Effect of Arthritis Self-Management Education on Self-Care Practices among Older Adults in Nigeria
Abstract
Arthritis is an important chronic health condition among older adults and may cause persistent joint pain, stiffness, reduced mobility, functional limitations, and difficulty performing activities of daily living. Poorly managed arthritis symptoms may contribute to physical inactivity, reduced independence, social isolation, and deterioration in quality of life. Effective self-management practices can help older adults manage symptoms, maintain physical function, and participate more actively in daily life. However, limited knowledge of arthritis management, misconceptions about physical activity, inadequate access to rehabilitation services, financial constraints, and reliance on inappropriate self-treatment may affect effective self-care. Arthritis self-management education provides an opportunity to improve older adults' knowledge, confidence, and practical skills regarding exercise, medication use, pain management, joint protection, weight management, assistive devices, and appropriate healthcare-seeking. Against this background, this study investigates the effect of arthritis self-management education on self-care practices among older adults in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Self-Efficacy Theory. The Health Belief Model explains how older adults' perceptions of the severity of arthritis, perceived benefits of self-management, perceived barriers, and cues to action may influence their self-care practices. Social Cognitive Theory emphasizes observational learning, behavioural reinforcement, social support, and environmental influences in the adoption and maintenance of appropriate self-management behaviours. Self-Efficacy Theory emphasizes individuals' confidence in their ability to perform recommended self-care behaviours and manage arthritis-related challenges. Collectively, these theoretical perspectives provide a suitable framework for explaining how arthritis self-management education may influence self-care practices among older adults in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adults aged 60 years and above diagnosed with arthritis and receiving care in selected hospitals, primary healthcare facilities, and community health programmes across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, healthcare facilities, communities, and eligible older adults. Arthritis self-management education will be assessed using indicators such as exposure to structured education, frequency and duration of educational sessions, information on arthritis symptoms, medication use, physical activity, therapeutic exercises, pain-management techniques, joint protection, weight management, use of assistive devices, nutrition, fall prevention, and appropriate healthcare-seeking. Self-care practices will be assessed using indicators such as adherence to prescribed medication, participation in recommended physical activity and exercises, appropriate pain-management practices, joint-protection behaviours, use of assistive devices where indicated, weight-management practices, attendance at healthcare appointments, adherence to rehabilitation recommendations, and timely reporting of worsening symptoms. Data will be collected using structured questionnaires, validated arthritis self-management and self-care practice instruments, patient education records, clinic records, rehabilitation records, and relevant community health programme documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to arthritis education, and self-care practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of arthritis self-management education on self-care practices. Where a quasi-experimental design is adopted, self-care practice scores before and after the educational intervention may be compared to determine changes associated with the programme. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that arthritis self-management education has a significant positive effect on self-care practices among older adults in Nigeria. Older adults who receive structured and practical arthritis self-management education are expected to demonstrate improved medication adherence, increased participation in appropriate physical activity, better pain-management practices, greater use of joint-protection strategies, and improved attendance at recommended healthcare and rehabilitation appointments. Practical demonstrations, individualized counselling, caregiver involvement, and repeated education may improve older adults' confidence in managing arthritis and incorporating recommended self-care behaviours into their daily routines. Education may also correct misconceptions that physical activity is always harmful for individuals with arthritis and encourage appropriate exercise and rehabilitation. However, pain severity, physical limitations, medication costs, limited access to physiotherapy and rehabilitation services, transportation difficulties, low health literacy, cultural beliefs, and inadequate family support may affect adherence to recommended self-care practices. The study therefore expects accessible, practical, culturally appropriate, and regularly reinforced arthritis self-management education to contribute significantly to improved self-care practices among older adults in Nigeria. The study is expected to contribute to the literature on arthritis self-management education, self-care practices, arthritis management, older adult health, chronic disease management, rehabilitation, pain management, healthy ageing, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Senior Citizens Centre, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, physiotherapists, occupational therapists, nurses, physicians, community health workers, caregivers, development partners, and policymakers regarding strategies for improving arthritis management among older adults. The study will also provide evidence-based recommendations for integrating arthritis self-management education into routine elderly healthcare, strengthening patient and caregiver counselling, improving access to rehabilitation services, promoting safe physical activity, supporting appropriate pain-management practices, and developing sustainable community-based arthritis self-management programmes across Nigeria.
