Effect of Community-Based Breast Cancer Screening on Breast Abnormality Detection among Women in Nigeria

Abstract

Breast cancer remains a major public health concern among women in Nigeria and is associated with substantial morbidity, mortality, and social and economic consequences. Many women may present with breast abnormalities at advanced stages because of limited awareness, inadequate access to screening services, financial constraints, geographical barriers, fear of diagnosis, and delays in seeking medical care. Early detection of breast abnormalities is important because it can facilitate timely diagnostic evaluation, appropriate referral, treatment, and improved health outcomes. Community-based breast cancer screening provides an opportunity to bring breast health services closer to women through community outreach programmes, primary healthcare centres, mobile screening services, women's organizations, workplaces, markets, religious institutions, and other community platforms. Such programmes may include breast health education, clinical breast examination, risk assessment, and referral for further diagnostic investigation where abnormalities are identified. However, inadequate screening coverage, shortages of trained healthcare personnel, limited access to appropriate diagnostic equipment, low awareness, concerns about stigma, and weak referral systems may limit the effectiveness of community-based breast cancer screening programmes in Nigeria. Against this background, this study investigates the effect of community-based breast cancer screening on breast abnormality detection among women in Nigeria. The study will be anchored on the Health Belief Model, Community Health Theory, and the Health Systems Framework. The Health Belief Model explains how women's perceptions of susceptibility to breast cancer, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in breast cancer screening. Community Health Theory emphasizes the delivery of health services through accessible community structures and the involvement of communities in disease prevention, health promotion, and early detection activities. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective screening and healthcare programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based breast cancer screening may influence breast abnormality detection among women in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise women aged 18 years and above residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, households, and eligible women. Community-based breast cancer screening will be measured using indicators such as availability of screening services, screening coverage, frequency of community outreach activities, accessibility of screening locations, number of women screened, availability of trained healthcare personnel, breast health education, clinical breast examination, risk assessment, availability of screening equipment, and referral arrangements. Breast abnormality detection will be assessed using indicators such as abnormal breast examination findings, breast lumps, changes in breast size or shape, nipple abnormalities, skin changes, suspected breast lesions, number of women referred for further diagnostic evaluation, confirmed breast abnormalities, and stage of confirmed breast disease where applicable. Data will be collected using structured questionnaires, community screening registers, clinical examination records, referral records, diagnostic reports, healthcare facility records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, and patterns of breast abnormality detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the effect of community-based breast cancer screening on breast abnormality detection. Where appropriate, detection rates before and after implementation of community-based screening activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based breast cancer screening has a significant positive effect on breast abnormality detection among women in Nigeria. Women who participate in community-based breast cancer screening are expected to have a higher likelihood of having previously undetected breast abnormalities identified than women without access to such services. Community-based screening may improve early identification of breast lumps, changes in breast appearance, nipple abnormalities, and other suspicious findings that require further diagnostic evaluation. Bringing screening services closer to women may reduce geographical, financial, and access barriers and may encourage greater participation in preventive breast health services. Early detection of abnormalities may facilitate timely referral for diagnostic imaging, biopsy, specialist consultation, and appropriate treatment where necessary. Conversely, inadequate screening coverage, limited availability of trained personnel, insufficient diagnostic resources, low awareness, fear of breast cancer, stigma, and weak referral and follow-up systems may reduce the effectiveness of community-based breast cancer screening. The study therefore expects accessible and well-organized community-based breast screening programmes to contribute significantly to improved detection of breast abnormalities among women in Nigeria. The study is expected to contribute to the literature on community-based breast cancer screening, breast abnormality detection, women's health, breast health, cancer prevention, early disease detection, community health services, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare centres, oncology centres, breast health programmes, healthcare providers, community health workers, women's organizations, development partners, and policymakers regarding strategies for improving early detection of breast abnormalities. The study will also provide evidence-based recommendations for expanding community-based breast screening programmes, improving access to clinical breast examination and appropriate diagnostic services, strengthening training of healthcare workers, increasing breast health awareness among women, reducing stigma and fear associated with breast cancer screening, strengthening referral and follow-up systems, and integrating community-based breast health screening into primary healthcare and women's health programmes across Nigeria.
