Effect of Obstetric Referral Protocols on Maternal Emergency Care in Nigerian Hospitals
Abstract
Maternal emergencies remain a major public health concern in Nigeria and contribute substantially to preventable maternal morbidity and mortality. Conditions such as postpartum haemorrhage, severe pre-eclampsia and eclampsia, obstructed labour, sepsis, ruptured uterus, and other obstetric complications require rapid recognition, appropriate stabilization, and timely access to facilities with the capacity to provide comprehensive emergency obstetric care. However, delays in referral may occur because of inadequate identification of complications, poor communication between referring and receiving facilities, incomplete referral documentation, transportation challenges, limited availability of receiving facilities, and weaknesses in referral coordination. Obstetric referral protocols provide standardized procedures for identifying women requiring higher-level care, initiating referral, communicating clinical information, stabilizing patients before transfer, arranging transportation, and ensuring continuity of care at the receiving facility. Effective referral protocols may therefore improve the speed, coordination, and quality of emergency maternal care. Maternal emergency care refers to the timely provision of appropriate clinical assessment, stabilization, treatment, surgical intervention, blood transfusion, and other emergency obstetric services required to manage life-threatening maternal conditions. Against this background, this study investigates the effect of obstetric referral protocols on maternal emergency care in Nigerian hospitals. The study will be anchored on the Three Delays Model, the Health Systems Framework, and Andersen's Behavioral Model of Health Services Use. The Three Delays Model explains maternal health outcomes through delays in deciding to seek care, delays in reaching an appropriate healthcare facility, and delays in receiving adequate care after arrival. The Health Systems Framework emphasizes service delivery, health workforce, health information, essential medicines and technologies, financing, and accessibility as important components of an effective healthcare system. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for understanding access to emergency maternal healthcare. Collectively, these theoretical perspectives provide a suitable framework for explaining how obstetric referral protocols may influence maternal emergency care in Nigerian hospitals. The study will adopt a quantitative retrospective or cross-sectional research design, depending on the availability of hospital referral and emergency-care records. The study population will comprise pregnant women and recently delivered women who are referred for emergency obstetric care in selected Nigerian hospitals, together with relevant healthcare providers and referral records where appropriate. A multistage sampling technique will be used to select states, hospitals, maternity units, and eligible participants or records. Obstetric referral protocols will be assessed using indicators such as availability of written referral guidelines, standardized referral forms, referral decision procedures, communication between referring and receiving facilities, pre-referral stabilization, transportation arrangements, referral documentation, feedback mechanisms, referral coordination, and staff awareness of referral procedures. Maternal emergency care will be assessed using indicators such as time from referral decision to arrival at the receiving facility, time from arrival to emergency assessment, time to initiation of treatment, availability of emergency obstetric interventions, maternal complications, emergency surgical intervention, blood transfusion where indicated, referral completion, and maternal outcomes. Data will be obtained from structured questionnaires, referral registers, patient records, emergency department records, maternity registers, theatre records, and relevant hospital documents. Descriptive statistics will be used to summarize referral protocol implementation and maternal emergency-care patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, logistic regression, and other appropriate regression methods, will be used to determine the effect of obstetric referral protocols on maternal emergency care. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective implementation of obstetric referral protocols has a significant positive effect on maternal emergency care in Nigerian hospitals. Hospitals with clear and consistently implemented referral protocols are expected to demonstrate shorter referral delays, improved communication between healthcare facilities, more complete referral documentation, faster emergency assessment, and more timely initiation of appropriate treatment. Standardized referral procedures may also improve pre-referral stabilization and ensure that receiving facilities are adequately informed and prepared for incoming emergencies. These improvements may contribute to better maternal outcomes and reduce preventable complications associated with delayed emergency care. Conversely, inadequate referral coordination, poor communication, incomplete documentation, transportation difficulties, insufficient staff awareness, limited emergency resources, and weak referral feedback mechanisms may reduce the effectiveness of referral protocols. The study may also demonstrate that the effectiveness of referral protocols depends partly on the capacity of receiving hospitals to provide timely emergency obstetric interventions. The study is expected to contribute to the literature on obstetric referral protocols, maternal emergency care, emergency obstetric services, maternal morbidity and mortality, health-system performance, maternal health, reproductive health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospital administrators, obstetricians, midwives, emergency healthcare providers, maternal health programme managers, development partners, and policymakers regarding strategies for strengthening maternal emergency referral systems. The study will also provide evidence-based recommendations for standardizing obstetric referral protocols, improving communication between referring and receiving facilities, strengthening pre-referral stabilization, improving emergency transportation arrangements, ensuring availability of referral documentation, establishing effective referral feedback mechanisms, training healthcare workers on referral procedures, and strengthening emergency obstetric capacity in Nigerian hospitals.
