Impact of Accessibility Audits on Disability-Friendly Practices in Nigerian Primary Healthcare Facilities

 

Abstract

Accessibility of healthcare facilities is an important component of equitable and inclusive healthcare delivery for persons with disabilities. Inadequate physical infrastructure, inaccessible entrances, unsuitable toilets, poor signage, limited communication support, inappropriate medical equipment, and insufficient disability awareness among healthcare workers may create barriers that discourage persons with disabilities from accessing primary healthcare services. Accessibility audits provide a systematic approach for identifying barriers within healthcare facilities and assessing compliance with disability-inclusive standards. By identifying gaps and providing a basis for corrective action, accessibility audits may support improvements in physical accessibility, communication, service delivery, and disability-friendly practices. However, limited implementation of accessibility assessments, inadequate funding, weak monitoring mechanisms, and insufficient knowledge of disability-inclusive healthcare standards may affect the adoption of recommended improvements in Nigerian primary healthcare facilities. Against this background, this study investigates the impact of accessibility audits on disability-friendly practices in Nigerian primary healthcare facilities. The study will be anchored on the Social Ecological Model, the Health Systems Framework, and the Social Model of Disability. The Social Ecological Model explains how individual, organizational, community, and environmental factors interact to influence accessibility and inclusion within healthcare facilities. The Health Systems Framework emphasizes service delivery, healthcare workforce, health information, medical products and technologies, financing, and accessibility as essential components of an effective healthcare system. The Social Model of Disability emphasizes that disability-related limitations may be increased by environmental, institutional, and social barriers rather than by an individual's impairment alone. Collectively, these theoretical perspectives provide a suitable framework for explaining how accessibility audits may influence disability-friendly practices in Nigerian primary healthcare facilities. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise selected public and private primary healthcare facilities across Nigeria, including healthcare managers, healthcare workers, and relevant facility personnel responsible for service delivery and facility management. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, primary healthcare facilities, and eligible healthcare personnel. Accessibility audits will be assessed using indicators such as availability of accessible entrances and ramps, accessible toilets, appropriate doorways and corridors, clear signage, accessible waiting areas, appropriate seating, availability of disability-friendly medical equipment, communication support, availability of sign-language assistance where required, accessible health information, emergency accessibility, staff disability-awareness training, and compliance with relevant accessibility standards. Disability-friendly practices will be assessed using indicators such as respectful treatment of persons with disabilities, provision of reasonable accommodations, accessible communication, priority assistance, appropriate patient registration procedures, inclusive clinical practices, confidentiality, patient participation in healthcare decisions, referral support, staff responsiveness, and implementation of disability-inclusive policies. Data will be collected using structured questionnaires, standardized facility accessibility audit checklists, direct observation, interviews with healthcare workers and facility managers, facility records, and relevant policy documents. Descriptive statistics will be used to summarize facility characteristics, accessibility audit findings, and levels of disability-friendly practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of accessibility audits on disability-friendly practices. Where a quasi-experimental design is adopted, disability-friendly practice scores before and after accessibility audits and implementation of corrective measures may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that accessibility audits have a significant positive impact on disability-friendly practices in Nigerian primary healthcare facilities. Facilities that undergo systematic accessibility audits are expected to demonstrate improved identification and correction of physical, communication, and service-delivery barriers affecting persons with disabilities. Regular audits may encourage healthcare managers and workers to improve facility accessibility, strengthen communication practices, provide reasonable accommodations, and implement disability-inclusive policies. Accessibility assessments may also improve staff awareness of disability-related healthcare needs and promote greater institutional accountability for inclusive service delivery. However, inadequate funding, structural limitations in older facilities, limited technical expertise, weak implementation of audit recommendations, shortage of accessible equipment, and insufficient monitoring may limit the effectiveness of accessibility audits. The study therefore expects regular, standardized, participatory, and adequately monitored accessibility audits to contribute significantly to improved disability-friendly practices in Nigerian primary healthcare facilities. The study is expected to contribute to the literature on accessibility audits, disability-friendly healthcare, disability inclusion, primary healthcare, healthcare accessibility, health equity, universal health coverage, and inclusive healthcare services in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, local government health authorities, primary healthcare facility managers, healthcare workers, disability organizations, development partners, and policymakers regarding strategies for improving healthcare accessibility for persons with disabilities. The study will also provide evidence-based recommendations for institutionalizing routine accessibility audits, strengthening monitoring and implementation of audit recommendations, improving physical infrastructure and accessible medical equipment, training healthcare workers in disability-inclusive practices, developing facility-level accessibility improvement plans, and integrating disability inclusion into primary healthcare quality-improvement programmes across Nigeria.
