Impact of Public Health Education on Cholera Prevention Practices among Residents of Flood-Prone Communities in Nigeria

 

Abstract

Cholera remains an important public health concern in Nigeria, particularly in communities affected by flooding, inadequate water supply, poor sanitation, and limited access to essential hygiene facilities. The disease is an acute diarrhoeal infection primarily associated with the consumption of food or water contaminated with Vibrio cholerae. Flood-prone communities may face increased risks of cholera transmission because flooding can contaminate drinking-water sources, damage sanitation infrastructure, increase exposure to contaminated environments, and disrupt access to healthcare and public health services. Public health education is an important strategy for improving community knowledge and encouraging preventive behaviours such as drinking treated water, proper handwashing, safe food handling, appropriate disposal of human waste, environmental sanitation, and early healthcare-seeking when symptoms occur. In Nigeria, however, inadequate awareness, misconceptions about cholera transmission, poor sanitation facilities, unsafe water sources, and limited access to reliable health information may reduce the adoption of appropriate preventive practices. Public health education can provide communities with accurate information about cholera causes, modes of transmission, symptoms, prevention, and appropriate responses during outbreaks. Against this background, this study investigates the impact of public health education on cholera prevention practices among residents of flood-prone communities in Nigeria. The study is anchored on the Health Belief Model (HBM), Social Cognitive Theory (SCT), and Social Ecological Model. The Health Belief Model explains preventive health behaviour through individuals' perceptions of susceptibility to disease, perceived severity, perceived benefits of preventive actions, perceived barriers, and cues to action. Public health education can serve as an important cue to action by increasing awareness of cholera risks and encouraging residents to adopt appropriate preventive behaviours. Social Cognitive Theory emphasizes the interaction between knowledge, behavioural capability, personal beliefs, social influences, and environmental conditions in shaping health behaviours. The Social Ecological Model recognizes that cholera prevention is influenced by individual, household, community, environmental, and institutional factors. Collectively, these theoretical perspectives provide a comprehensive framework for explaining how public health education may influence cholera prevention practices among residents of flood-prone communities in Nigeria. The study adopts a quantitative quasi-experimental research design. Primary data will be collected from adult residents of selected flood-prone communities in Nigeria using a structured interviewer-administered questionnaire. A multistage sampling technique will be employed to select states, local government areas, flood-prone communities, households, and eligible respondents. Where feasible, selected communities will be assigned to intervention and comparison groups. Residents in the intervention group will receive a structured public health education programme covering cholera transmission, safe water practices, hand hygiene, food safety, sanitation, environmental hygiene, early recognition of cholera symptoms, and appropriate healthcare-seeking behaviour. Cholera prevention practices will be measured using indicators such as treatment and safe storage of drinking water, handwashing practices, food hygiene, proper disposal of human waste, environmental sanitation, protection of food and water from contamination, and appropriate responses to suspected cholera symptoms. Data will be collected before and after the educational intervention to assess changes in preventive practices. Descriptive statistics will be used to summarize respondents' sociodemographic characteristics, baseline knowledge, exposure to public health education, and prevention practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression analysis, will be employed to determine the impact of public health education on cholera prevention practices. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, and the robustness of the findings. The study anticipates that public health education will have a significant positive impact on cholera prevention practices among residents of flood-prone communities in Nigeria. Residents exposed to structured and accurate health education are expected to demonstrate improved knowledge of cholera transmission and increased adoption of appropriate preventive practices. Public health education may encourage residents to treat drinking water, wash their hands appropriately, maintain environmental sanitation, handle food safely, protect water sources from contamination, and seek prompt healthcare when symptoms occur. However, health education alone may not be sufficient to ensure sustained preventive practices when residents lack access to safe water, functional sanitation facilities, waste disposal systems, healthcare services, and other essential resources. Flooding may further disrupt these resources and make it difficult for residents to implement recommended behaviours. Consequently, the effectiveness of public health education is expected to be greater when educational interventions are accompanied by improved water, sanitation, and hygiene infrastructure and effective community-level outbreak preparedness. This study is expected to contribute to the literature on cholera prevention, health education, environmental health, water sanitation and hygiene, and public health in Nigeria by providing empirical evidence on the relationship between public health education and cholera prevention practices in flood-prone communities. The findings will provide useful information for the Federal Ministry of Health and Social Welfare, Nigeria Centre for Disease Control and Prevention (NCDC), National Primary Health Care Development Agency (NPHCDA), National Emergency Management Agency (NEMA), state ministries of health, local government health authorities, environmental health departments, community health workers, humanitarian organizations, development partners, and policymakers regarding strategies for preventing cholera outbreaks. The study will also provide evidence-based recommendations for strengthening community health education, improving risk communication during floods, promoting safe water and sanitation practices, increasing access to hygiene facilities, strengthening community-based surveillance, and integrating public health education into flood preparedness and cholera outbreak-response programmes across Nigeria.
