Impact of Postpartum Follow-Up Services on Postpartum Care Attendance among Mothers in Nigeria

 

Abstract

Postpartum care is an essential component of maternal and newborn healthcare because the period following childbirth is associated with significant physiological changes and risks of maternal complications. Despite the importance of postnatal care, many mothers in Nigeria do not receive adequate or timely postpartum follow-up, which may limit opportunities for early detection and management of complications such as postpartum haemorrhage, infections, hypertensive disorders, anaemia, breastfeeding difficulties, and other health concerns. Postpartum follow-up services provide an opportunity to maintain contact with mothers after childbirth through scheduled clinic visits, telephone reminders, community-based follow-up, home visits, counselling, and referral services. These services may improve continuity of care and encourage mothers to attend recommended postpartum appointments. However, financial constraints, geographical barriers, transportation difficulties, inadequate staffing, limited awareness, cultural practices, poor communication, and weak follow-up systems may affect postpartum care attendance in Nigeria. Against this background, this study investigates the impact of postpartum follow-up services on postpartum care attendance among mothers in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Health Systems Framework. The Health Belief Model explains how mothers' perceptions of susceptibility to postpartum complications, perceived severity, perceived benefits of follow-up services, perceived barriers, and cues to action may influence their attendance at postpartum care services. The Social Ecological Model emphasizes the influence of individual, household, community, healthcare, cultural, and socioeconomic factors on postpartum healthcare utilization. The Health Systems Framework emphasizes service delivery, health workforce, health information systems, transportation, financing, governance, and accessibility as essential components of effective postpartum follow-up services. Collectively, these theoretical perspectives provide a suitable framework for explaining how postpartum follow-up services may influence postpartum care attendance among mothers in Nigeria. The study will adopt a quantitative analytical cohort or quasi-experimental research design. The study population will comprise mothers who have recently delivered and are receiving maternity or postnatal services in selected healthcare facilities across Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, postnatal clinics, and eligible mothers. Postpartum follow-up services will be assessed using indicators such as availability of scheduled follow-up appointments, telephone or SMS reminders, community health worker follow-up, postnatal home visits, counselling, appointment tracking, referral support, communication with mothers, timing of follow-up contacts, and continuity of care. Postpartum care attendance will be assessed using indicators such as attendance at the recommended postnatal visits, timing of the first postpartum visit, number of postpartum visits attended, completion of scheduled follow-up appointments, and adherence to facility-recommended postpartum care schedules. Data will be collected using structured questionnaires, postnatal care registers, appointment records, follow-up registers, electronic or telephone reminder records where available, maternal medical records, and community health programme documents. Descriptive statistics will be used to summarize maternal characteristics, exposure to postpartum follow-up services, and patterns of postpartum care attendance. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of postpartum follow-up services on postpartum care attendance. Where a quasi-experimental design is adopted, postpartum care attendance among mothers receiving structured follow-up services may be compared with attendance among mothers receiving routine postpartum services without structured follow-up. Relevant maternal, socioeconomic, geographical, and healthcare-related factors will be appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that postpartum follow-up services have a significant positive impact on postpartum care attendance among mothers in Nigeria. Mothers who receive timely reminders, community follow-up, home visits, counselling, appointment tracking, and other structured follow-up support are expected to have a higher likelihood of attending recommended postpartum care visits than mothers without access to such services. Regular follow-up may improve awareness of the importance of postpartum care, reduce missed appointments, strengthen communication between mothers and healthcare providers, and facilitate early identification of mothers who require additional support or referral. However, financial constraints, transportation difficulties, distance to healthcare facilities, competing household responsibilities, cultural beliefs, inadequate staffing, poor communication systems, and limited availability of postpartum services may reduce the effectiveness of follow-up interventions. The study therefore expects accessible, well-coordinated, and adequately supported postpartum follow-up services to contribute significantly to improved postpartum care attendance in Nigeria. The study is expected to contribute to the literature on postpartum follow-up services, postpartum care attendance, postnatal care, maternal healthcare utilization, continuity of care, maternal health, community-based healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, healthcare facilities, maternal and child health programmes, obstetricians, midwives, nurses, community health workers, development partners, and policymakers regarding strategies for improving postpartum service utilization. The study will also provide evidence-based recommendations for strengthening postpartum appointment tracking, expanding SMS and telephone reminder systems, increasing community health worker follow-up, improving postnatal home visits, strengthening referral and communication systems, addressing transportation and financial barriers, and integrating structured postpartum follow-up into broader maternal and newborn health programmes across Nigeria.
