Influence of Roadside Emergency Medical Services on Emergency Care Utilization among Road Traffic Accident Victims in Nigeria

Abstract

Road traffic accidents remain a major public health and emergency healthcare challenge in Nigeria, contributing to substantial morbidity, mortality, disability, and economic losses among affected individuals and their families. Victims of road traffic accidents may experience severe injuries such as fractures, head injuries, spinal injuries, haemorrhage, and other life-threatening conditions that require immediate medical attention. The period between the occurrence of an accident and the arrival of a victim at an appropriate healthcare facility is critical because delays in receiving emergency care may increase the risk of complications, permanent disability, and death. Roadside emergency medical services provide immediate first aid, injury assessment, stabilization, basic life support, and referral or transportation of accident victims to appropriate healthcare facilities. These services may help reduce delays in accessing emergency care and improve the likelihood that injured persons receive appropriate medical attention promptly. However, limited availability of emergency response personnel, inadequate ambulances and equipment, poor road infrastructure, traffic congestion, limited public awareness, weak communication systems, and geographical barriers may affect the accessibility and utilization of roadside emergency medical services in Nigeria. Against this background, this study investigates the influence of roadside emergency medical services on emergency care utilization among road traffic accident victims in Nigeria. The study will be anchored on the Andersen Behavioral Model of Health Services Use, the Health Systems Framework, and the Theory of Emergency Medical Services. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for understanding how access to roadside emergency services may influence victims' use of emergency healthcare. The Health Systems Framework emphasizes service delivery, health workforce, medical products and technologies, health information, financing, and governance as essential components of an effective emergency healthcare system. The Theory of Emergency Medical Services emphasizes rapid response, prehospital assessment, stabilization, transportation, and timely access to appropriate emergency facilities as essential components of effective emergency care. Collectively, these theoretical perspectives provide a suitable framework for explaining how roadside emergency medical services may influence emergency care utilization among road traffic accident victims in Nigeria. The study will adopt a quantitative cross-sectional analytical or retrospective research design. The study population will comprise road traffic accident victims who receive or require emergency medical care following road traffic accidents in selected locations across Nigeria. A multistage sampling technique will be used to select states, local government areas, major roads, emergency response points, hospitals, and eligible accident victims or relevant medical records. Roadside emergency medical services will be measured using indicators such as availability of emergency response services, response time, availability of trained emergency personnel, provision of first aid, basic life support, injury stabilization, emergency transportation, ambulance availability, communication systems, and referral arrangements. Emergency care utilization will be assessed using indicators such as use of emergency departments, time taken to access emergency healthcare, arrival at appropriate healthcare facilities, use of ambulance transportation, receipt of emergency treatment, referral completion, and utilization of appropriate emergency medical services. Data will be collected using structured questionnaires, emergency medical service records, ambulance records, hospital emergency department registers, accident records, medical records, and relevant programme documents. Descriptive statistics will be used to summarize victims' characteristics, roadside emergency service availability, and patterns of emergency care utilization. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the influence of roadside emergency medical services on emergency care utilization. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective roadside emergency medical services have a significant positive influence on emergency care utilization among road traffic accident victims in Nigeria. Accident victims who receive prompt roadside emergency assistance are expected to have a higher likelihood of reaching appropriate healthcare facilities and utilizing emergency medical services than victims without access to timely roadside assistance. Immediate first aid, stabilization, and emergency transportation may reduce delays between injury and treatment and increase the likelihood of receiving appropriate emergency care. Roadside emergency medical services may also improve referral processes by ensuring that seriously injured victims are transported to healthcare facilities capable of providing the required level of emergency treatment. Conversely, long response times, inadequate ambulance availability, shortages of trained personnel, poor communication systems, traffic congestion, inadequate equipment, and limited coverage of emergency services may reduce emergency care utilization and contribute to delays in treatment. The study therefore expects accessible, timely, and adequately equipped roadside emergency medical services to contribute significantly to improved utilization of emergency healthcare among road traffic accident victims in Nigeria. The study is expected to contribute to the literature on roadside emergency medical services, emergency care utilization, road traffic injuries, prehospital care, emergency healthcare, injury prevention, trauma care, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Emergency Medical Treatment Committee, National Emergency Management Agency, state ministries of health, emergency medical service providers, teaching hospitals, general hospitals, road safety authorities, local government health authorities, healthcare professionals, development partners, and policymakers regarding strategies for strengthening emergency medical care. The study will also provide evidence-based recommendations for expanding roadside emergency medical service coverage, reducing emergency response times, increasing the availability of ambulances and emergency equipment, strengthening training of emergency medical personnel, improving emergency communication systems, establishing effective referral networks, improving coordination between emergency responders and healthcare facilities, and integrating roadside emergency services into Nigeria's broader emergency healthcare and road safety systems.
