Impact of Glaucoma Screening Outreach on Glaucoma Detection among Adults Aged 40 Years and Above in Nigeria
Abstract
Glaucoma is a major eye health concern and an important cause of irreversible visual impairment and blindness. The condition may develop gradually without noticeable symptoms during its early stages, making early detection through appropriate eye examination and screening particularly important for adults at increased risk. Adults aged 40 years and above may face increased risk of glaucoma, while limited awareness, financial barriers, geographical distance, shortage of eye-care professionals, and inadequate access to eye health services may contribute to delayed detection. Glaucoma screening outreach provides an opportunity to bring eye health assessment closer to communities and identify adults who may require further comprehensive ophthalmic evaluation. Against this background, this study investigates the impact of glaucoma screening outreach on glaucoma detection among adults aged 40 years and above in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Health Systems Framework. The Health Belief Model explains how adults' perceptions of susceptibility to glaucoma, perceived severity of visual impairment, perceived benefits of screening, perceived barriers, and cues to action may influence participation in glaucoma screening. The Social Ecological Model emphasizes the influence of individual, interpersonal, community, healthcare, and environmental factors on access to and utilization of glaucoma screening services. The Health Systems Framework highlights the importance of service delivery, healthcare workforce, health information, medical equipment and technologies, financing, accessibility, and referral systems in the effective delivery of glaucoma screening programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how glaucoma screening outreach may influence glaucoma detection among adults aged 40 years and above in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adults aged 40 years and above residing in selected urban and rural communities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, households, and eligible adults. Glaucoma screening outreach will be assessed using indicators such as availability and frequency of outreach activities, screening coverage, accessibility of screening sites, availability of trained ophthalmic personnel, availability of appropriate screening equipment, visual acuity assessment, intraocular pressure measurement, optic nerve assessment where feasible, risk assessment, participant education, referral arrangements, and follow-up mechanisms. Glaucoma detection will be assessed using indicators such as abnormal screening findings, elevated intraocular pressure where relevant, suspicious optic nerve findings, visual field abnormalities where assessed, suspected glaucoma cases, referrals for comprehensive ophthalmic examination, and confirmed glaucoma diagnoses following appropriate clinical assessment. Data will be collected using structured questionnaires, standardized eye screening forms, screening registers, ophthalmic examination records, referral registers, and relevant community eye health programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening participation, and glaucoma screening findings. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of glaucoma screening outreach on glaucoma detection. Where a quasi-experimental design is adopted, glaucoma detection rates before and after implementation of screening outreach may be compared to determine changes associated with the programme. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that glaucoma screening outreach has a significant positive impact on glaucoma detection among adults aged 40 years and above in Nigeria. Adults who participate in community-based glaucoma screening are expected to have a greater likelihood of having previously undetected glaucoma or suspicious findings identified than adults without access to screening outreach. Bringing screening services closer to communities may reduce geographical and financial barriers and increase opportunities for early referral and comprehensive eye assessment. Outreach programmes may also improve awareness of glaucoma and encourage adults to seek appropriate eye care even when they have few or no noticeable symptoms. However, inadequate screening equipment, shortage of trained ophthalmic personnel, limited funding, low community awareness, poor referral and follow-up systems, and barriers to accessing confirmatory diagnostic services may reduce the effectiveness of glaucoma screening outreach. The study therefore expects accessible, well-organized, adequately staffed, and properly linked glaucoma screening outreach programmes to contribute significantly to improved detection and early management of glaucoma among adults aged 40 years and above in Nigeria. The study is expected to contribute to the literature on glaucoma screening outreach, glaucoma detection, eye health, visual impairment prevention, community eye health, screening services, early detection, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, ophthalmic hospitals, primary healthcare centres, ophthalmologists, optometrists, ophthalmic nurses, community health workers, eye health organizations, development partners, and policymakers regarding strategies for strengthening glaucoma prevention and early detection. The study will also provide evidence-based recommendations for expanding community glaucoma screening outreach, improving access to appropriate screening equipment and trained eye-care personnel, strengthening referral and follow-up systems, integrating glaucoma risk assessment into primary healthcare and community eye health services, increasing public awareness, and improving access to comprehensive diagnostic and treatment services across Nigeria.
