Effect of Antenatal Blood Group Education on Knowledge of Rh Incompatibility among Pregnant Women in Nigeria

 

Abstract

Rh incompatibility is an important maternal and newborn health concern that may occur when an Rh-negative pregnant woman carries an Rh-positive fetus. Without appropriate antenatal assessment and preventive management, maternal sensitization may contribute to haemolytic disease of the fetus and newborn in subsequent or affected pregnancies. Despite the availability of blood-group testing and preventive interventions, some pregnant women may have limited knowledge of their blood group, Rh status, the meaning of Rh incompatibility, potential pregnancy-related risks, and the importance of appropriate antenatal care. Antenatal blood group education provides an opportunity to improve pregnant women's understanding of blood-group differences, Rh incompatibility, preventive measures, and the importance of following recommended antenatal and specialist care. Against this background, this study investigates the effect of antenatal blood group education on knowledge of Rh incompatibility among pregnant women in Nigeria. The study will be anchored on the Health Belief Model, Health Literacy Framework, and Social Cognitive Theory. The Health Belief Model explains how pregnant women's perceptions of susceptibility to Rh incompatibility, perceived severity of possible complications, perceived benefits of appropriate antenatal care, perceived barriers, and cues to action may influence their knowledge and preventive health behaviours. The Health Literacy Framework emphasizes women's ability to access, understand, evaluate, and apply information concerning blood groups and Rh incompatibility. Social Cognitive Theory emphasizes observational learning, self-efficacy, social support, and behavioural reinforcement in the acquisition and application of health knowledge. Collectively, these theoretical perspectives provide a suitable framework for explaining how antenatal blood group education may influence knowledge of Rh incompatibility among pregnant women in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise pregnant women attending selected public and private antenatal clinics across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, healthcare facilities, antenatal clinics, and eligible pregnant women. Antenatal blood group education will be assessed using indicators such as exposure to structured educational sessions, frequency and duration of education, information about ABO and Rh blood groups, interpretation of blood-group results, understanding of Rh-positive and Rh-negative status, explanation of Rh incompatibility, maternal sensitization, potential pregnancy and newborn complications, preventive measures, recommended antenatal investigations, and appropriate referral or specialist care. Knowledge of Rh incompatibility will be assessed using indicators such as identification of Rh status, understanding of maternal-fetal Rh differences, recognition of the potential consequences of sensitization, knowledge of the importance of antenatal blood-group testing, awareness of preventive interventions such as anti-D immunoglobulin where clinically indicated, understanding of the importance of follow-up and appropriate medical care, and ability to identify situations requiring further professional assessment. Data will be collected using structured questionnaires, validated maternal health knowledge instruments, antenatal education attendance records, and relevant antenatal clinic documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to blood group education, and knowledge scores. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of antenatal blood group education on knowledge of Rh incompatibility. Where a quasi-experimental design is adopted, knowledge scores before and after the educational intervention may be compared to determine changes associated with the programme. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that antenatal blood group education has a significant positive effect on knowledge of Rh incompatibility among pregnant women in Nigeria. Pregnant women exposed to structured and clear blood-group education are expected to demonstrate improved understanding of ABO and Rh blood groups, maternal Rh status, Rh incompatibility, maternal sensitization, potential complications, and the importance of appropriate antenatal management. Education may also improve women's understanding of the need for blood-group testing and appropriate preventive interventions when clinically indicated. Interactive antenatal counselling, visual educational materials, group discussions, and individualized explanations may improve women's ability to understand their blood-group results and communicate effectively with healthcare providers. However, low health literacy, language barriers, limited antenatal attendance, misconceptions, inadequate counselling time, cultural beliefs, and limited access to appropriate preventive interventions may reduce the effectiveness of blood-group education. The study therefore expects accessible, culturally appropriate, understandable, and regularly reinforced antenatal blood group education to contribute significantly to improved knowledge of Rh incompatibility among pregnant women in Nigeria. The study is expected to contribute to the literature on antenatal blood group education, Rh incompatibility, maternal health knowledge, antenatal care, maternal and newborn health, reproductive health education, health literacy, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare facilities, antenatal clinics, obstetricians, midwives, nurses, laboratory scientists, maternal health educators, development partners, and policymakers regarding strategies for improving awareness of Rh incompatibility. The study will also provide evidence-based recommendations for strengthening blood-group counselling within antenatal care, improving access to blood-group and Rh testing, standardizing patient education, strengthening referral systems, and improving access to appropriate preventive management for Rh-negative pregnant women across Nigeria.
