Impact of Community-Based Rehabilitation on Functional Independence among Persons with Disabilities in Nigeria

 

Abstract

Persons with disabilities in Nigeria may experience significant limitations in mobility, self-care, communication, education, employment, and participation in community life due to physical, sensory, intellectual, or developmental impairments. Limited access to rehabilitation services, inadequate specialized healthcare facilities, shortage of rehabilitation professionals, financial constraints, transportation difficulties, social barriers, and limited availability of assistive devices may further restrict the ability of persons with disabilities to achieve functional independence. Community-based rehabilitation provides an approach to delivering rehabilitation and support services within or close to the communities where persons with disabilities live. Such services may include physical rehabilitation, occupational therapy, speech and communication support, provision of assistive devices, skills training, health education, referral services, and support for social and economic participation. By bringing rehabilitation closer to individuals and involving families, community health workers, local organizations, and other community stakeholders, community-based rehabilitation may improve access to appropriate rehabilitation services and promote greater independence in daily activities. However, inadequate programme coverage, limited funding, shortages of trained personnel, insufficient assistive devices, poor referral systems, and weak community participation may limit the effectiveness of community-based rehabilitation services in Nigeria. Against this background, this study investigates the impact of community-based rehabilitation on functional independence among persons with disabilities in Nigeria. The study will be anchored on the Social Model of Disability, the International Classification of Functioning, Disability and Health (ICF) Framework, and the Health Systems Framework. The Social Model of Disability emphasizes that disability is influenced not only by individual impairments but also by environmental, social, and institutional barriers that restrict participation and independence. The ICF Framework provides a comprehensive approach to understanding functioning and disability through the interaction of health conditions, body functions and structures, activities, participation, environmental factors, and personal factors. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective rehabilitation services. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based rehabilitation may influence functional independence among persons with disabilities in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise persons with disabilities residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, rehabilitation programmes, and eligible participants with disabilities. Community-based rehabilitation will be measured using indicators such as availability of rehabilitation services within the community, frequency of rehabilitation sessions, access to physical rehabilitation, occupational therapy, speech and communication support, provision and use of assistive devices, rehabilitation counselling, home-based rehabilitation support, referral services, vocational support, and participation of family or community members in rehabilitation activities. Functional independence will be assessed using indicators such as ability to perform activities of daily living, mobility, self-care, communication, use of assistive devices, ability to perform household activities, participation in community activities, and level of dependence on caregivers. Data will be collected using structured questionnaires, functional assessment tools, rehabilitation records, assistive-device records, healthcare facility records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, rehabilitation service utilization, and levels of functional independence. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of community-based rehabilitation on functional independence. Where appropriate, functional independence scores before and after participation in community-based rehabilitation programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based rehabilitation has a significant positive impact on functional independence among persons with disabilities in Nigeria. Persons with disabilities who participate in community-based rehabilitation programmes are expected to demonstrate greater ability to perform activities of daily living, mobility, self-care, communication, household activities, and community participation than individuals with limited or no access to rehabilitation services. Regular rehabilitation support may improve physical functioning, develop adaptive skills, facilitate appropriate use of assistive devices, and reduce dependence on caregivers. Community-based rehabilitation may also improve access to rehabilitation services by reducing geographical and transportation barriers associated with facility-based rehabilitation. Conversely, inadequate rehabilitation coverage, insufficient trained personnel, limited availability of assistive devices, irregular rehabilitation sessions, financial constraints, poor referral systems, and inadequate community support may reduce the effectiveness of rehabilitation programmes. The study therefore expects accessible, continuous, and appropriately designed community-based rehabilitation services to contribute significantly to improved functional independence among persons with disabilities in Nigeria. The study is expected to contribute to the literature on community-based rehabilitation, disability health, functional independence, rehabilitation services, assistive technology, disability inclusion, community health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, National Commission for Persons with Disabilities, state ministries of health, rehabilitation centres, physiotherapists, occupational therapists, speech therapists, community health workers, disability organizations, development partners, and policymakers regarding strategies for strengthening rehabilitation services. The study will also provide evidence-based recommendations for expanding community-based rehabilitation coverage, increasing the availability of trained rehabilitation professionals, improving access to affordable assistive devices, strengthening community and family participation, improving rehabilitation referral systems, integrating disability rehabilitation into primary healthcare services, increasing disability-inclusive health financing, and strengthening community support programmes to promote greater functional independence and participation among persons with disabilities across Nigeria.
