Impact of Community-Based Prostate Health Screening on Prostate Disorder Detection among Older Men in Nigeria

 

Abstract

Prostate disorders remain an important health concern among older men in Nigeria, particularly as ageing is associated with an increased likelihood of developing prostate-related conditions such as benign prostatic enlargement, prostatitis, and prostate cancer. Some prostate disorders may develop gradually and may not produce noticeable symptoms during their early stages, resulting in delayed detection and presentation for medical care. Undetected prostate disorders may contribute to urinary difficulties, recurrent complications, impaired quality of life, and increased healthcare needs. Prostate cancer is also an important concern because delayed identification may result in diagnosis at a more advanced stage and reduce opportunities for timely management. Community-based prostate health screening provides an opportunity to identify prostate-related abnormalities among older men through accessible health outreach programmes, primary healthcare centres, community health campaigns, mobile screening services, and other community platforms. Screening may involve assessment of urinary symptoms and risk factors, prostate-specific antigen testing where clinically appropriate, physical examination, and referral for further evaluation. Early identification of abnormal findings may facilitate timely clinical assessment, diagnostic investigations, treatment, and appropriate follow-up. However, limited awareness, financial barriers, geographical inaccessibility, fear of diagnosis, stigma, shortage of trained healthcare personnel, limited diagnostic facilities, and weak referral and follow-up systems may restrict participation in community-based prostate health screening in Nigeria. Against this background, this study investigates the impact of community-based prostate health screening on prostate disorder detection among older men in Nigeria. The study will be anchored on the Health Belief Model, Community Health Theory, and the Health Systems Framework. The Health Belief Model explains how older men's perceptions of susceptibility to prostate disorders, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in community-based prostate health screening. Community Health Theory emphasizes the delivery of accessible preventive health services through community structures and active community participation in health promotion, disease prevention, and early detection. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective prostate health screening and referral programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based prostate health screening may influence prostate disorder detection among older men in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise older men aged 60 years and above residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, households, and eligible older men. Community-based prostate health screening will be measured using indicators such as availability of screening services, screening coverage, frequency of community screening activities, accessibility of screening locations, assessment of urinary symptoms and prostate-related risk factors, prostate-specific antigen testing where clinically appropriate, physical prostate assessment where indicated, availability of trained healthcare personnel, screening equipment, prostate health education, and referral arrangements. Prostate disorder detection will be assessed using indicators such as abnormal prostate-specific antigen findings where tested, abnormal prostate examination findings where clinically indicated, newly identified prostate abnormalities, symptoms suggestive of prostate disorders, suspected benign prostatic enlargement, prostatitis or other prostate conditions, referrals for further diagnostic assessment, specialist evaluation, confirmatory diagnosis where appropriate, and linkage to treatment and follow-up services. Data will be collected using structured questionnaires, community screening registers, screening assessment forms, laboratory reports, healthcare facility records, referral registers, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, prostate health findings, and patterns of prostate disorder detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of community-based prostate health screening on prostate disorder detection. Where appropriate, prostate disorder detection rates before and after implementation of community-based screening activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based prostate health screening has a significant positive impact on prostate disorder detection among older men in Nigeria. Older men who participate in community-based prostate health screening are expected to have a higher likelihood of having previously undetected prostate abnormalities identified than older men without access to such services. Community-based screening may facilitate early recognition of urinary symptoms and abnormal screening findings and provide opportunities for further clinical evaluation and appropriate diagnostic assessment. Bringing prostate health services closer to older men may reduce geographical, financial, and time-related barriers and encourage men who may otherwise avoid routine healthcare to participate in preventive health assessments. Early detection may support timely management of prostate disorders and reduce the likelihood of complications associated with delayed presentation. Community-based prostate health education may also improve awareness of prostate health, warning signs, risk factors, and the importance of seeking appropriate medical evaluation. Conversely, inadequate screening coverage, limited diagnostic facilities, shortage of trained healthcare personnel, high screening costs, fear of diagnosis, stigma, low participation, and weak referral and follow-up systems may reduce the effectiveness of community-based prostate health screening. The study therefore expects accessible and well-organized community-based prostate health screening to contribute significantly to improved detection and early management of prostate disorders among older men in Nigeria. The study is expected to contribute to the literature on community-based prostate health screening, prostate disorder detection, older men's health, prostate health, prostate cancer prevention, benign prostatic disorders, community health services, preventive healthcare, healthy ageing, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare centres, hospitals, urologists, oncologists, community health workers, geriatric health programmes, development partners, and policymakers regarding strategies for improving prostate health among older men. The study will also provide evidence-based recommendations for expanding community-based prostate health screening programmes, improving awareness of prostate disorders, increasing access to appropriate screening and diagnostic services, strengthening training and deployment of healthcare personnel, reducing financial and geographical barriers to screening, improving referral and follow-up systems, strengthening linkage to specialist care, and integrating prostate health assessment into appropriate community-based healthy ageing programmes across Nigeria.
