Effect of Eye Health Education on Eye Care-Seeking Practices among Adults in Rural Nigeria
Abstract
Eye health is an important component of overall health and quality of life, as untreated eye conditions may result in visual impairment, reduced productivity, limitations in daily activities, and avoidable blindness. Adults living in rural communities in Nigeria may face greater challenges in accessing eye care because of geographical distance, limited availability of eye-care professionals, financial constraints, inadequate transportation, low awareness, cultural beliefs, and reliance on informal sources of treatment. Eye health education provides an opportunity to improve knowledge about common eye conditions, warning signs, preventive practices, available eye-care services, and the importance of seeking professional care when visual problems occur. Against this background, this study investigates the effect of eye health education on eye care-seeking practices among adults in rural Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Health Literacy Framework. The Health Belief Model explains how adults' perceptions of susceptibility to eye conditions, perceived severity of visual problems, perceived benefits of professional eye care, perceived barriers, and cues to action may influence their care-seeking practices. The Social Ecological Model emphasizes the interaction between individual, family, community, healthcare, and environmental factors in shaping access to and utilization of eye-care services. The Health Literacy Framework emphasizes individuals' ability to access, understand, evaluate, and apply health information when making decisions about eye health and healthcare utilization. Collectively, these theoretical perspectives provide a suitable framework for explaining how eye health education may influence eye care-seeking practices among adults in rural Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adults aged 18 years and above residing in selected rural communities in Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, households, and eligible adults. Eye health education will be assessed using indicators such as exposure to structured eye health education, frequency of educational sessions, information on common eye conditions, recognition of eye symptoms and warning signs, prevention of avoidable visual impairment, importance of routine eye examinations, appropriate use of prescribed spectacles, risks of self-medication, dangers of harmful traditional eye practices, and information about available eye-care services. Eye care-seeking practices will be assessed using indicators such as seeking professional eye care when symptoms occur, utilization of eye clinics or hospitals, attendance for routine eye examinations, timely presentation following changes in vision, adherence to referrals, use of prescribed spectacles or medications, avoidance of inappropriate self-medication, and use of qualified eye-care professionals. Data will be collected using structured questionnaires, validated eye health knowledge and healthcare-seeking instruments, health education attendance records, eye-care utilization records where available, and relevant community health programme documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to eye health education, and patterns of eye care-seeking. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of eye health education on eye care-seeking practices. Where a quasi-experimental design is adopted, eye care-seeking practice scores before and after the educational intervention may be compared to determine changes associated with the programme. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that eye health education has a significant positive effect on eye care-seeking practices among adults in rural Nigeria. Adults exposed to structured eye health education are expected to demonstrate greater willingness to seek professional eye care when experiencing visual symptoms and to utilize appropriate eye-care services more frequently than adults with limited exposure to eye health information. Education may improve recognition of warning signs, reduce misconceptions about eye diseases, discourage harmful self-medication and unqualified treatment, and increase awareness of the availability and benefits of professional eye care. Community-based education delivered in locally appropriate languages and through trusted community structures may further improve acceptance and utilization of eye-care services. However, geographical distance, transportation costs, financial limitations, shortage of eye-care professionals, cultural beliefs, reliance on traditional remedies, and limited availability of eye-care facilities may continue to affect care-seeking practices. The study therefore expects accessible, culturally appropriate, and community-based eye health education combined with available eye-care services to contribute significantly to improved eye care-seeking practices among adults in rural Nigeria. The study is expected to contribute to the literature on eye health education, eye care-seeking practices, rural eye health, visual impairment prevention, health literacy, healthcare utilization, community health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, eye-care hospitals, primary healthcare centres, optometrists, ophthalmologists, community health workers, public health educators, development partners, and policymakers regarding strategies for improving eye health service utilization in rural communities. The study will also provide evidence-based recommendations for strengthening community eye health education, integrating eye health promotion into primary healthcare services, improving rural access to eye-care professionals, reducing financial and geographical barriers, strengthening referral systems, and increasing awareness of appropriate eye care and early treatment across rural Nigeria.
