Impact of Community-Based Eye Screening on Visual Impairment Detection among Older Adults in Nigeria
Abstract
Visual impairment remains an important public health concern among older adults in Nigeria and may result from preventable or treatable conditions such as cataract, glaucoma, refractive errors, diabetic eye disease, age-related macular degeneration, and other ocular disorders. As the population of older adults increases, the burden of age-related eye conditions is expected to place greater demands on healthcare services and may contribute to reduced independence, falls, social isolation, difficulty performing daily activities, and reduced quality of life. Early detection of visual impairment and underlying eye disorders is essential for timely treatment, referral, rehabilitation, and prevention of avoidable vision loss. Community-based eye screening provides an opportunity to bring basic eye assessment services closer to older adults who may face financial, geographical, mobility, or other barriers to accessing conventional eye-care facilities. Through community outreach programmes, primary healthcare centres, mobile screening services, and other community platforms, individuals at risk of visual impairment can be identified and referred for comprehensive eye examination. However, inadequate screening coverage, shortage of trained personnel, limited diagnostic equipment, low awareness, transportation difficulties, and poor referral systems may reduce the effectiveness of community-based eye screening programmes in Nigeria. Against this background, this study investigates the impact of community-based eye screening on visual impairment detection among older adults in Nigeria. The study will be anchored on the Health Belief Model, Community Health Theory, and Health Systems Framework. The Health Belief Model explains how older adults' perceptions of susceptibility to visual impairment, perceived severity, perceived benefits of eye screening, perceived barriers, and cues to action may influence participation in community-based eye screening services. Community Health Theory emphasizes the importance of delivering health services through accessible community structures and involving local populations in disease prevention, health promotion, and early detection activities. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective healthcare delivery. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based eye screening may influence visual impairment detection among older adults in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise adults aged 60 years and above residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, households, and eligible older adults. Community-based eye screening will be measured using indicators such as availability of screening programmes, screening coverage, frequency of outreach activities, accessibility of screening locations, number of older adults screened, availability of trained eye health personnel, availability of screening equipment, and referral arrangements. Visual impairment detection will be assessed using indicators such as visual acuity assessment results, number of older adults identified with visual impairment, severity of visual impairment, detection of suspected cataract or glaucoma, identification of refractive errors and other eye conditions, confirmation through comprehensive eye examination, and referral for appropriate treatment. Data will be collected using structured questionnaires, visual acuity screening records, community health registers, eye screening reports, referral records, healthcare facility records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, and visual impairment detection patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of community-based eye screening on visual impairment detection. Where appropriate, detection rates before and after community-based screening activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based eye screening has a significant positive impact on visual impairment detection among older adults in Nigeria. Older adults who have access to community-based eye screening are expected to have a higher likelihood of having visual impairment and suspected eye disorders identified than those without access to such services. Community-based screening may improve early identification of reduced visual acuity, cataract, glaucoma, refractive errors, diabetic eye disease, and other conditions that could otherwise remain undiagnosed. Bringing screening services closer to older adults may reduce geographical and mobility barriers and increase opportunities for timely referral to appropriate eye-care facilities. Early detection may also facilitate treatment of preventable or treatable conditions and reduce the progression of avoidable visual impairment. Conversely, inadequate screening coverage, limited availability of trained eye health personnel, insufficient screening equipment, poor referral systems, transportation challenges, and low awareness may reduce the effectiveness of community-based eye screening. The study therefore expects well-organized and accessible community-based eye screening to contribute significantly to improved detection of visual impairment among older adults in Nigeria. The study is expected to contribute to the literature on community-based eye screening, visual impairment, older adult health, eye health, early disease detection, community health services, preventive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, National Eye Health Programme, ophthalmologists, optometrists, primary healthcare providers, community health workers, local government health authorities, development partners, and policymakers regarding strategies for improving eye health among older adults. The study will also provide evidence-based recommendations for expanding community-based eye screening programmes, improving access to basic eye screening services, training community health workers in appropriate eye health assessment, increasing availability of screening equipment, strengthening referral pathways, improving public awareness of age-related eye conditions, reducing barriers to specialist eye care, and integrating eye screening into existing primary healthcare and older adult health programmes across Nigeria.
