Effect of School Health Services on Health Service Utilization among Secondary School Students in Nigeria

Abstract

School health services are an important component of school-based healthcare because they provide opportunities for promoting health, preventing disease, identifying health problems early, and facilitating access to appropriate healthcare among students. Secondary school students may experience health problems such as infectious diseases, injuries, reproductive health concerns, nutritional problems, mental health challenges, vision and hearing difficulties, and other conditions that may affect their wellbeing and academic participation. However, utilization of health services among secondary school students in Nigeria may be influenced by limited availability of school health services, inadequate health personnel, poor awareness, financial constraints, concerns about confidentiality, stigma, and weak referral systems. School health services can provide accessible health education, screening, first aid, counselling, preventive services, basic treatment, referral, and follow-up within the school environment. Effective implementation of these services may encourage students to seek appropriate healthcare when health needs arise. Against this background, this study investigates the effect of school health services on health service utilization among secondary school students in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Donabedian Model of Healthcare Quality. The Health Belief Model explains how students' perceptions of susceptibility to health problems, perceived severity, perceived benefits of seeking care, perceived barriers, and cues to action may influence their utilization of available health services. The Social Ecological Model emphasizes the influence of individual, family, peer, school, community, and broader environmental factors on students' access to and utilization of healthcare services. The Donabedian Model explains healthcare quality through structure, process, and outcome, with the availability and delivery of school health services representing important structural and process components and health service utilization serving as a measurable outcome. Collectively, these theoretical perspectives provide a suitable framework for explaining how school health services may influence health service utilization among secondary school students in Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise students enrolled in selected public and private secondary schools in Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, schools, classes, and eligible students. School health services will be assessed using indicators such as availability of school health personnel, school clinics or sick bays, health screening, first-aid services, health education, immunization support, counselling services, reproductive health information, nutrition services, mental health support, disease prevention activities, referral services, follow-up arrangements, health records, accessibility of services, confidentiality, frequency of health activities, and availability of essential first-aid and healthcare supplies. Health service utilization will be assessed using indicators such as attendance at school health clinics, use of first-aid services, participation in health screening, utilization of counselling services, uptake of preventive health services, use of referral services, seeking healthcare when ill or injured, follow-up attendance after referral, and frequency of accessing available school or external healthcare services. Data will be collected using structured questionnaires, school health records, clinic or sick-bay registers, referral records, screening records, health education records, and relevant school documents. Descriptive statistics will be used to summarize students' demographic characteristics, availability of school health services, health needs, and patterns of service utilization. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of school health services on health service utilization. Where a quasi-experimental design is adopted, health service utilization before and after implementation of enhanced school health services may be compared. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective and accessible school health services have a significant positive effect on health service utilization among secondary school students in Nigeria. Students attending schools with functional clinics or sick bays, trained health personnel, regular health screening, health education, counselling, first-aid services, and effective referral systems are expected to demonstrate greater utilization of appropriate health services than students in schools with limited or poorly organized health services. Convenient access to healthcare within the school environment may reduce delays in seeking care and enable early identification and management of health problems. Health education and counselling may also improve students' awareness of available services and encourage appropriate healthcare-seeking behaviour. Confidentiality and student-friendly services may be particularly important in encouraging utilization of sensitive services. However, inadequate funding, shortage of trained school health personnel, limited healthcare supplies, poor infrastructure, stigma, concerns about confidentiality, weak referral systems, and low awareness may reduce utilization. The study therefore expects accessible, comprehensive, student-friendly, and adequately supported school health services to contribute significantly to improved health service utilization among secondary school students in Nigeria. The study is expected to contribute to the literature on school health services, health service utilization, adolescent health, school-based healthcare, healthcare-seeking behaviour, preventive healthcare, student wellbeing, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, state ministries of health and education, school management authorities, school health personnel, healthcare providers, parents, development partners, and policymakers regarding strategies for strengthening school-based healthcare. The study will also provide evidence-based recommendations for establishing and strengthening functional school health units, increasing the availability of trained school health personnel, expanding routine health screening and health education, improving counselling and referral services, strengthening confidentiality and student-friendly healthcare practices, ensuring adequate healthcare supplies, and integrating school health services into broader adolescent health and primary healthcare programmes across Nigeria.
