Impact of Community Health Worker Immunization Training on Routine Immunization Knowledge in Nigeria
Abstract
Routine immunization is an essential public health intervention for preventing vaccine-preventable diseases and reducing childhood morbidity and mortality. Community health workers play an important role in supporting routine immunization programmes through community mobilization, health education, identification of eligible children, defaulter tracing, referral, immunization support, and communication with caregivers. Their knowledge of immunization schedules, vaccine eligibility, contraindications, cold-chain requirements, adverse events following immunization, documentation, and communication with caregivers is important for effective programme implementation. However, gaps in training, limited refresher education, inadequate access to updated immunization guidelines, and variations in supervision may affect routine immunization knowledge among community health workers in Nigeria. Community health worker immunization training provides an opportunity to strengthen knowledge, improve understanding of immunization procedures, and support more effective delivery of routine immunization services. Against this background, this study investigates the impact of community health worker immunization training on routine immunization knowledge in Nigeria. The study will be anchored on Social Cognitive Theory, the Competency-Based Education Framework, and the Donabedian Model of Healthcare Quality. Social Cognitive Theory explains how training, observational learning, self-efficacy, reinforcement, and the work environment may influence community health workers' acquisition and application of immunization knowledge. The Competency-Based Education Framework emphasizes the development and assessment of relevant knowledge, skills, attitudes, and competencies required for effective immunization service delivery. The Donabedian Model explains healthcare quality through structure, process, and outcome, with immunization training representing an important input for strengthening service delivery and routine immunization knowledge serving as a measurable outcome. Collectively, these theoretical perspectives provide a suitable framework for explaining how community health worker immunization training may influence routine immunization knowledge in Nigeria. The study will adopt a quantitative quasi-experimental research design. The study population will comprise community health workers involved in routine immunization activities in selected primary healthcare facilities and communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, primary healthcare facilities, and eligible community health workers. Community health worker immunization training will be assessed using indicators such as participation in immunization training programmes, training frequency, duration, content, use of standardized immunization guidelines, practical demonstrations, training materials, refresher training, competency assessment, mentorship, supportive supervision, and access to updated immunization information. Routine immunization knowledge will be assessed using indicators such as knowledge of national immunization schedules, vaccine-preventable diseases, vaccine eligibility, recommended ages and intervals, contraindications and precautions, vaccine handling, cold-chain requirements, injection safety, adverse events following immunization, vaccine wastage prevention, documentation, defaulter tracing, caregiver education, and referral procedures. Data will be collected using structured questionnaires, standardized knowledge assessment tools, training records, supervision reports, immunization registers, primary healthcare facility records, and relevant programme documents. Descriptive statistics will be used to summarize community health workers' demographic and professional characteristics, training exposure, and immunization knowledge levels. Inferential statistical techniques, including paired-sample tests, chi-square tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of immunization training on routine immunization knowledge. Where a quasi-experimental design is adopted, knowledge scores before and after the training intervention may be compared. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community health worker immunization training has a significant positive impact on routine immunization knowledge in Nigeria. Community health workers who receive structured, standardized, and competency-based immunization training are expected to demonstrate higher knowledge scores regarding vaccine schedules, eligibility, vaccine handling, contraindications, adverse events, documentation, and caregiver communication than their pre-training performance. Practical demonstrations, updated guidelines, refresher training, and supportive supervision are expected to strengthen retention and application of immunization knowledge. Improved knowledge may also enhance community health workers' ability to educate caregivers, identify children who are due for vaccination, support appropriate vaccine delivery, recognize adverse events, maintain accurate records, and trace children who miss scheduled vaccinations. However, inadequate training resources, irregular refresher training, staff turnover, limited supervision, poor access to updated guidelines, and weak programme support may reduce the sustainability of knowledge gains. The study therefore expects regular, standardized, practical, and adequately supported immunization training to contribute significantly to improved routine immunization knowledge among community health workers in Nigeria. The study is expected to contribute to the literature on community health worker training, routine immunization, immunization knowledge, vaccine-preventable diseases, primary healthcare, community-based healthcare, health workforce development, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, National Primary Health Care Development Agency's immunization programmes, state ministries of health, local government health authorities, primary healthcare facilities, development partners, immunization programme managers, and policymakers regarding strategies for strengthening community-based immunization services. The study will also provide evidence-based recommendations for expanding standardized immunization training, providing regular refresher courses, strengthening supportive supervision, improving access to updated immunization guidelines, incorporating practical competency assessments, strengthening training documentation, and integrating continuous immunization education into routine capacity-building programmes for community health workers across Nigeria.
