Impact of Public Health Emergency Drills on Emergency Preparedness among Healthcare Workers in Nigeria
Abstract
Public health emergencies, including infectious disease outbreaks, disease epidemics, natural disasters, mass-casualty incidents, and other health-related emergencies, can place substantial pressure on healthcare systems and require healthcare workers to respond rapidly and effectively. Emergency preparedness among healthcare workers is therefore essential for ensuring timely coordination, communication, infection prevention and control, triage, resource mobilization, patient management, and continuity of essential health services during emergencies. Public health emergency drills provide a practical approach to strengthening preparedness by allowing healthcare workers to rehearse emergency response procedures, identify operational gaps, test communication systems, clarify roles and responsibilities, and improve coordination before an actual emergency occurs. However, irregular drills, inadequate resources, limited participation, insufficient feedback, and weak implementation of lessons identified during exercises may reduce their effectiveness. Against this background, this study investigates the impact of public health emergency drills on emergency preparedness among healthcare workers in Nigeria. The study will be anchored on the Social Cognitive Theory, Experiential Learning Theory, and Health Systems Framework. Social Cognitive Theory explains how observational learning, behavioural rehearsal, self-efficacy, feedback, and environmental factors influence healthcare workers' preparedness and response behaviour. Experiential Learning Theory emphasizes learning through concrete experience, reflection, conceptualization, and active experimentation, making emergency drills an important mechanism for developing practical preparedness skills. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, leadership, and governance as essential components of an effective emergency response system. Collectively, these theoretical perspectives provide a suitable framework for explaining how public health emergency drills may influence emergency preparedness among healthcare workers in Nigeria. The study will adopt a quantitative quasi-experimental research design. The study population will comprise healthcare workers, including doctors, nurses, midwives, laboratory personnel, pharmacists, community health workers, emergency medical personnel, and other eligible healthcare staff working in selected public and private healthcare facilities in Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, departments, and eligible healthcare workers. Public health emergency drills will be assessed using indicators such as frequency of drills, type of emergency scenario simulated, duration of exercises, healthcare worker participation, simulation realism, coverage of emergency response procedures, triage exercises, infection prevention and control simulations, communication and coordination exercises, evacuation procedures, emergency resource mobilization, role clarity, post-drill debriefing, feedback, and implementation of corrective actions. Emergency preparedness will be assessed using indicators such as knowledge of emergency response procedures, awareness of roles and responsibilities, emergency communication skills, triage competence, infection prevention and control preparedness, availability and use of emergency response plans, confidence in responding to emergencies, coordination and teamwork, resource mobilization, referral procedures, and ability to maintain essential healthcare services during emergencies. Data will be collected using structured questionnaires, emergency preparedness assessment tools, knowledge tests, drill observation checklists, facility emergency plans, drill reports, attendance records, and post-drill evaluation documents. Descriptive statistics will be used to summarize healthcare workers' characteristics, drill participation, preparedness levels, and facility preparedness indicators. Inferential statistical techniques, including chi-square tests, paired-sample tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of public health emergency drills on emergency preparedness. In the quasi-experimental component, emergency preparedness before and after participation in emergency drills will be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that participation in public health emergency drills has a significant positive impact on emergency preparedness among healthcare workers in Nigeria. Healthcare workers who participate in regular and well-designed emergency drills are expected to demonstrate improved knowledge, confidence, role clarity, communication, teamwork, triage competence, infection prevention preparedness, and familiarity with emergency response procedures compared with their preparedness before the drills. Practical simulation may allow healthcare workers to identify weaknesses in existing emergency plans and develop greater confidence in responding to actual emergencies. Post-drill debriefing and corrective action may further strengthen preparedness by addressing identified gaps in communication, coordination, equipment, staffing, and response procedures. However, irregular exercise schedules, limited funding, inadequate simulation resources, staff shortages, low participation, poor documentation, and failure to implement lessons from previous drills may reduce the effectiveness of emergency exercises. The study therefore expects regular, realistic, adequately resourced, and systematically evaluated public health emergency drills to contribute significantly to improved emergency preparedness among healthcare workers in Nigeria. The study is expected to contribute to the literature on public health emergency preparedness, emergency drills, healthcare worker preparedness, disaster preparedness, outbreak response, health emergency management, healthcare systems, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Nigeria Centre for Disease Control and Prevention, National Primary Health Care Development Agency, state ministries of health, healthcare facility administrators, emergency management agencies, infection prevention and control committees, healthcare professional bodies, development partners, and policymakers regarding strategies for strengthening emergency preparedness. The study will also provide evidence-based recommendations for institutionalizing regular public health emergency drills, improving simulation-based training, strengthening post-drill evaluation and feedback, ensuring implementation of corrective actions, improving emergency communication systems, strengthening healthcare worker capacity, maintaining functional emergency response plans, and integrating emergency drills into routine healthcare facility preparedness and national public health emergency response programmes across Nigeria.
