to improved functional mobility among older adults in Nigeria. The study is expected to contribute to the literature on physiotherapy services, functional mobility, older adult health, geriatric rehabilitation, physical functioning, disability prevention, fall prevention, rehabilitation services, healthy ageing, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, rehabilitation centres, physiotherapy departments, physiotherapists, geriatric healthcare providers, caregivers, development partners, and policymakers regarding strategies for improving rehabilitation services for older adults. The study will also provide evidence-based recommendations for expanding access to physiotherapy services, integrating rehabilitation into primary and geriatric healthcare, increasing the availability of trained physiotherapists, improving rehabilitation infrastructure, strengthening referral systems, reducing financial and transportation barriers, promoting home-based physiotherapy, and increasing awareness of the importance of physical rehabilitation for maintaining functional independence among older adults across Nigeria.
