Effect of Testicular Health Education on Testicular Self-Examination Knowledge among Male University Students in Nigeria
Abstract
Testicular health is an important component of men's health, particularly among adolescents and young adult men. Testicular conditions, including testicular cancer, infections, trauma, torsion, and other abnormalities, may require prompt recognition and appropriate medical evaluation. University students may have limited knowledge of testicular health, warning signs, risk factors, and appropriate methods of conducting testicular self-examination. Limited awareness and embarrassment about discussing reproductive health may also discourage young men from seeking information or reporting unusual changes. Testicular health education provides an opportunity to improve students' knowledge of testicular anatomy, common testicular conditions, warning signs, appropriate self-examination techniques, and the importance of seeking professional medical evaluation when abnormalities are identified. Against this background, this study investigates the effect of testicular health education on testicular self-examination knowledge among male university students in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Health Literacy Framework. The Health Belief Model explains how students' perceptions of susceptibility to testicular health problems, perceived severity, perceived benefits of self-examination, perceived barriers, and cues to action may influence their knowledge and willingness to engage in preventive practices. Social Cognitive Theory emphasizes observational learning, self-efficacy, behavioural reinforcement, and practical skill development in the acquisition of health-related knowledge and behaviours. The Health Literacy Framework emphasizes students' ability to access, understand, evaluate, and appropriately apply health information concerning testicular health and self-examination. Collectively, these theoretical perspectives provide a suitable framework for explaining how testicular health education may influence testicular self-examination knowledge among male university students in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise male undergraduate students enrolled in selected public and private universities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, universities, faculties or departments, and eligible students. Testicular health education will be assessed using indicators such as exposure to structured educational sessions, frequency and duration of education, knowledge of testicular anatomy, common testicular conditions, risk factors, warning signs, appropriate timing and procedure for testicular self-examination, recognition of abnormal findings, and appropriate healthcare-seeking pathways. Testicular self-examination knowledge will be assessed using indicators such as knowledge of the purpose of self-examination, understanding of the appropriate examination procedure, knowledge of how to identify unusual lumps or swelling, recognition of changes in size or shape, awareness of pain or discomfort that may require medical evaluation, knowledge of appropriate reporting and referral procedures, and ability to correctly respond to testicular health scenarios. Data will be collected using structured questionnaires, validated men's reproductive-health knowledge instruments, testicular self-examination knowledge assessments, and educational programme attendance records. Descriptive statistics will be used to summarize participants' characteristics, exposure to testicular health education, and self-examination knowledge scores. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of testicular health education on testicular self-examination knowledge. Where a quasi-experimental design is adopted, knowledge scores before and after the educational intervention may be compared to determine changes associated with the programme. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that testicular health education has a significant positive effect on testicular self-examination knowledge among male university students in Nigeria. Students exposed to structured and practical health education are expected to demonstrate improved understanding of testicular anatomy, common testicular conditions, warning signs, the appropriate principles of self-examination, and the importance of seeking professional evaluation when abnormalities are noticed. Demonstrations, visual educational materials, peer education, interactive discussions, and practical instruction may improve students' confidence in understanding testicular health and recognizing abnormal changes. Education may also reduce embarrassment and misconceptions surrounding men's reproductive health and encourage timely communication with healthcare professionals. However, cultural attitudes, embarrassment, limited access to reliable reproductive-health information, misconceptions about self-examination, and reluctance to discuss testicular health may affect students' knowledge. The study therefore expects accessible, culturally sensitive, age-appropriate, and practically oriented testicular health education to contribute significantly to improved testicular self-examination knowledge among male university students in Nigeria. The study is expected to contribute to the literature on testicular health education, testicular self-examination knowledge, men's reproductive health, university student health, cancer awareness, preventive healthcare, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, universities, university health services, healthcare professionals, urologists, reproductive-health educators, student organizations, youth-focused organizations, development partners, and policymakers regarding strategies for improving awareness of testicular health among young men. The study will also provide evidence-based recommendations for integrating testicular health education into university health programmes, strengthening men's reproductive-health education, improving access to reliable health information, promoting appropriate awareness of testicular abnormalities, and encouraging timely professional medical evaluation of concerning findings among male university students across Nigeria.
