Effect of Hospital Discharge Summaries on Follow-Up Care Attendance among Patients in Nigerian Hospitals

Abstract

Continuity of care following hospital discharge is an important component of effective healthcare delivery and patient safety. Discharge summaries provide essential information about a patient's diagnosis, treatment received, medications, clinical condition, investigations, referrals, and recommended follow-up care. However, incomplete, delayed, inaccurate, or poorly communicated discharge summaries may create gaps in information between hospitals, patients, caregivers, and healthcare providers, potentially affecting patients' ability to attend and receive appropriate follow-up care. In Nigeria, challenges such as inadequate documentation, poor communication between healthcare facilities, limited patient understanding of discharge instructions, transportation difficulties, financial constraints, and weak follow-up systems may further affect continuity of care. Effective and comprehensive hospital discharge summaries may improve patients' understanding of their ongoing healthcare needs and facilitate coordination between healthcare providers. Against this background, this study investigates the effect of hospital discharge summaries on follow-up care attendance among patients in Nigerian hospitals. The study will be anchored on the Donabedian Model of Healthcare Quality, the Transitional Care Model, and Andersen's Behavioral Model of Health Services Use. The Donabedian Model explains healthcare quality through structure, process, and outcome, with discharge documentation representing an important healthcare process and follow-up attendance serving as a measurable outcome. The Transitional Care Model emphasizes effective communication, patient preparation, and coordination of care during the transition from hospital to community or outpatient services. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors, with adequate discharge information functioning as an enabling factor that may facilitate continued healthcare utilization. Collectively, these theoretical perspectives provide a suitable framework for explaining how hospital discharge summaries may influence follow-up care attendance among patients in Nigerian hospitals. The study will adopt a quantitative analytical cross-sectional or prospective cohort research design. The study population will comprise adult patients discharged from selected public and private hospitals in Nigeria who have been scheduled for follow-up care. A multistage sampling technique will be used to select states, local government areas, hospitals, departments, and eligible patients. Hospital discharge summaries will be assessed using indicators such as availability of a discharge summary, completeness of patient identification, diagnosis, treatment information, medication details, investigation results, clinical condition at discharge, follow-up instructions, referral information, scheduled appointment dates, responsible healthcare provider, clarity of information, accuracy, and timeliness of summary completion. Follow-up care attendance will be assessed using indicators such as attendance at scheduled appointments, attendance within the recommended period, completion of referred consultations, attendance at specialist follow-up visits where indicated, laboratory or diagnostic follow-up, medication review appointments, missed appointments, delayed attendance, and loss to follow-up. Data will be collected using structured questionnaires, hospital discharge summaries, patient medical records, appointment registers, referral records, and follow-up documentation. Descriptive statistics will be used to summarize patients' demographic and clinical characteristics, discharge-summary quality, and follow-up attendance patterns. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of discharge-summary quality on follow-up care attendance. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that comprehensive and timely hospital discharge summaries have a significant positive effect on follow-up care attendance among patients in Nigerian hospitals. Patients who receive complete and clearly documented discharge summaries are expected to demonstrate higher rates of attendance at recommended follow-up appointments than patients whose discharge information is incomplete or unavailable. Clear documentation of diagnoses, medications, treatment plans, referrals, and appointment dates may improve patients' understanding of their continuing healthcare needs and facilitate communication between healthcare providers. Accurate discharge information may also reduce confusion during transitions between hospital and community-based care and support timely follow-up. However, poor documentation practices, delays in preparing discharge summaries, limited patient literacy, financial constraints, transportation difficulties, and weak communication between healthcare facilities may reduce the extent to which discharge summaries improve follow-up attendance. The study therefore expects standardized, accurate, complete, and timely discharge summaries to contribute significantly to improved follow-up care attendance among patients in Nigerian hospitals. The study is expected to contribute to the literature on hospital discharge summaries, follow-up care attendance, discharge documentation, continuity of care, transitional care, healthcare utilization, patient safety, clinical communication, and healthcare quality in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, healthcare facilities, physicians, nurses, pharmacists, medical records officers, health information managers, healthcare administrators, and policymakers regarding strategies for improving post-discharge continuity of care. The study will also provide evidence-based recommendations for standardizing discharge-summary formats, improving the completeness and accuracy of discharge documentation, ensuring timely preparation of summaries, strengthening patient and caregiver education, integrating electronic discharge-summary systems, and improving communication between hospitals and receiving healthcare facilities to promote effective follow-up care across Nigeria.
