Effect of Community-Based Hearing Screening on Hearing Loss Detection among Older Adults in Nigeria

Abstract

Hearing loss is a common health concern among older adults and can negatively affect communication, social participation, independence, emotional wellbeing, and quality of life. Age-related hearing loss often develops gradually and may remain undetected because some older adults may perceive hearing difficulties as a normal consequence of ageing or may face barriers to accessing audiological services. Community-based hearing screening provides an opportunity to identify older adults with suspected hearing impairment within their communities and facilitate timely referral for comprehensive audiological assessment, treatment, rehabilitation, and appropriate hearing support. However, limited availability of trained audiologists, inadequate screening equipment, low awareness of hearing health, financial constraints, geographical barriers, and weak referral systems may restrict access to hearing screening services in Nigeria. Against this background, this study investigates the effect of community-based hearing screening on hearing loss detection among older adults in Nigeria. The study will be anchored on the Health Belief Model, Andersen's Behavioral Model of Health Services Use, and the Social Ecological Model. The Health Belief Model explains how older adults' perceptions of susceptibility to hearing loss, perceived severity of its consequences, perceived benefits of screening, perceived barriers, and cues to action may influence participation in hearing screening. Andersen's Behavioral Model explains how predisposing characteristics, enabling resources, and perceived healthcare needs influence utilization of hearing screening and audiological services. The Social Ecological Model emphasizes the interaction between individual, family, community, healthcare-system, and broader environmental factors in determining access to hearing healthcare. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based hearing screening may influence hearing loss detection among older adults in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adults aged 60 years and above residing in selected urban and rural communities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, households, and eligible older adults. Community-based hearing screening will be assessed using indicators such as availability of screening programmes, screening coverage, frequency of screening activities, availability of trained audiologists or hearing-care personnel, availability and functionality of screening equipment, use of standardized screening procedures, community mobilization, referral arrangements, and follow-up services. Hearing loss detection will be assessed using indicators such as abnormal screening results, elevated hearing thresholds, suspected hearing impairment, newly identified cases, degree and type of hearing loss where confirmed, referral for comprehensive audiological assessment, confirmed diagnosis, and recommendations for hearing rehabilitation or assistive hearing devices. Data will be collected using structured questionnaires, hearing screening records, audiometric screening results, referral registers, healthcare records, and relevant community health programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, hearing assessment findings, and patterns of hearing loss detection. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of community-based hearing screening on hearing loss detection. Where a quasi-experimental design is adopted, hearing loss detection rates before and after implementation of community-based screening programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based hearing screening has a significant positive effect on hearing loss detection among older adults in Nigeria. Older adults who participate in community-based hearing screening are expected to have a higher likelihood of previously undetected hearing difficulties being identified than those without access to screening services. Conducting screening within communities may reduce geographical and financial barriers and improve access for older adults who may not ordinarily seek audiological services. Early identification may facilitate comprehensive audiological assessment, medical evaluation where necessary, hearing rehabilitation, provision of hearing aids or other assistive devices, communication support, and appropriate follow-up. Community-based programmes may also increase awareness among older adults, caregivers, and family members about hearing health and the potential consequences of untreated hearing loss. However, shortages of trained hearing-care personnel, inadequate screening equipment, limited referral facilities, financial barriers, transportation difficulties, low awareness, and weak follow-up mechanisms may reduce the effectiveness of community-based screening. The study therefore expects accessible, affordable, culturally appropriate, and well-coordinated community-based hearing screening to contribute significantly to improved detection and early management of hearing loss among older adults in Nigeria. The study is expected to contribute to the literature on community-based hearing screening, hearing loss detection, older adult health, age-related hearing loss, audiological services, hearing rehabilitation, healthy ageing, community healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare centres, hospitals, audiologists, otolaryngologists, community health workers, elderly health programmes, development partners, and policymakers regarding strategies for improving hearing health among older adults. The study will also provide evidence-based recommendations for expanding community-based hearing screening programmes, improving access to functional screening equipment and trained audiologists, integrating hearing screening into primary healthcare and elderly health services, strengthening referral and follow-up systems, increasing awareness of age-related hearing loss, and improving access to appropriate hearing rehabilitation and assistive devices across Nigeria.
