Effect of Community-Based Lead Exposure Assessment on Lead Poisoning Detection among Children in Mining Communities in Nigeria
Abstract
Lead exposure is a serious environmental and public health concern among children living in mining communities in Nigeria, particularly in areas where mining, ore processing, transportation, and informal mineral processing activities may contaminate soil, dust, water, and household environments. Children are particularly vulnerable to the harmful effects of lead exposure, which may affect neurological development, cognitive function, behaviour, growth, and overall well-being. Lead poisoning may remain undetected because early exposure may not produce obvious symptoms, while affected families may have limited access to environmental health services, laboratory testing, and specialized healthcare. Community-based lead exposure assessment provides an opportunity to identify children and households at increased risk through environmental assessment, exposure-history evaluation, blood lead testing where available, household inspections, and community outreach services. Early detection can facilitate removal or reduction of exposure sources, clinical evaluation, nutritional support, environmental remediation, and appropriate follow-up. Against this background, this study investigates the effect of community-based lead exposure assessment on lead poisoning detection among children in mining communities in Nigeria. The study will be anchored on Environmental Health Theory, the Health Belief Model, and the Social Ecological Model. Environmental Health Theory emphasizes the relationship between environmental exposures and human health and supports systematic identification, assessment, and control of environmental hazards. The Health Belief Model explains how parents' or caregivers' perceptions of children's susceptibility to lead poisoning, perceived severity, perceived benefits of exposure assessment, perceived barriers, and cues to action may influence participation in community-based lead exposure assessment. The Social Ecological Model emphasizes the influence of individual, household, community, socioeconomic, occupational, environmental, and institutional factors on children's exposure to lead and access to preventive and healthcare services. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based lead exposure assessment may influence lead poisoning detection among children in mining communities in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise children aged 1–14 years residing in selected mining communities across Nigeria, with parents or caregivers providing relevant information. A multistage sampling technique will be used to select mining regions, states, local government areas, communities, households, and eligible children. Community-based lead exposure assessment will be measured using indicators such as exposure assessment coverage, frequency of community assessment activities, environmental assessment of soil and household dust, assessment of drinking-water sources, evaluation of proximity to mining and mineral-processing activities, household exposure-history assessment, assessment of children's contact with contaminated materials, availability of trained environmental health personnel, availability of blood lead testing, community education, and referral arrangements. Lead poisoning detection will be assessed using indicators such as elevated blood lead levels where testing is conducted, children identified as having increased exposure risk, abnormal exposure assessment findings, newly identified children requiring further clinical evaluation, referrals for confirmatory or clinical assessment, and documented lead poisoning where appropriately confirmed. Data will be collected using structured questionnaires, environmental assessment forms, household observation checklists, exposure-history instruments, laboratory records, blood lead test results, healthcare facility records, and referral registers. Where blood lead testing is available, samples will be collected and analysed using appropriate standardized procedures. Descriptive statistics will be used to summarize children's characteristics, household and environmental exposure patterns, assessment coverage, laboratory findings, and patterns of lead poisoning detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the effect of community-based lead exposure assessment on lead poisoning detection. Where appropriate, detection rates before and after implementation of community-based lead exposure assessment activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based lead exposure assessment has a significant positive effect on lead poisoning detection among children in mining communities in Nigeria. Children living in communities with systematic lead exposure assessment are expected to have a higher likelihood of previously unrecognized lead exposure or lead poisoning being identified than children without access to such assessment. Community-based assessment may facilitate early identification of contaminated environmental sources, high-risk household practices, children with elevated exposure levels, and children requiring further clinical evaluation. Early detection may enable appropriate interventions, including removal or reduction of exposure sources, environmental remediation, improved household hygiene, safe water and food practices, nutritional support, clinical management, and follow-up. Community education may also improve parents' and caregivers' awareness of lead exposure pathways and encourage practices that reduce children's contact with contaminated soil, dust, water, and mining-related materials. Conversely, inadequate laboratory capacity, shortage of trained personnel, financial constraints, difficult access to mining communities, low awareness, weak environmental monitoring, and inadequate follow-up services may reduce the effectiveness of community-based lead exposure assessment. The study therefore expects accessible and systematically implemented community-based lead exposure assessment to contribute significantly to improved detection and early management of lead poisoning among children in mining communities in Nigeria. The study is expected to contribute to the literature on community-based lead exposure assessment, lead poisoning detection, childhood environmental health, mining-community health, environmental toxicology, child health, occupational and environmental exposure, preventive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Environment, Federal Ministry of Solid Minerals Development, National Primary Health Care Development Agency, state ministries of health and environment, environmental health agencies, mining authorities, primary healthcare centres, community health workers, development partners, mining-community organizations, and policymakers regarding strategies for preventing childhood lead exposure. The study will also provide evidence-based recommendations for strengthening community-based lead exposure assessment, expanding access to blood lead testing, improving environmental monitoring of mining communities, strengthening laboratory capacity, increasing public education on lead exposure prevention, improving environmental remediation, strengthening referral and follow-up systems, and integrating lead exposure assessment into appropriate community-based environmental and child health programmes across mining communities in Nigeria.
