Impact of Laboratory Quality Management Systems on Diagnostic Accuracy in Nigerian Medical Laboratories
Abstract
Accurate laboratory diagnosis is essential for effective healthcare delivery because laboratory test results provide critical information for disease diagnosis, treatment selection, patient monitoring, and public health decision-making. In Nigeria, challenges such as inadequate laboratory infrastructure, equipment malfunction, inconsistent quality control, shortage of skilled laboratory personnel, poor specimen management, reagent shortages, and weaknesses in laboratory processes may compromise the accuracy and reliability of diagnostic results. Laboratory Quality Management Systems (LQMS) provide a structured approach for improving the quality, consistency, and reliability of laboratory services through standardized procedures, quality control, equipment management, personnel competency assessment, documentation, internal audits, corrective actions, and continuous quality improvement. However, variations in the implementation and sustainability of quality management systems across Nigerian medical laboratories may affect diagnostic performance. Against this background, this study investigates the impact of laboratory quality management systems on diagnostic accuracy in Nigerian medical laboratories. The study will be anchored on the Donabedian Model of Healthcare Quality, Total Quality Management Theory, and Systems Theory. The Donabedian Model explains quality through the relationship between healthcare structure, processes, and outcomes, with laboratory infrastructure and quality-management processes influencing the reliability of diagnostic results. Total Quality Management Theory emphasizes continuous improvement, standardized procedures, staff participation, quality control, customer focus, and organizational commitment to quality. Systems Theory emphasizes the interdependence of laboratory personnel, equipment, reagents, information systems, procedures, management, and external healthcare processes in determining diagnostic performance. Collectively, these theoretical perspectives provide a suitable framework for explaining how laboratory quality management systems may influence diagnostic accuracy in Nigerian medical laboratories. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise selected public and private medical laboratories and laboratory professionals working in Nigeria. A multistage sampling technique will be used to select states, local government areas, medical laboratories, laboratory departments, and eligible laboratory personnel. Laboratory Quality Management Systems will be assessed using indicators such as availability of standard operating procedures, internal quality control, external quality assessment participation, equipment calibration and maintenance, reagent management, personnel competency assessment, specimen collection and handling procedures, documentation, internal audits, corrective and preventive actions, biosafety procedures, quality indicators, and continuous improvement activities. Diagnostic accuracy will be assessed using indicators such as concordance with reference or confirmatory test results, false-positive and false-negative rates, sensitivity, specificity, error rates, proficiency-testing performance, repeat-test rates, and accuracy of selected laboratory tests. Data will be collected using structured questionnaires, laboratory assessment checklists, quality-control records, proficiency-testing reports, equipment maintenance records, laboratory registers, and relevant diagnostic records. Descriptive statistics will be used to summarize laboratory characteristics, quality management practices, and diagnostic performance. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and multiple regression analysis where appropriate, will be used to determine the impact of LQMS on diagnostic accuracy. Where a quasi-experimental design is adopted, diagnostic accuracy indicators before and after implementation or strengthening of a structured quality management system may be compared, while relevant laboratory and operational factors are appropriately controlled. Diagnostic and model-assumption tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective laboratory quality management systems have a significant positive impact on diagnostic accuracy in Nigerian medical laboratories. Laboratories with stronger quality management systems are expected to demonstrate lower diagnostic error rates, improved concordance with reference results, better quality-control performance, and more reliable laboratory results than laboratories with weak or inconsistently implemented quality management systems. Standardized procedures, regular equipment calibration, effective internal quality control, participation in external quality assessment programmes, competent personnel, appropriate specimen management, and effective corrective actions may contribute to improved diagnostic reliability. However, inadequate funding, equipment breakdown, reagent shortages, shortage of trained personnel, weak management commitment, poor documentation, and limited access to external quality assessment programmes may constrain effective implementation. The study therefore expects comprehensive, sustainable, and adequately supported laboratory quality management systems to contribute significantly to improved diagnostic accuracy and laboratory service quality in Nigeria. The study is expected to contribute to the literature on laboratory quality management systems, diagnostic accuracy, laboratory medicine, healthcare quality, patient safety, medical laboratory services, quality assurance, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Medical Laboratory Science Council of Nigeria, National Health Laboratory Service structures, state ministries of health, public and private medical laboratories, laboratory scientists, pathologists, healthcare administrators, accreditation bodies, development partners, and policymakers regarding strategies for strengthening laboratory quality. The study will also provide evidence-based recommendations for expanding laboratory quality management systems, strengthening internal and external quality assessment, improving equipment maintenance and calibration, ensuring adequate supplies of quality reagents, strengthening laboratory personnel competency, improving documentation and auditing, supporting laboratory accreditation, and integrating continuous quality improvement into diagnostic laboratory services across Nigeria.
