Impact of Insecticide-Treated Net Distribution Campaigns on Malaria Incidence among Households in Nigeria
Abstract
Malaria remains one of the most important public health challenges in Nigeria and continues to contribute to substantial morbidity, mortality, healthcare utilization, and economic losses, particularly in malaria-endemic communities. Insecticide-treated nets (ITNs) are an important malaria prevention intervention because they provide a physical barrier against mosquito bites while the insecticidal treatment helps reduce contact between humans and malaria-transmitting mosquitoes. Insecticide-treated net distribution campaigns are designed to increase household access to and ownership of ITNs by providing nets through organized mass distribution programmes. However, factors such as inadequate distribution coverage, damaged or unavailable nets, inconsistent use, household sleeping arrangements, inadequate community awareness, and challenges in replacing old nets may reduce the effectiveness of ITN distribution campaigns in preventing malaria. Malaria incidence refers to the occurrence of new malaria cases within a defined population during a specified period. Effective distribution of ITNs to households may increase access to malaria prevention and reduce exposure to malaria-transmitting mosquitoes, thereby contributing to lower malaria incidence. Against this background, this study investigates the impact of insecticide-treated net distribution campaigns on malaria incidence among households in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Health Systems Framework. The Health Belief Model explains how perceived susceptibility to malaria, perceived severity, perceived benefits of ITN use, perceived barriers, and cues to action may influence household adoption and use of insecticide-treated nets. The Social Ecological Model recognizes that health behaviours and malaria prevention are influenced by individual, household, community, institutional, and environmental factors. The Health Systems Framework emphasizes service delivery, health information, health workforce, medical products and technologies, financing, and governance as important components of effective malaria prevention programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how ITN distribution campaigns may influence malaria incidence among households in Nigeria. The study will adopt a quantitative quasi-experimental, retrospective, or cross-sectional analytical research design, depending on the availability of household and malaria surveillance data. The study population will comprise households in selected malaria-endemic communities in Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, and eligible households. ITN distribution campaigns will be assessed using indicators such as campaign coverage, number of nets distributed, household ITN ownership, number of ITNs available per household, duration since distribution, replacement of damaged or expired nets, and access to distributed nets. Malaria incidence will be assessed using indicators such as the number of new malaria cases among household members during a specified period, malaria cases confirmed through diagnostic testing, household malaria incidence rates, and malaria-related healthcare visits. Data will be collected using structured household questionnaires, malaria surveillance records, healthcare facility records, ITN distribution registers, programme reports, and relevant community health records. Descriptive statistics will be used to summarize household characteristics, ITN distribution coverage, ownership, and malaria incidence patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or Poisson regression analysis, will be used to determine the impact of ITN distribution campaigns on malaria incidence. Where appropriate, malaria incidence before and after ITN distribution campaigns may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective insecticide-treated net distribution campaigns have a significant negative impact on malaria incidence among households in Nigeria. Households that receive adequate numbers of ITNs through distribution campaigns are expected to experience fewer malaria cases than households with limited or no access to ITNs. Increased household ownership of ITNs may reduce exposure to malaria-transmitting mosquitoes, particularly when nets are consistently used by household members during sleeping hours. Regular distribution and replacement campaigns are also expected to maintain access to effective nets as older nets become damaged or lose insecticidal effectiveness. Conversely, inadequate campaign coverage, insufficient numbers of nets, delayed replacement, poor household access, and limited awareness of correct net use may reduce the effectiveness of distribution campaigns and contribute to continued malaria transmission. The study therefore expects effective and sustained ITN distribution campaigns to contribute significantly to reducing malaria incidence among households in Nigeria. The study is expected to contribute to the literature on malaria prevention, insecticide-treated nets, malaria incidence, vector control, household health, infectious disease prevention, epidemiology, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Malaria Elimination Programme, state ministries of health, local government health authorities, malaria control programme managers, community health workers, development partners, and policymakers regarding strategies for strengthening malaria prevention programmes. The study will also provide evidence-based recommendations for improving ITN distribution coverage, ensuring adequate household access to nets, strengthening replacement and distribution systems, improving community education on appropriate ITN use, monitoring household ownership and utilization, and integrating ITN distribution campaigns with other malaria control interventions to reduce malaria incidence across Nigeria.
