Impact of School-Based Dental Screening on Dental Caries Detection among Secondary School Students in Nigeria
Abstract
Dental caries remains a major oral health concern among secondary school students in Nigeria and may negatively affect students' comfort, nutrition, concentration, school attendance, academic participation, and overall quality of life. Dental caries develops when acids produced by bacteria in dental plaque progressively damage tooth structures, with risk influenced by factors such as frequent consumption of sugary foods and beverages, inadequate oral hygiene, limited access to preventive dental services, insufficient fluoride exposure, and delayed treatment of early dental problems. Dental caries may remain undetected during its early stages because students may experience minimal or no symptoms until the condition becomes more advanced. Delayed detection may result in toothache, infection, tooth loss, difficulty eating, school absenteeism, and the need for more complex and costly treatment. School-based dental screening provides an opportunity to identify dental caries and other oral health problems among students within the school environment through systematic oral examinations and referral for appropriate dental care. Early detection may facilitate preventive measures, professional dental treatment, oral health education, and regular follow-up. However, limited availability of school dental programmes, shortages of dental professionals, inadequate screening equipment, low awareness, financial barriers, and weak referral and follow-up systems may limit the effectiveness of school-based dental screening in Nigeria. Against this background, this study investigates the impact of school-based dental screening on dental caries detection among secondary school students in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and the Health Systems Framework. The Health Belief Model explains how students' and parents' perceptions of susceptibility to dental caries, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in school-based dental screening. The Social Ecological Model emphasizes the influence of individual, family, school, community, and broader environmental factors on students' oral health behaviours and access to dental services. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective school-based oral healthcare programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how school-based dental screening may influence dental caries detection among secondary school students in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise students enrolled in selected public and private secondary schools across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, schools, classes, and eligible students. School-based dental screening will be measured using indicators such as availability of school dental screening programmes, screening coverage, frequency of oral health examinations, availability of trained dentists or oral health personnel, availability of dental screening equipment, oral health education, student participation, parental consent, and referral arrangements. Dental caries detection will be assessed using indicators such as presence of dental caries, number of affected teeth, severity of caries, untreated dental caries, newly identified cases, dental examination findings, caries experience where applicable, referrals for further dental assessment, and linkage to appropriate dental treatment. Data will be collected using structured questionnaires, dental screening records, oral examination forms, school health records, referral registers, and relevant programme documents. Descriptive statistics will be used to summarize students' characteristics, screening coverage, oral health findings, and patterns of dental caries detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of school-based dental screening on dental caries detection. Where appropriate, dental caries detection rates before and after implementation of school-based dental screening programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that school-based dental screening has a significant positive impact on dental caries detection among secondary school students in Nigeria. Students who participate in school-based dental screening are expected to have a higher likelihood of having previously undetected dental caries identified than students without access to routine screening. Systematic oral examination within schools may facilitate early identification of dental caries and provide opportunities for preventive dental care, oral hygiene education, dietary counselling, fluoride-related interventions where appropriate, restorative treatment, and referral to dental facilities. Conducting screening within schools may reduce geographical, financial, and time-related barriers and increase students' awareness of the importance of maintaining good oral health. Early detection may reduce the progression of dental caries and prevent complications such as severe pain, infection, tooth loss, and school absenteeism. Conversely, inadequate screening coverage, shortages of dental professionals, limited screening equipment, low parental and student awareness, financial constraints, and weak referral and follow-up systems may reduce the effectiveness of school-based dental screening. The study therefore expects accessible and well-organized school-based dental screening to contribute significantly to improved detection and early management of dental caries among secondary school students in Nigeria. The study is expected to contribute to the literature on school-based dental screening, dental caries detection, adolescent oral health, school health services, preventive dentistry, oral health education, community health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, National Primary Health Care Development Agency, state ministries of health and education, secondary schools, dentists, dental therapists, oral health professionals, community health workers, development partners, and policymakers regarding strategies for improving oral health among secondary school students. The study will also provide evidence-based recommendations for expanding school-based dental screening programmes, improving access to oral health examinations and preventive dental services, strengthening oral health education, increasing availability of trained dental personnel and screening equipment, improving referral and follow-up systems, promoting healthy dietary and oral hygiene practices, and integrating dental screening into comprehensive school health programmes across Nigeria.
