Impact of Occupational Skin Screening on Occupational Dermatitis Detection among Industrial Workers in Nigeria

Abstract

Occupational dermatitis remains an important public health concern among industrial workers in Nigeria, particularly among employees who are regularly exposed to chemicals, solvents, detergents, oils, lubricants, metals, dust, heat, moisture, and other workplace irritants and allergens. Repeated occupational exposure to these substances may cause irritant contact dermatitis, allergic contact dermatitis, and other work-related skin conditions. Occupational dermatitis may initially present with symptoms such as itching, redness, dryness, scaling, swelling, or skin irritation that workers may overlook or consider minor. Delayed detection and continued exposure may result in persistent or worsening symptoms, reduced productivity, absenteeism, occupational discomfort, and impaired quality of life. Occupational skin screening provides an opportunity to identify dermatological abnormalities among workers at an early stage through systematic workplace skin assessments and evaluation of occupational exposure history. Early detection may facilitate timely treatment, reduction of exposure, appropriate use of personal protective equipment, workplace modifications, and referral for specialist dermatological assessment. However, inadequate occupational health services, limited availability of trained healthcare personnel, irregular screening schedules, low worker awareness, insufficient screening resources, and weak enforcement of occupational health and safety standards may limit the effectiveness of occupational skin screening programmes in Nigeria. Against this background, this study investigates the impact of occupational skin screening on occupational dermatitis detection among industrial workers in Nigeria. The study will be anchored on Occupational Health Theory, the Health Belief Model, and the Health Systems Framework. Occupational Health Theory emphasizes the prevention, identification, monitoring, and management of workplace hazards and their effects on workers' health. The Health Belief Model explains how workers' perceptions of susceptibility to occupational dermatitis, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in occupational skin screening. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective occupational healthcare programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how occupational skin screening may influence occupational dermatitis detection among industrial workers in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise industrial workers employed in selected manufacturing, construction, mining, chemical, processing, and other industrial facilities across Nigeria. A multistage sampling technique will be used to select states, industrial areas, organizations, factories, departments, and eligible workers. Occupational skin screening will be measured using indicators such as availability of workplace skin screening services, screening coverage, frequency of occupational skin examinations, assessment of occupational exposure history, availability of trained occupational health personnel, availability of screening facilities, workplace hazard assessment, worker education on skin protection, use of personal protective equipment, and referral arrangements. Occupational dermatitis detection will be assessed using indicators such as abnormal skin examination findings, newly identified cases of irritant contact dermatitis, allergic contact dermatitis, occupational eczema, skin irritation, severity of identified dermatitis, confirmation following appropriate clinical assessment, referral for specialist dermatological evaluation, and recommendations for treatment or workplace intervention. Data will be collected using structured questionnaires, occupational skin screening records, dermatological assessment records, workplace health records, occupational exposure reports, referral registers, and relevant organizational documents. Descriptive statistics will be used to summarize workers' characteristics, occupational exposures, screening coverage, and patterns of occupational dermatitis detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of occupational skin screening on occupational dermatitis detection. Where appropriate, dermatitis detection rates before and after implementation of occupational skin screening programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that occupational skin screening has a significant positive impact on occupational dermatitis detection among industrial workers in Nigeria. Industrial workers who participate in regular occupational skin screening are expected to have a higher likelihood of having previously undetected occupational dermatitis identified than workers without access to routine screening. Systematic skin assessment and evaluation of occupational exposure history may facilitate early identification of irritant and allergic contact dermatitis and other work-related skin abnormalities. Early detection may provide opportunities for timely treatment, counselling on skin protection, reduction or elimination of harmful workplace exposures, appropriate use of personal protective equipment, workplace modifications, and referral for specialist dermatological care where necessary. Occupational skin screening may also enable employers to identify workplace conditions that contribute to dermatitis and strengthen preventive occupational health measures. Conversely, inadequate screening coverage, shortages of trained occupational health personnel, limited screening facilities, low worker participation, inadequate awareness, and weak enforcement of occupational health and safety standards may reduce the effectiveness of occupational skin screening. The study therefore expects regular and comprehensive occupational skin screening to contribute significantly to improved detection and prevention of occupational dermatitis among industrial workers in Nigeria. The study is expected to contribute to the literature on occupational skin screening, occupational dermatitis detection, industrial worker health, occupational dermatology, workplace safety, occupational exposure, preventive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Labour and Employment, employers, industrial organizations, occupational health practitioners, dermatologists, workplace safety officers, healthcare providers, trade unions, development partners, and policymakers regarding strategies for strengthening occupational skin health programmes. The study will also provide evidence-based recommendations for expanding periodic occupational skin screening, strengthening workplace hazard assessment, improving training of occupational health personnel, increasing worker awareness of occupational dermatitis and skin hazards, promoting appropriate use of personal protective equipment, improving access to dermatological services, strengthening referral and follow-up systems, and ensuring compliance with occupational health and safety standards to prevent and detect occupational dermatitis among industrial workers across Nigeria.
