Influence of Community-Based Occupational Health Assessment on Occupational Health Problems Detection among Informal-Sector Workers in Nigeria

Abstract

Occupational health problems are an important public health concern among informal-sector workers in Nigeria, who constitute a substantial part of the workforce and often operate outside formal occupational health and safety systems. Informal-sector workers such as market traders, artisans, mechanics, tailors, welders, hairdressers, transport workers, small-scale processors, and other self-employed workers may be exposed to physical, chemical, biological, ergonomic, environmental, and psychosocial hazards. These exposures may contribute to musculoskeletal disorders, respiratory problems, hearing impairment, skin conditions, occupational injuries, visual problems, chemical exposure effects, and other work-related health conditions. However, many occupational health problems may remain undetected because informal-sector workers often have limited access to occupational health services, irregular employment arrangements, inadequate workplace safety monitoring, financial constraints, limited awareness, and poor access to preventive healthcare. Community-based occupational health assessment provides an opportunity to bring occupational health services closer to informal-sector workers through community outreach, markets, artisan clusters, primary healthcare centres, mobile health services, and other accessible platforms. Early detection may facilitate appropriate clinical evaluation, health education, exposure reduction, treatment, referral, and preventive interventions. Against this background, this study investigates the influence of community-based occupational health assessment on occupational health problems detection among informal-sector workers in Nigeria. The study will be anchored on Occupational Health Theory, the Health Belief Model, and the Social Ecological Model. Occupational Health Theory emphasizes systematic identification, assessment, prevention, and control of workplace hazards and occupational health problems. The Health Belief Model explains how informal-sector workers' perceptions of susceptibility to occupational health problems, perceived severity, perceived benefits of occupational health assessment, perceived barriers, and cues to action may influence participation in community-based occupational health services. The Social Ecological Model emphasizes the influence of individual, interpersonal, workplace, community, socioeconomic, environmental, and regulatory factors on occupational exposures, health-seeking behaviour, and access to occupational health services. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based occupational health assessment may influence occupational health problem detection among informal-sector workers in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise informal-sector workers aged 18 years and above operating in selected markets, artisan clusters, workshops, transport locations, commercial centres, and other informal workplaces across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, informal-sector clusters, workplaces, and eligible workers. Community-based occupational health assessment will be measured using indicators such as availability of assessment services, assessment coverage, frequency of community occupational health activities, occupational history assessment, physical health assessment, musculoskeletal assessment, respiratory assessment, hearing assessment, vision assessment, skin health assessment, ergonomic assessment, occupational exposure assessment, mental health assessment, personal protective equipment assessment, health education, availability of trained occupational health personnel, and referral arrangements. Occupational health problem detection will be assessed using indicators such as newly identified musculoskeletal problems, respiratory conditions, hearing impairment, visual problems, occupational skin conditions, work-related injuries, ergonomic problems, chemical exposure-related symptoms, psychosocial health problems, workers requiring further clinical evaluation, newly detected occupational health conditions, referrals for healthcare assessment, and confirmed occupational health conditions where appropriate. Data will be collected using structured questionnaires, occupational health assessment forms, validated screening instruments, workplace observation checklists, community health records, healthcare facility records, and referral registers. Descriptive statistics will be used to summarize participants' characteristics, occupational activities, exposure patterns, assessment coverage, identified health problems, and patterns of occupational health problem detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the influence of community-based occupational health assessment on occupational health problem detection. Where appropriate, detection rates before and after implementation of community-based occupational health assessment activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based occupational health assessment has a significant positive influence on occupational health problem detection among informal-sector workers in Nigeria. Workers who participate in community-based occupational health assessment are expected to have a higher likelihood of previously unrecognized occupational health problems being identified than workers without access to such services. Community-based assessment may facilitate early recognition of musculoskeletal disorders, respiratory problems, hearing impairment, visual difficulties, occupational skin conditions, work-related injuries, ergonomic problems, chemical exposure-related symptoms, and other work-related health conditions. Early detection may provide opportunities for appropriate treatment, workplace modifications, exposure reduction, health education, use of personal protective equipment, rehabilitation, and timely referral. Bringing occupational health assessment closer to informal-sector workers may reduce geographical, financial, and time-related barriers and reach workers who are rarely covered by conventional workplace occupational health programmes. Community-based health education may also improve workers' awareness of occupational hazards and encourage safer work practices and appropriate healthcare-seeking behaviour. Conversely, inadequate assessment coverage, shortage of trained occupational health personnel, financial constraints, low awareness, limited protective equipment, irregular work arrangements, self-medication, and weak referral and follow-up systems may reduce the effectiveness of community-based occupational health assessment. The study therefore expects accessible and systematically implemented community-based occupational health assessment to contribute significantly to improved detection and early management of occupational health problems among informal-sector workers in Nigeria. The study is expected to contribute to the literature on community-based occupational health assessment, occupational health problem detection, informal-sector workers' health, occupational disease prevention, workplace safety, environmental health, preventive occupational healthcare, community health services, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Labour and Employment, Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, occupational health and safety authorities, local government councils, primary healthcare centres, community health workers, occupational health practitioners, labour organizations, informal-sector associations, development partners, and policymakers regarding strategies for improving occupational health among informal-sector workers. The study will also provide evidence-based recommendations for expanding community-based occupational health assessment programmes, strengthening occupational health services at primary healthcare level, increasing access to trained occupational health personnel, improving occupational health education, promoting appropriate use of personal protective equipment, strengthening referral and follow-up systems, improving workplace hazard monitoring, reducing financial and geographical barriers to occupational healthcare, and integrating occupational health assessment into appropriate community-based primary healthcare programmes across Nigeria.
