Impact of Community Health Champions on Participation in Preventive Health Activities among Rural Residents in Nigeria

 

Abstract

Participation in preventive health activities is essential for reducing the burden of preventable diseases, promoting early detection of health conditions, and improving community health outcomes. Rural residents in Nigeria may experience barriers to participation in preventive health activities because of limited health awareness, geographical distance from healthcare facilities, financial constraints, cultural beliefs, mistrust of health services, competing livelihood responsibilities, and inadequate community engagement. Community health champions, who may include trained community volunteers, respected community members, peer educators, or other locally recognized health advocates, can serve as a link between healthcare providers and community residents by providing health information, encouraging participation, promoting preventive practices, and directing residents to appropriate health services. Against this background, this study investigates the impact of community health champions on participation in preventive health activities among rural residents in Nigeria. The study will be anchored on the Social Ecological Model, Social Cognitive Theory, and Health Belief Model. The Social Ecological Model emphasizes the influence of individual, interpersonal, community, organizational, and environmental factors on participation in preventive health activities. Social Cognitive Theory explains how observation, social modelling, reinforcement, interpersonal interaction, and self-efficacy may influence residents' preventive health behaviours. The Health Belief Model explains how perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action may influence individuals' decisions to participate in preventive health activities. Collectively, these theoretical perspectives provide a suitable framework for explaining how community health champions may influence preventive health participation among rural residents in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adults aged 18 years and above residing in selected rural communities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, rural communities, households, and eligible participants. Community health champion activities will be assessed using indicators such as number of active health champions, training received, frequency of community engagement, household visits, health education activities, community mobilization, referral activities, health-message dissemination, follow-up activities, collaboration with primary healthcare facilities, and community members' accessibility to champions. Participation in preventive health activities will be assessed using indicators such as attendance at health screening programmes, immunization activities where applicable, health education sessions, maternal and child health services, family planning services, infectious disease prevention activities, chronic disease screening, sanitation activities, health campaigns, and other preventive programmes relevant to the participating communities. Data will be collected using structured questionnaires, community health champion activity records, attendance registers, household visit records, referral records, primary healthcare programme documents, and relevant community health registers. Descriptive statistics will be used to summarize participants' characteristics, exposure to community health champions, and patterns of preventive health participation. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of community health champions on participation in preventive health activities. Where a quasi-experimental design is adopted, participation rates before and after implementation of community health champion activities may be compared with those of a comparison community to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community health champions have a significant positive impact on participation in preventive health activities among rural residents in Nigeria. Residents who have regular contact with trained community health champions are expected to demonstrate higher participation in preventive health programmes than residents without comparable community-level support. Community health champions may improve participation by providing understandable health information, addressing misconceptions, encouraging residents to attend screening and health education programmes, reminding households about scheduled preventive services, and linking residents with nearby primary healthcare facilities. Their familiarity with local customs and community structures may also increase trust and facilitate communication between healthcare providers and residents. However, inadequate training, limited incentives, volunteer fatigue, insufficient supervision, weak links with healthcare facilities, inadequate resources, community mistrust, and inconsistent programme support may reduce their effectiveness. The study therefore expects adequately trained, supervised, supported, and community-accepted health champions to contribute significantly to improved participation in preventive health activities among rural residents in Nigeria. The study is expected to contribute to the literature on community health champions, community health volunteers, preventive healthcare participation, health promotion, community mobilization, primary healthcare, rural health, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, local government health authorities, primary healthcare centres, community health workers, community leaders, non-governmental organizations, development partners, and policymakers regarding strategies for strengthening community participation in preventive healthcare. The study will also provide evidence-based recommendations for recruiting and training community health champions, strengthening supervision and referral linkages, providing appropriate incentives and support, involving community leaders in health mobilization, and integrating community health champions into routine primary healthcare and preventive health programmes across rural Nigeria.
