Impact of Community Pharmacy Services on Self-Medication Practices among Adults in Nigeria

 

Abstract

Self-medication is an important public health concern in Nigeria and involves the use of medicines by individuals to treat perceived illnesses or symptoms without appropriate consultation with a qualified healthcare professional. Although community pharmacies can provide accessible sources of medicines and health information, the quality and nature of pharmacy services may influence how adults obtain and use medicines. Appropriate community pharmacy services, including medication counselling, assessment of symptoms, referral to healthcare facilities, advice on appropriate medicine use, dosage guidance, and identification of potentially harmful medicine combinations, may encourage safer medicine-use practices and reduce inappropriate self-medication. However, easy access to medicines, inadequate regulation, limited consultation time, commercial pressures, and variations in pharmacy practice may contribute to inappropriate medicine use. Against this background, this study investigates the impact of community pharmacy services on self-medication practices among adults in Nigeria. The study will be anchored on the Theory of Planned Behavior, Health Belief Model, and Andersen's Behavioral Model of Health Services Use. The Theory of Planned Behavior explains how adults' attitudes toward self-medication, perceived social expectations, perceived behavioural control, and behavioural intentions may influence their medicine-use decisions. The Health Belief Model explains how perceptions of illness severity, perceived benefits of self-treatment, perceived barriers to professional healthcare, and cues to action may influence self-medication behaviour. Andersen's Behavioral Model explains how predisposing, enabling, and need-related factors influence healthcare utilization, including the choice between professional healthcare and self-treatment. Collectively, these theoretical perspectives provide a suitable framework for explaining how community pharmacy services may influence self-medication practices among adults in Nigeria. The study will adopt a quantitative analytical cross-sectional research design. The study population will comprise adults aged 18 years and above who obtain medicines or healthcare-related services from selected community pharmacies across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, community pharmacies, and eligible adult participants. Community pharmacy services will be measured using indicators such as availability of pharmacist consultation, medication counselling, symptom assessment, dosage and administration guidance, medicine-use instructions, identification of drug interactions, adverse-effect counselling, referral to healthcare facilities, follow-up services, provision of health information, and pharmacist involvement in medicine selection. Self-medication practices will be assessed using indicators such as use of medicines without prescription, frequency of self-medication, types of medicines used, use of leftover medicines, use of medicines recommended by friends or relatives, use of previously prescribed medicines for new symptoms, use of antibiotics without prescription, adherence to recommended dosage, duration of self-treatment, reasons for self-medication, and seeking professional advice before medicine use. Data will be collected using structured questionnaires, pharmacy service assessment tools, and relevant pharmacy records where available. Descriptive statistics will be used to summarize participants' demographic characteristics, pharmacy-service experiences, medicine-use patterns, and self-medication practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of community pharmacy services on self-medication practices. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that appropriate community pharmacy services have a significant positive effect on safer self-medication practices among adults in Nigeria. Adults who receive adequate pharmacist counselling, medicine-use guidance, symptom assessment, and referral advice are expected to demonstrate lower levels of inappropriate self-medication and greater adherence to recommended medicine-use practices. Effective pharmacy services may discourage unnecessary antibiotic use, inappropriate dosage, prolonged self-treatment, use of contraindicated medicines, and treatment of potentially serious conditions without professional assessment. Community pharmacists may also serve as an accessible point of contact for early identification of conditions requiring referral to hospitals or other healthcare facilities. However, limited pharmacist availability, inadequate consultation spaces, high customer volume, commercial pressures, poor enforcement of medicine regulations, and unrestricted access to certain medicines may reduce the effectiveness of community pharmacy services. The study therefore expects accessible, professional, patient-centred, and adequately regulated community pharmacy services to contribute significantly to safer medicine-use practices among adults in Nigeria. The study is expected to contribute to the literature on community pharmacy services, self-medication practices, medicine use, pharmaceutical care, pharmacist counselling, responsible medicine use, antibiotic use, healthcare-seeking behaviour, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Pharmacy Council of Nigeria, state ministries of health, community pharmacists, pharmaceutical organizations, healthcare providers, regulatory agencies, public health practitioners, development partners, and policymakers regarding strategies for promoting rational medicine use. The study will also provide evidence-based recommendations for strengthening pharmacist-led counselling, improving referral practices, increasing public awareness of the risks of inappropriate self-medication, enforcing medicine-dispensing regulations, expanding professional pharmacy services, and developing community-based interventions that promote safe and responsible medicine use across Nigeria.
