Effect of Blood Group Education on Voluntary Blood Donation Knowledge among University Students in Nigeria
Abstract
Voluntary blood donation is an essential component of a safe and sustainable blood supply and plays an important role in the management of obstetric emergencies, trauma, surgery, severe anaemia, cancer, and other medical conditions requiring blood transfusion. However, inadequate knowledge of blood groups, blood compatibility, donor eligibility, donation procedures, safety, and the importance of regular voluntary donation may contribute to misconceptions and low participation in blood donation programmes. University students represent an important population for health education because of their potential to become regular voluntary blood donors and influence donation practices within their families and communities. Blood group education provides an opportunity to improve students' understanding of blood groups and their relationship to blood donation and transfusion safety. Against this background, this study investigates the effect of blood group education on voluntary blood donation knowledge among university students in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Health Literacy Framework. The Health Belief Model explains how students' perceptions of the importance of blood donation, perceived benefits, perceived barriers, and cues to action may influence their acquisition of blood donation knowledge. Social Cognitive Theory emphasizes observational learning, self-efficacy, social influence, and reinforcement in the development of health-related knowledge and behaviours. The Health Literacy Framework emphasizes the ability of students to access, understand, evaluate, and apply blood donation and transfusion-related health information. Collectively, these theoretical perspectives provide a suitable framework for explaining how blood group education may influence voluntary blood donation knowledge among university students in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise undergraduate students enrolled in selected public and private universities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, universities, faculties or departments, and eligible students. Blood group education will be assessed using indicators such as exposure to structured educational sessions, frequency and duration of education, information on ABO and Rh blood groups, blood group compatibility, donor eligibility, blood donation procedures, pre-donation assessment, safety requirements, post-donation care, misconceptions about blood donation, importance of voluntary non-remunerated donation, and recommended donation practices. Voluntary blood donation knowledge will be assessed using indicators such as understanding of donor eligibility criteria, blood group compatibility, donation safety, preparation for donation, donation procedures, possible temporary reactions, post-donation care, confidentiality, importance of regular voluntary donation, and common misconceptions about blood donation. Data will be collected using structured questionnaires, validated blood donation knowledge assessment instruments, educational programme attendance records, and relevant university health programme documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to blood group education, and levels of voluntary blood donation knowledge. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of blood group education on voluntary blood donation knowledge. Where a quasi-experimental design is adopted, knowledge scores before and after the educational intervention may be compared to determine changes associated with the programme. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that blood group education has a significant positive effect on voluntary blood donation knowledge among university students in Nigeria. Students who receive structured blood group education are expected to demonstrate improved understanding of ABO and Rh blood groups, blood compatibility, donor eligibility, donation procedures, safety requirements, and the importance of voluntary blood donation. Education may correct misconceptions regarding blood donation, including fears that donating blood causes permanent weakness or serious health problems. Interactive teaching, practical explanations, visual materials, and opportunities to discuss blood transfusion and donation may improve students' knowledge and confidence. However, cultural beliefs, fear of needles, misconceptions, misinformation, limited exposure to reliable health information, and concerns about the safety of blood donation may continue to affect students' understanding. The study therefore expects accessible, accurate, culturally appropriate, and regularly reinforced blood group education to contribute significantly to improved voluntary blood donation knowledge among university students in Nigeria. The study is expected to contribute to the literature on blood group education, voluntary blood donation knowledge, blood transfusion safety, health education, university students, blood donation awareness, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Blood Service Agency, universities, university health services, blood transfusion centres, healthcare professionals, public health educators, student organizations, development partners, and policymakers regarding strategies for strengthening voluntary blood donation programmes. The study will also provide evidence-based recommendations for integrating blood group and blood donation education into university health programmes, developing culturally appropriate educational materials, correcting misconceptions about blood donation, strengthening campus blood donation campaigns, improving students' understanding of donor eligibility and safety, and promoting a sustainable culture of voluntary blood donation among young adults across Nigeria.
