Impact of Pregnancy Oral Health Counselling on Dental Care-Seeking Practices among Pregnant Women in Nigeria

 

Abstract

Pregnancy is associated with physiological and hormonal changes that may influence women's oral health and increase susceptibility to conditions such as gingivitis, dental caries, periodontal problems, tooth sensitivity, and other dental disorders. Timely utilization of professional dental care during pregnancy can facilitate early assessment, appropriate management, and prevention of complications. However, many pregnant women may delay seeking dental care because of inadequate knowledge, misconceptions about the safety of dental treatment during pregnancy, fear of dental procedures, financial constraints, limited access to dental professionals, and competing maternal healthcare needs. Pregnancy oral health counselling provides an opportunity to educate women about oral health changes during pregnancy, appropriate preventive practices, the importance of professional dental assessment, and circumstances requiring timely dental care. Against this background, this study investigates the impact of pregnancy oral health counselling on dental care-seeking practices among pregnant women in Nigeria. The study will be anchored on the Health Belief Model, Andersen's Behavioral Model of Health Services Use, and the Theory of Planned Behavior. The Health Belief Model explains how pregnant women's perceptions of susceptibility to oral health problems, perceived severity, perceived benefits of professional dental care, perceived barriers, and cues to action may influence their care-seeking behaviour. Andersen's Behavioral Model explains how predisposing characteristics, enabling resources, and perceived healthcare needs influence women's utilization of dental services. The Theory of Planned Behavior explains how attitudes toward dental care, perceived social expectations, perceived behavioural control, and intentions may influence women's decisions to seek professional dental care during pregnancy. Collectively, these theoretical perspectives provide a suitable framework for explaining how pregnancy oral health counselling may influence dental care-seeking practices among pregnant women in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise pregnant women attending selected antenatal clinics, primary healthcare centres, hospitals, maternity clinics, and community maternal health programmes across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, and eligible pregnant women. Pregnancy oral health counselling will be assessed using indicators such as exposure to structured counselling, frequency and duration of counselling sessions, information on oral health changes during pregnancy, dental hygiene, common pregnancy-related dental problems, safety of appropriate dental treatment, benefits of professional dental care, warning signs requiring dental evaluation, available dental services, referral information, and opportunities for women to ask questions. Dental care-seeking practices will be assessed using indicators such as attendance at dental clinics during pregnancy, seeking professional care for tooth pain or other oral symptoms, completion of recommended dental appointments, willingness to undergo dental assessment, timely response to oral health problems, compliance with dental referrals, and follow-up after treatment. Data will be collected using structured questionnaires, validated oral health care-seeking assessment instruments, antenatal records, dental clinic records, referral registers, and relevant healthcare documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to oral health counselling, and patterns of dental care-seeking. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of pregnancy oral health counselling on dental care-seeking practices. Where a quasi-experimental design is adopted, dental care-seeking practice scores before and after the counselling intervention will be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that pregnancy oral health counselling has a significant positive impact on dental care-seeking practices among pregnant women in Nigeria. Women exposed to structured and evidence-based oral health counselling are expected to demonstrate greater awareness of the importance of professional dental care and a higher likelihood of seeking dental services when they experience oral health symptoms or require preventive assessment. Counselling may correct misconceptions about the safety of appropriate dental treatment during pregnancy, reduce fear, improve confidence in communicating oral health concerns, and encourage women to seek timely professional care rather than relying on self-medication or delaying treatment. Integrating oral health counselling into antenatal care may also provide an effective opportunity to connect pregnant women with available dental services. However, financial barriers, geographical distance, limited availability of dental professionals, fear of dental procedures, cultural beliefs, inadequate referral systems, and competing maternal healthcare demands may continue to influence care-seeking practices. The study therefore expects accessible, culturally appropriate, practical, and continuous pregnancy oral health counselling to contribute significantly to improved dental care-seeking practices among pregnant women in Nigeria. The study is expected to contribute to the literature on pregnancy oral health counselling, dental care-seeking practices, maternal oral health, antenatal care, preventive dentistry, pregnancy-related oral health, healthcare utilization, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, dental clinics, dentists, dental therapists, nurses, midwives, community health workers, maternal health organizations, development partners, and policymakers regarding strategies for improving oral healthcare utilization during pregnancy. The study will also provide evidence-based recommendations for integrating oral health counselling into routine antenatal care, strengthening referral pathways between maternal and dental services, training antenatal healthcare providers in oral health promotion, improving access to appropriate dental services, addressing misconceptions about dental treatment during pregnancy, and promoting timely professional dental care among pregnant women across Nigeria.
