Effect of Perineal Care Education on Perineal Care Practices among Women after Vaginal Delivery in Nigeria

 

Abstract

Perineal care is an important component of postpartum care for women who undergo vaginal delivery, particularly those who experience perineal tears, episiotomy, swelling, bruising, or other forms of perineal trauma. Appropriate perineal care can promote wound healing, reduce discomfort, maintain hygiene, and help prevent infection and other postpartum complications. However, some women may have inadequate knowledge of appropriate perineal hygiene and care practices after childbirth due to insufficient postpartum education, cultural practices, misconceptions, discomfort discussing perineal health, limited access to healthcare providers, and inadequate follow-up support. Perineal care education provides an opportunity to equip women with practical knowledge about hygiene, wound care, pain management, warning signs, and appropriate healthcare-seeking after vaginal delivery. Against this background, this study investigates the effect of perineal care education on perineal care practices among women after vaginal delivery in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Self-Efficacy Theory. The Health Belief Model explains how women's perceptions of their susceptibility to perineal infection and other complications, perceived severity, perceived benefits of appropriate care, perceived barriers, and cues to action may influence their perineal care practices. Social Cognitive Theory emphasizes observational learning, practical demonstration, behavioural reinforcement, environmental influences, and social support in the development of appropriate self-care practices. Self-Efficacy Theory emphasizes women's confidence in their ability to perform perineal care correctly, recognize warning signs, manage discomfort, and seek appropriate professional care. Collectively, these theoretical perspectives provide a suitable framework for explaining how perineal care education may influence perineal care practices among women following vaginal delivery in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise women who have undergone vaginal delivery and are receiving postnatal care at selected hospitals, primary healthcare centres, maternity facilities, and postnatal clinics across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, and eligible postpartum women. Perineal care education will be assessed using indicators such as exposure to structured education, timing and duration of education, personal hygiene instructions, handwashing, appropriate cleaning techniques, sanitary pad changing, care of perineal wounds where applicable, pain-management advice, instructions regarding sitting and physical activities, recognition of abnormal discharge or bleeding, infection warning signs, medication adherence, and follow-up instructions. Perineal care practices will be assessed using indicators such as hand hygiene before and after perineal care, appropriate cleaning and drying techniques, frequency of sanitary pad changes, appropriate disposal of used materials, adherence to prescribed medications, avoidance of potentially harmful substances, appropriate wound-care practices where applicable, monitoring for signs of infection, compliance with follow-up appointments, and timely healthcare-seeking when complications occur. Data will be collected using structured questionnaires, perineal care practice checklists, postnatal records, discharge instructions, follow-up registers, and relevant healthcare documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to perineal care education, and patterns of perineal care practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of perineal care education on perineal care practices. Where a quasi-experimental design is adopted, perineal care practice scores before and after the educational intervention may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that perineal care education has a significant positive effect on perineal care practices among women after vaginal delivery in Nigeria. Women who receive structured, practical, and culturally appropriate perineal care education are expected to demonstrate better hygiene and self-care practices than women who receive routine postpartum instructions alone. Education may improve women's understanding of appropriate perineal cleaning, sanitary pad use, wound care, pain management, and recognition of infection or other warning signs. Practical demonstrations and individualized counselling may further improve women's confidence in performing perineal care correctly and seeking professional attention when necessary. However, cultural practices, misconceptions, postpartum discomfort, limited privacy, inadequate social support, financial barriers, early discharge, and limited access to follow-up services may influence women's ability to maintain appropriate perineal care practices. The study therefore expects accessible, practical, individualized, and continuous perineal care education to contribute significantly to improved perineal care practices and safer postpartum recovery in Nigeria. The study is expected to contribute to the literature on perineal care education, postpartum self-care, perineal hygiene, vaginal delivery, postpartum wound care, maternal health education, postnatal care, infection prevention, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, maternity facilities, nurses, midwives, community health workers, maternal health organizations, development partners, and policymakers regarding strategies for strengthening postpartum care. The study will also provide evidence-based recommendations for integrating structured perineal care education into postnatal services, providing practical demonstrations before discharge, training healthcare providers in effective postpartum counselling, developing culturally appropriate educational materials, strengthening postnatal follow-up, and improving access to professional care for women experiencing perineal complications after vaginal delivery across Nigeria.
