Impact of School Dental Health Education on Oral Hygiene Practices among Secondary School Students in Nigeria

Abstract

Oral health is an important component of adolescent health and wellbeing, as poor oral hygiene may contribute to dental caries, gingivitis, periodontal disease, toothache, bad breath, and other oral health problems that may affect students' comfort, concentration, school attendance, and quality of life. Secondary school students may be particularly vulnerable to poor oral hygiene because of dietary habits, inadequate tooth-brushing practices, limited access to dental services, peer influence, and insufficient knowledge of preventive oral healthcare. School dental health education provides an opportunity to deliver oral health information and practical guidance within the school environment, where students can be reached systematically. Against this background, this study investigates the impact of school dental health education on oral hygiene practices among secondary school students in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Theory of Planned Behavior. The Health Belief Model explains how students' perceptions of susceptibility to oral diseases, perceived severity, perceived benefits of good oral hygiene, perceived barriers, and cues to action may influence their oral hygiene practices. Social Cognitive Theory emphasizes observational learning, behavioural modelling, reinforcement, self-efficacy, and environmental influences in the development of healthy oral hygiene behaviours. The Theory of Planned Behavior explains how attitudes, subjective norms, and perceived behavioural control influence students' intentions and subsequent oral hygiene practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how school dental health education may influence oral hygiene practices among secondary school students in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise students enrolled in selected public and private secondary schools across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, schools, classes, and eligible students. School dental health education will be assessed using indicators such as frequency and duration of education sessions, information on tooth-brushing techniques, frequency of brushing, appropriate use of fluoride toothpaste, dietary advice, dental flossing or other interdental cleaning practices, recognition of oral disease warning signs, importance of routine dental visits, avoidance of harmful oral habits, and availability of practical demonstrations. Oral hygiene practices will be assessed using indicators such as frequency and technique of tooth brushing, use of fluoride toothpaste, replacement of toothbrushes, interdental cleaning, oral rinsing practices, dietary habits related to oral health, dental check-up attendance, and timely reporting of oral health problems. Where appropriate, an oral hygiene assessment or validated oral hygiene index may also be used to provide an objective measure of students' oral hygiene status. Data will be collected using structured questionnaires, oral hygiene practice checklists, school health records, dental screening records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to dental health education, and oral hygiene practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of school dental health education on oral hygiene practices. Where a quasi-experimental design is adopted, oral hygiene practice scores before and after the educational intervention may be compared to determine changes associated with the programme. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that school dental health education has a significant positive impact on oral hygiene practices among secondary school students in Nigeria. Students exposed to structured and practical dental health education are expected to demonstrate better oral hygiene practices than students without regular exposure to school-based dental education. Education may improve students' knowledge of appropriate tooth-brushing techniques, fluoride toothpaste use, dietary practices, interdental cleaning, dental check-up attendance, and early recognition of oral health problems. Practical demonstrations and repeated health messages may strengthen students' confidence and motivation to maintain appropriate oral hygiene. However, factors such as parental influence, household income, availability and affordability of oral hygiene products, school resources, cultural practices, dietary habits, peer influence, and access to dental services may affect students' ability to maintain recommended practices. The study therefore expects regular, practical, age-appropriate, and culturally relevant school dental health education to contribute significantly to improved oral hygiene practices among secondary school students in Nigeria. The study is expected to contribute to the literature on school dental health education, oral hygiene practices, adolescent oral health, preventive dentistry, school health services, dental health promotion, oral disease prevention, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, state ministries of health and education, schools, dentists, dental therapists, dental hygienists, school health personnel, parents, development partners, and policymakers regarding strategies for improving oral health among adolescents. The study will also provide evidence-based recommendations for integrating structured dental health education into school health programmes, strengthening practical tooth-brushing demonstrations, improving students' access to oral health information and preventive dental services, promoting parental involvement in adolescent oral health, and establishing regular school-based oral health promotion and screening programmes across Nigeria.
